The article provides information about the XIX World Congress of Psychiatry (August 21-24, 2019, Lisbon, Portugal). The issues of implementing the new ICD-11 diagnostic principles are considered; the main results of changes and modifications in schizophrenic disorders, mood disorders and new diagnostic categories in the new concept of “obsessive-compulsive disorders (OCD) and obsessive-compulsive related disorders” are presented.The clinical utility of ICD-11 consists in the ability to use the diagnostic criteria more accurately than before. The talks of the keynote congress speakers are highlighted, the bottom line in the speeches of experts from many countries is finding effective ways of improving the mental health of a person. В статье представлена информация о XIX всемирном конгрессе по психиатрии (21–24 августа 2019 г.,г. Лиссабон, Португалия). Рассмотрены вопросы внедрения новых принципов диагностики МКБ-11,представлены основные результаты изменений и модификаций в рамках расстройств шизофренического спектра, расстройств настроения и новых диагностических категорий в объединенном новомпонятии обсессивно-комульсивные расстройства и связанные расстройства. ПОСЛЕРОДОВАЯ ДЕПРЕССИЯ – ЦЕНТРАЛЬНАЯ ПРОБЛЕМА ОХРАНЫ ПСИХИЧЕСКОГО ЗДОРОВЬЯ РАННЕГО МАТЕРИНСТВА

Perinatal mental health of the mother occupies a central position in healthcare, the same as caring for a newborn baby. Maternal depression is the lead pathology of postpartum period. Despite the high prevalence and importance to the overall development of children, most cases of depression are unappreciated, unrecognized and untreated. Inattention to issues which are related to physical and mental health of women, leads to severe negative social and economic consequences for society. Statistical comparison of the total number of pregnant women, the number of births in the United States in 2007, excluding cases of fetal loss, including still-births and miscarriages with similar Russian data allowed to calculate the approximate prevalence of postpartum depression in the same period. Presumable probability of postpartum depression is 15% in different regions of the world, and the diagnostic criteria of DSM-IV for postpartum depression are representative for comparison. Considering these assumptions, according to 2007 each year about 260,000 out of 1 mln. 600,000 of Russian women in childbirth may suffer from post-natal depression. The results have a probabilistic character. For the mothers the consequences of unrecognized and untreated depression after childbirth are the most frequent. It should be seen as a depressive episode or recurrent depressive disorder which has a singular coloring due to the appearance in the peak period of reproductive age of women. Postpartum depression may increase the possibility of occurrence of relapse of a depressive disorder as well as the transition to a state of chronic depression. Alcoholism or drug use is a common complication of depression; the risk of suicide increases, especially during the time of reducing the severity of the most severe symptoms of depression, though it is below the prevalence of suicide among the population. Particular attention should be paid to such symptoms as instability in mood and irritability as during the postpartum depression and in its residual period. They can cause child abuse. This paper also presents current data on the epidemiology, etiology, risk factors for postpartum depression, its clinical manifestations, the influence of untreated maternal depression on child development, therapy and educational modules to spread multidisciplinary and inter-agency approach in perinatal mental health problems.В перинаталогии психическое здоровье матери занимает такое же важное положение, как и уход за новорожденным ребенком. Несмотря на высокую распространенность и значение для общего развития детей, депрессии в большинстве случаев остаются непонятыми, нераспознанными и нелеченными.  Невнимание к проблемам, касающимся физического и психического здоровья женщин в репродуктивном периоде, влечет за собой серьезные негативные социальные и экономические последствия для всего общества. Статистическое сопоставление общего количества беременных, числа новорожденных в США в 2007 г. без учета случаев потери плода с аналогичными российскими данными, позволило рассчитать приблизительную распространенность послеродовых депрессий за тот же период. Предположительно вероятностное количество послеродовых депрессий составляет 15% в разных регионах мира, а диагностические критерии DSM-IV послеродовой депрессии репрезентативны для сравнения. При этих допущениях ежегодно в среднем около 260 тыс. из 1,6 млн российских рожениц по данным 2007 г. могут страдать послеродовой депрессией. Для матерей наиболее частыми последствиями после родов являются нераспознанные и нелеченые депрессии, которые следует рассматривать в спектре аффективных расстройств в пиковый период репродуктивного периода женщин. Послеродовые депрессии могут увеличивать возможность наступления рецидивов депрессивного расстройства, а также переходов в хронические состояния. Употребление алкоголя и наркотиков является распространенным осложнением депрессии; риск самоубийства (особенно в период уменьшения выраженности наиболее тяжелых симптомов депрессии) повышается. Особое внимание во время послеродовой депрессии следует уделять таким симптомам, как вспыльчивость и раздражительность. Они могут стать причиной жестокого обращения с детьми. В статье также представлены современные данные об эпидемиологии, этиологии, факторах риска возникновения послеродовых депрессий, их клинических проявлений, влиянии нелеченной материнской депрессии на развитие ребенка, терапии и образовательных модулях с целью распространения полидисциплинарного и межведомственного подхода при перинатальных проблемах охраны психического здоровья

