Analiza agenților patogeni parodontali în microbiomul oral la pacienții cu carcinom cu celule scuamoase cap-gât
Background. Research on the relationship between the human microbiome and malignant tumors has received particular interest in recent years. Regarding the oral microbiota,
there are different types of bacteria that are associated with
oncogenesis of head and neck squamous cell carcinoma
(HNSCC). Objective of the study. The purpose of our study
was to determine the concentration of periodontal pathogens in the oral microbiome in patients with HNSCC. Material and methods. 26 saliva and oral buffer samples were
analyzed from patients with HNSCC by quantitative PCR
method. A comprehensive quantitative research kit „Dentoscreen” (Litech) was used, which allows the identification of the DNA of 7 periodontal pathogens: Porphyromonas
endodontalis, Porphyromonas gingivalis, Aggregatibacter actinomycetemcomitans, Treponema denticola, Fusobacterium
nucella. Results. Show that F. nucleatum exceeds the norm
in 20 samples and T. forsythia in 18. F. nucleatum is found
in high concentrations in most smokers and only in one patient in the “Non-smokers” category. Samples with abnormal concentrations of A. actinomycetemcomitans show high
concentrations for most species. In addition, the integration
with clinical data indicates the pathological presence of P.
endodontalis at all patients with laryngeal carcinoma and
approx. in 30% of patients with lip and oropharynx carcinoma. Conclusion. Of all the 7 species studied, F. nucleatum
and T. forsythia have pathological concentrations in most
samples.Introducere. Cercetările privind relația dintre microbiomul uman și tumorile maligne au înregistrat un interes
deosebit în ultimii ani. În ceea ce privește microbiota orală, există diferite tipuri de bacterii care sunt asociate cu
oncogeneza carcinomului cu celule scuamoase de cap și
gât (HNSCC). Scopul lucrării. Determinarea concentrației
agenților patogeni parodontali în microbiomul oral la pacienții cu HNSCC. Material și Metode. Au fost analizate 26 de
probe de salivă și tampon bucal de la pacienți cu HCSCC prin
metoda PCR cantitativ. S-a utilizat un kit cuprinzător pentru
cercetare cantitativă „Dentoscreen” (Litech), care permite
identificarea ADN-ului a 7 agenți patogeni parodontali: Porphyromonas endodontalis, Porphyromonas gingivalis, Aggregatibacter actinomycetemcomitans, Treponema denticola,
Fusobacterium nucleatum, Prevotella intermedia și Tannerella forsythia. Rezultate. Arată că F. nucleatum depășește
norma în 20 de probe și T. forsythia în 18. F. nucleatum se
găsește în concentrații mari la majoritatea fumătorilor și
doar la un pacient din categoria „Nefumători”. Probele cu
concentrații anormale de A. actinomycetemcomitans prezintă concentrații mari pentru majoritatea speciilor. De asemenea, integrarea cu datele clinice indică prezența patologică a
P. endodontalis la toți pacienții cu carcinom laringian și cca.
la 30% dintre pacienții cu carcinom al buzei și orofaringelui.
Concluzii. Din toate cele 7 specii studiate, F. nucleatum și T.
forsythia au concentrații patologice în majoritatea probelor
Prevalence of some mutations in the PIK3CA gene in patients with cervical squamous carcinoma from the Republic of Moldova
Introducere. Carcinomul scuamos cervical (CESC) este o
problemă de sănătate semnificativă care necesită noi modele de clasificare moleculară direcționate către terapiile de
precizie, în special pentru CESC metastatic. Genele implicate
în calea de semnalizare PI3K, cum ar fi PIK3CA, reprezintă
o țintă potențială și un biomarker util pentru tratamentul
țintit. Scopul lucrării. Testarea a 3 mutații în gena PIK3CA la pacientele cu CESC din Republica Moldova. Material și metode. Au fost analizate 92 probe de țesut tumoral
proaspăt recoltate de la paciente diagnosticate primar cu
CESC. ADN-ul izolat a fost testat pentru 3 mutații în gena
PIK3CA: c.1624G>A, c.1633G>A, c.3140A>G prin metoda
castPCR. Rezultate. Prevalența mutațiilor PIK3CA în grupul
nostru de studiu a constituit 29,35% (27/92) dintre care
27,17% (25/92) au fost pozitive pentru o singura mutație,
iar 2,17% (2/92) au prezentat mutații duble. Din acestea,
17,39% paciente au fost pozitive pentru mutația c.1624G>A,
9,78% pentru mutația c.1633G>A și 2,17% pentru mutația
c.3140A>G. Concluzie. Prevalența mutațiilor PIK3CA testate
a constituit 29,35%. Prezența acestor mutații la un număr
mare de paciente reprezintă o oportunitate pentru dezvoltarea terapiei țintite anti PI3K în CESC.Background. Cervical squamous cell carcinoma (CESC) is
a significant health problem that requires new molecular
classification models for the precision therapies, especially for metastatic CESC. Genes involved in the PI3K signaling pathway, such as PIK3CA, serve as potential target and
a useful biomarker for targeted therapy. Objective of the
study. To test 3 mutations in the PIK3CA gene in patients
with CESC from the Republic of Moldova. Material and
methods. Were analyzed 92 samples of freshly collected
tumor tissue from patients primarily diagnosed with CESC.
