    Occupational Therapy in Oncology and Palliative Care

    Cancer is a chronic disease that may occur in both children and adults. Occupational therapy focuses on the activity limitations and participation problems in their life. Oncology rehabilitation involves in helping an individual with cancer to regain maximum physical, psychological, cognitive, social, and vocational functioning with the limits up to disease and its treatments in an interdisciplinary team concept. These treatment options are associated with the risk of some side effects, including fatigue, pain, cognitive problems, decrease in bone density and muscle endurance, weight loss, and stress- or anxiety-related psychosocial problems. Occupational therapy approaches are a holistic view in a client center and use training in activities of daily living, assistive technology, education of energy conservation techniques, and management of treatment-related problems, such as pain, fatigue, and nausea. In palliative and hospice care, occupational therapists support clients with cancer by minimizing the secondary symptoms related to cancer and its treatments. At the end of life, occupational therapy offers to identify the roles and activities that are meaningful and purposeful to the client with cancer and try to determine the barriers that limit their performance. Clients with cancer who have childhood cancer or adult cancer can face problems about body structure and functions, activity, and participation, which may limit their participation to their daily life

    Animal-Assisted Therapy in Occupational Therapy

    Developing technology, rehabilitation services, and health definitions have brought about the use of different treatments as well as traditional treatments. Some of these methods are virtual reality, animal-assisted practices, and aqua therapy. Animal-assisted approaches are the therapeutic methods of eliminating and using the problems that an individual experiences in the physical, emotional, psychological, social, sensory, and environmental development areas by taking advantage of human and animal interaction. These methods consist of two basic chapters: animal-assisted activities (AAAs) and animal-assisted therapies (AATs). An interdisciplinary team is needed to implement the methods. This team is composed of personnel, such as occupational therapist (OT), physiotherapist, speech therapist, special education specialist, and a psychologist, who have been trained in this area. In recent years, these methods have been used to increase social interaction with physical structure and functions in neurodevelopmental disorders such as autism spectrum disorder and cerebral palsy. In addition, it is actively used in mental disorders such as depression and anxiety because an improvement is observed by providing the individual to feel safe.     Rehabilitation of Patients with Prostate Cancer

    Cancer rehabilitation involves helping an individual with cancer to regain maximum psychological, physical, cognitive, social, and vocational functioning with the limits up to disease and its treatments in an interdisciplinary team concept. Prostate cancer is one of the most frequent male malignancies in the world. Prostate cancer treatment options have the risk of some side effects including loss of muscle strength, fatigue, pain, urinary incontinence, erectile dysfunction, cognitive problems, decrease in bone density, weight loss, gynecomastia, and hot flushes with stress-related psychosocial problems. Relative to other cancers, the prognosis of men with prostate cancer is much better and the potential treatment-related side effects have important implications which can affect the health-related quality of life (QOL) of this population. Recent studies support the efficiency of multimodal treatment to recognize, prevent, and increase functional recovery with an interdisciplinary rehabilitation team which includes physical and occupational therapists. This chapter describes briefly cancer rehabilitation and rehabilitation approaches at every stage of patients with prostate cancer for minimizing the morbidity rate associated with prostate cancer treatment to increase occupational participation and improve QOL

    Pigmented Bowen’s Disease of the Genitalia Masquerading as Malignant Melanoma

    Abstract Pigmented Bowen’s disease is a rare subtype of in situ squamous cell carcinoma of the skin and mucosa, with a potential risk of invasion and metastasis. It is universally accepted that human papillomavirus (HPV) is the cause of genital Bowen’s disease. Herein we report an unusual case of pigmented Bowen’s disease of the genital area that clinically simulated malignant melanoma. Accurate diagnosis could only be established after histological examination. Polymerase chain reaction (PCR) analysis showed that the lesion harbored HPV 16 DNA. Although pigmented Bowen’s disease is rare, it should be considered in the differential diagnosis of all pigmented lesions of the genitalia. This case report highlights the necessity of biopsy and histopathological examination for every suspicious cutaneous lesion.  Key words: pigmented Bowen’s disease; human papillomavirus </p