Исторические и современные предпосылки к созданию факультета социальной работы в Сибирском государственном медицинском университете

Historical and modern data of the need of the creation of social work faculty in Siberian State Medical University is discussed in the present paper. The need of university education renovation in the sphere of mental health protection is a ground of the social specialist training. It is suggested a development of biopsychosocial education model that requires the training of social workers in the sphere of clinical psychology, psychotherapeutic supports and psychiatry. The analysis of the current state of social service efficiency in Russia reveals the need of the training of new generation specialists for the development of the social net, social assistance, support and rehabilitation of persons having temporary or chronic mental and behavioural disorders or in crisis living situations.В работе обсуждаются исторические и современные данные о необходимости открытия факультета социальной работы в Сибирском государственном медицинском университете. Обоснованием для подготовки специалистов по социальной работе является необходимость реконструкции университетского образования в сфере охраны психического здоровья. Предлагается развитие биопсихосоциальной модели образования, которая требует подготовки социальных работников в области клинической психологии, психотерапевтических интервенций и психиатрии. Анализ текущего состояния эффективности служб социальной работы в России выявляет необходимость подготовки новой генерации специалистов для развития социальной сети, социальной помощи, поддержки и реабилитации лицам, имеющим временные или хронические психические и поведенческие расстройства или попавшим в кризисные жизненные ситуации

СЕМИОТИКА, ДИАГНОСТИКА И ТАКТИКА ВЕДЕНИЯ ТЕРАПИИ ДЕПРЕССИВНОГО РАССТРОЙСТВА В КЛИНИЧЕСКОЙ МЕДИЦИНЕ

Study objectives are dedicated to brief synthesized establishment of diagnostics general standard, management and therapy of major depressive disorders (MDD) in clinical medicine to provide education in medical practice.Study methods are based on short-term, medium-term and long-term educational programs in 24 Russian and Ukraine cities, which were based on original educational programs under the direction of World Psychiatric Association (WPA) and International Committee For Prevention and Treatment of Depression (PTD). There, about 1450 doctors of different occupations were acquainted with the detection, management and treatment of MDD. The Russian version of WPA/PTD programs was created. The program of education included 4 modules. The Core module included an overview of the epidemiology, impact, concepts and classification, and etiology of depressive disorders as well as their recognition, diagnosis, and management in the primary care setting. The second module focused on depressive disorders in physical illness and covered those major illnesses for which is reasonable evidence for an association with depressive disorders. The third module included the development of the depressive disorders in older persons. The forth module included training physicians in mental health skills. 39 theme improvements for 858 primary care setting physicians within the framework of faculty training program of physicians’ development were the part of other programs of physicians’ occupations.Results. A number of highlights were included in the general algorithm of the educational programs. The creation of optimum «physician-depressive patient» contact demands a number of new skills to establish effective communication. These skills involve training of meeting of depressive patient and physician, the ability to follow a certain communication style; identify emotional, cognitive, psychomotor and nonverbal patterns of behavior. Besides that, the education included adherence to therapy and therapeutic alliance during the period of informed agreement. There was an education of fluent grasp of medicaments, which are the first choice in therapy. Integration of this knowledge is done during the periods of repeated interactive discussions of antidepressants’ effectiveness in depression therapy which is based on three-phased MDD therapy model. The process of working out the depressive patients’ management was based on systematization and diagnostic of depression disorders (Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, or International Classification of Diseases, Revision 10); detailed discussions of manifestation and thorough depression symptomatology; management approaches in depression disorders, different in manifestation.Conclusion. The basis of physicians' education was the idea that depressive disorder in holistic approach can't be considered as an independent psychiatric disease in its mild and moderate forms of manifestation. Depression, along with coronary artery disease, cerebrovascular diseases, diabetes, osteoporosis, obesity and fatness should be considered by clinical medicine within one cluster of noninfectious multifactorial chronic diseases.Исследование посвящено синтезированному созданию стандарта диагностики, тактики ведения терапии депрессивных расстройств (ДР) в клинической медицине для преподавания в общемедицинской практике.Настоящее исследование основано на собственных данных и 4 модулях образовательной программы Всемирной психиатрической ассоциации (ВПА) и Международного комитета по терапии и профилактике депрессий. Различные части и формы обучающей программы по депрессивным расстройствам в клинической практике врачей различных специальностей были апробированы при участии ВПА в 24 городах России и Украины. Кроме того, с 2004 по 2013 г. были проведены 39 тематических усовершенствований 858 врачам-интернистам в первичной медицинской сети на факультете усовершенствования врачей СибГМУ.В алгоритм образовательной программы был включен ряд основных положений. Создание оптимального контакта врача с депрессивным пациентом для выявления признаков и симптомов расстройства, выработка навыка встречи. Умение выделять при опросе эмоциональные, когнитивные, вегетативные, психомоторные и невербальные паттерны поведения. Помимо этого, после получения информированного согласия необходимо развивать приверженность терапии и терапевтический альянс. Отдельно проводилось обучение по стандартам применения лекарств, являющихся первым выбором в терапии. Закрепление этих знаний производилось при интерактивных многократных обсуждениях эффективности антидепрессантов в терапии депрессий. Они основаны на трехфазной модели лечения ДР. Процесс выработки тактики ведения депрессивных пациентов основывался на систематике и диагностике депрессивных расстройств (Международная классификация болезней 10-го пересмотра), обсуждениях степени выраженности ДР, развернутой семиотики депрессии и подходах в тактике ведения разных по степени выраженности ДР.Основу образования врачей составляла идея о том, что ДР в целостном подходе нельзя рассматривать как отдельное психиатрическое заболевание при его легких и умеренных формах проявления. Депрессия наряду с ишемической болезнью сердца, цереброваскулярными заболеваниями, сахарным диабетом, остеопорозом и ожирением должна рассматриваться клинической медициной в одном кластере неинфекционных, мультифакториальных хронических заболеваний