The isolated DNA was tested for 3 mutations in the PIK3CA gene: c.1624G>A, c.1633G>A, c.3140A>G by the castPCR method. Results. The prevalence of PIK3CA mutations
in our study group was 29.35% (27/92) of which 27.17%
(25/92) were positive for a single mutation, and 2.17%
(2/92) presented double mutations. Of these, 17.39% patients were positive for the c.1624G>A mutation, 9.78% for
the c.1633G>A mutation and 2.17% for the c.3140A>G mutation. Conclusion. The prevalence of the analyzed PIK3CA
mutations was 29.35%. The presence of these mutations in
a significant number of patients represents an opportunity
for the development of targeted anti PI3K therapy in CESC

Material and methods 26 saliva and oral buffer samples
were analyzed from patients with HNSCC by quantitative
PCR method for 7 periodontal pathogens: Porphyromonas
endodontalis, Porphyromonas gingivalis, Aggregatibacter
actinomycetemcomitans, Treponema denticola,
Fusobacterium nucleatum, Prevotella intermedia
and Tannerella forsythia.
Purpose of the study was to determine the concentration
of periodontal pathogens in the oral microbiome in
patients with HNSCC.
Results show F. nucleatum exceeds the norm in 20
samples (19 smokers and 1 non-smoker) and T. forsythia
in 18. P. endodontalis was identified in all patients with
laryngeal carcinoma and approx. in 30% of patients with
carcinoma of the lip and oropharynx.

Достижения и противоречия в диагностике и лечении пожилых пациентов с хроническими миелопролиферативными новообразованиями
Rezumat
Neoplaziile mieloproliferative cronice (NMC) la persoanele în
etate reprezintă cele mai frecvente leucemii cronice în structura
morbidității prin hemopatii maligne, cu afectarea primară a
măduvei osoase, în fazele avansate se caracterizează prin
evoluție gravă, recidivantă și au un impact socioeconomic
negativ. A fost realizat un studiu analitic, secundar – trecerea
în revistă a literaturii sub formă de articol de sinteză. Au fost
identificate și selectate 50 de surse primare relevante, semnificative după factorul de impact. Conform datelor din sursele
bibliografi ce mondiale, diagnosticul contemporan al NMC,
indiferent de vârstă, se bazează pe examenul histopatologic
al măduvei osoase, pe screeningul mutațional și citogenetic
al sângelui venos. Managementul pacienților în etate cu
NMC Ph-negative clasice include: controlul mieloproliferării
și al splenomegaliei, prevenirea și combaterea complicațiilor
trombovasculare și hemoragice, controlul terapeutic simptomatic al manifestărilor maladiei, ameliorarea calității vieții
pacienților și a indicilor supraviețuirii generale și fără recidive.
Chimioterapia „de țintă” cu inhibitori ai tirozinkinazei rămâne
o opțiune curativă de elecție pentru pacienții cu leucemie mieloidă cronică, indiferent de categoria de vârstă. Chimioterapia
cu inhibitori de JAK kinaze constituie o opțiune eficientă de
tratament al mielofibrozei primare și al policitemiei vera la
pacienții vârstnici cu risc înalt și fără comorbidități severe.
Prezența mutațiilor cu risc molecular înalt afectează semnificativ supraviețuirea pacienților cu NMC, cu contrabalansarea
impactului îmbătrânirii.Summary
Chronic myeloproliferative neoplasms (CMN) are the most
common chronic leukemias in the elderly within the structure
of morbidity by hematological malignancies with primary
bone marrow involvement, being characterized in the advanced stages by severe, recurrent evolution and negative
socio-economic impact. An analytical, secondary study was
performed - the narrative review of the literature in the form
of a synthesis article. Fifty relevant primary sources are identified and selected, being significant according to the impact
factor. According to the data from the world bibliographic
sources, the contemporary diagnosis of CMN regardless of
age is based on histopathological examination of the bone
marrow, mutational screening and cytogenetics of the venous
blood. Management of elderly patients with classic Ph-negative
CMN includes control of myeloproliferation and splenomegaly,
prevention and treatment of vasculo-thrombotic and hemorrhagic complications, symptomatic therapeutic control of
disease manifestations, improvement of the quality of life and
of the overall and relapse-free survival. ”Target” chemotherapy
with tyrosine kinase inhibitors remains a curative option of
choice for patients with CML regardless the age. Chemotherapy
with JAK kinase inhibitors is an effective treatment option for
primary myelofibrosis and polycythemia vera in the high risk
elderly patients without severe comorbidities. The presence of
mutations with high molecular risk affects significantly the
survival of patients with CMN, counterbalancing the impact
of aging.Резюме
Хронические миелопролиферативные новообразования
(ХМН) являются наиболее распространенными хроническими лейкозами у пожилых людей в структуре заболеваемости злокачественными гемопатиями с первичным
поражением костного мозга, характеризующиеся в
поздних стадиях тяжелым, рецидивирующим течением
и негативным социально-экономическим воздействием.
Проведено аналитическое, вторичное исследование – обзор литературы в виде синтез-статьи. Oпределены и
выбраны 50 релевантных первичных источников, значимые в соответствии с фактором воздействия. Согласно
данным мировых библиографических источников, современная диагностика ХМН, независимо от возраста,
основана на гистопатологическом исследовании костного мозга, мутационном и цитогенетическом скрининге
венозной крови. Ведение пациентов пожилого возраста
с классическими Ph-отрицательными ХМН включает: контроль миелопролиферации и спленомегалии,
профилактику и лечение сосудисто-тромботических
и геморрагических осложнений, симптоматический
терапевтический контроль проявлений заболевания,
улучшение качества жизни пациентов и показателей
общей и безрецидивной выживаемости. Независимо
от возраста «таргет» химиотерапия ингибиторами
тирозинкиназы остается лечебной опцией выбора для
пациентов с хроническим миелолейкозом. Химиотерапия ингибиторами JAK-киназы является эффективным
методом лечения первичного миелофиброза и истинной
полицитемии у пожилых пациентов с высоким риском
и без тяжелых сопутствующих заболеваний. Наличие
мутаций с высоким молекулярным риском значительно
снижает выживаемость пациентов с ХМН, уравновешивая влияние старения