COGNITIVE IMPAIRMENTS AFTER STROKE

Thirty-two patients (mean age 53.912.4 years) with prior stroke were examined to clarify the pattern and degree of cognitive impairments depending on the site of a lesion. A control group comprised 32 subjects (mean age 52.112.9 years) without nervous system pathology. The authors made a complete clinical study to evaluate the neurological and mental status, a standardized interview, and A.R. Luria's neuropsychological tests. The patients with prior stroke were found to have lower values of auditory-verbal short-term and long-term memory, impaired concentration and voluntary attention scope. Mild cognitive disorders were established in middle-aged patients and moderate cognitive disorders of amnestic multifunctional type were in elderly patients. Auditory-verbal memory, visual gnosis, and some speech-related thought functions were impaired when in the involvement of the temporal and posterior frontal lobes in patients with prior stroke in the middle cerebral artery bed. Impaired visual image recognition and lower visual voluntary attention were detected in patients with prior stroke in the anterior and posterior cerebral arteries

Клинические особенности депрессивных расстройств у больных со злокачественными опухолями желудочно-кишечного тракта

Unipolar depression is prevalent in inpatients with GIT tumors. Following types of depressions have been distinguished: with cognitive disturbances, anxious-hypochondriac and agitated ones. For oncologists and surgeons clinical symptoms of high likelihood of depressive disorder in oncological pathology of GIT are as follows: pain, disturbance of quality of sleep, experienced weakness and breakdown, experienced fear, tension and sudden panic with agitation.Униполярная депрессия имеет высокую распространенность у стационарных больных с опухолями желудочно-кишечного тракта (ЖКТ). Выделены следующие типы депрессии: с когнитивными нарушениями, тревожно-ипохондрический, ажитированный. Для онкологов и хирургов клиническими симптомами высокой вероятности депрессивного расстройства при онкологической патологии ЖКТ являются боль, нарушение качества сна, ощущение слабости и упадка сил, чувство страха, чувство напряженности и внезапной паники с ажитацией

Терапевтическая эффективность расстройств депрессивного спектра в зависимости от врачебных вмешательств на догоспитальном этапе

Open clinical comparative assessment of therapy effectiveness of depressive spectrum disorders was carried out. We compared patients who began treatment in special psychiatric department and those who underwent different consultations, investigations and took medication in primary care before hospitalization. This investigation showed high effectiveness of therapy, reduction of depressive symptoms, improving of psychological state and increasing level of social adaptation in patients who didn’t contact with primary care settings.Проведено открытое клиническое сравнительное исследование эффективности терапии расстройств депрессивного спектра у пациентов, которые непосредственно начинали лечение в специализированном отделении, и у больных, проходивших и на догоспитальном этапе различные консультации, обследования и назначения в первичной медицинской сети. Исследование показало высокую эффективность терапии в плане редукции депрессивной симптоматики, улучшения психологического состояния и показателей социальной адаптации в группе пациентов, не обращавшихся в амбулаторнополиклинические службы