    Virtual Reality and Occupational Therapy

    Virtual reality is three dimensional, interactive and fun way in rehabilitation. Its first known use in rehabilitation published by Max North named as “Virtual Environments and Psychological Disorders” (1994). Virtual reality uses special programmed computers, visual devices and artificial environments for the clients’ rehabilitation. Throughout technological improvements, virtual reality devices changed from therapeutic gloves to augmented reality environments. Virtual reality was being used in different rehabilitation professions such as occupational therapy, physical therapy, psychology and so on. In spite of common virtual reality approach of different professions, each profession aims different outcomes in rehabilitation. Virtual reality in occupational therapy generally focuses on hand and upper extremity functioning, cognitive rehabilitation, mental disorders, etc. Positive effects of virtual reality were mentioned in different studies, which are higher motivation than non‐simulated environments, active participation of the participants, supporting motor learning, fun environment and risk‐free environment. Additionally, virtual reality was told to be used as assessment. This chapter will focus on usage of virtual reality in occupational therapy, history and recent developments, types of virtual reality technologic equipment, pros and cons, usage for pediatric, adult and geriatric people and recent research and articles

    Onychomatricoma masquerading as candidal onychomycosis and paronychia

    Onychomatricoma is a benign slow-growing fibroepithelial tumor arising from the nail matrix. The tumor has been described as a new entity almost two decades ago. Although the clinical appearance is typical, most cases are probably misdiagnosed by physicians because of unfamiliarity with the condition. Herein we describe a case of onychomatricoma masquerading as candidal onychomycosis and paronychia and treated erroneously as such. We believe that OM is still an unrecognized entity and probably more common than cited in the relevant literature. Awareness of the condition not only offers accurate diagnosis and appropriate management but also precludes the long-term use of potentially toxic oral antifungal medications. </span

    Yeni teknolojilerle baharatlardan esansiyel yağ ekstraksiyonu ve bu yağların fiziksel, antioksidan ve antimikrobiyal özellikleri

    Bu çalışmanın amacı yeni ekstraksiyon teknolojileri kullanılarak Türkiye’de yetişen kekik, defne ve biberiye (esansiyel) uçucu yağlarının eldesidir. Farklı ekstraksiyon metodları ile elde edilen ürün yağ içeriği ve kompozisyonu açısından karşılaştırılmıştır. Ayrıca, farklı ekstraksiyon metodlarıyla elde edilen uçucu yağların antioksidan ve antimikrobiyal özellikleri tespit edilmiştir. Bu çalışmada, kekik ve defne için çözgensiz mikrodalga ekstraksiyonu (ÇME) ve biberiye için mikrodalga yardımlı hidrodistilasyon (MYH) kullanılmıştır. Ayrıca, baharatlardan yağ elde etmek için süperkritik akışkan ekstraksiyonu denenmiştir. Kontrol olarak hidrodistilasyon kullanılmıştır. ÇME ya da süperkritik karbondioksit ekstraksiyon metodlarının, hidrodistilasyonla kıyaslandığında daha fazla kekik uçucu yağı verdiği tespit edilmiştir. ÇME ve hidrodistilasyonla elde edilen uçucu yağların kompozisyonları dolayısıyla antioksidan ve antimikrobiyal aktiviteleri arasında önemli bir fark bulunmazken süperkritik akışkan ekstraksiyonu ile elde edilen yağın kompozisyonu farklı bulunmuştur. ÇME yönteminde hidrodistilasyona göre işlem süresi %80 azalmıştır. ÇME ve hidrodistilasyon yöntemleriyle elde edilen defne yağları arasında verim, antioksidan ve antimikrobiyal aktivite bakımından önemli bir fark bulunmamıştır. ÇME yöntemi kullanıldığında işlem süresinin hidrodistilasyona göre % 55-60 oranında kısaldığı görülmüştür. En yüksek verim süperkritik karbondioksit ekstraksiyonunda elde edilirken, en çok oksijenli bileşen miktarı ÇME yöntemi kullanıldığında elde edilmiştir. Biberiye yaprakları suyu yeterince emmediği için ÇME yöntemi bu bitki için başarılı olmamış ve onun yerine MYH metodu kullanılmıştır. Bu metod, hidrodistilasyona göre işlem süresini % 65 oranında kısaltmıştır. MYH metodu ile hidrodistilasyona ve süperkritik akışkan ekstraksiyonuna göre daha fazla oksijenli bileşen elde edilmiştir. MYH metodu ve hidrodistilasyon ile elde edilen biberiye uçucu yağları antioksidan ve antimikrobiyal aktiviteleri açısından benzerlik göstermiştir. Hidrodistilasyona göre daha kısa sürede daha kaliteli yağ elde edilen çözgensiz mikrodalga ekstraksiyonu kekik ve defne için ve mikrodalga yardımlı hidrodistilasyon biberiye için en uygun ekstraksiyon metodları olarak seçilmiştir.The objective of this study is to extract essential oils of oregano, laurel and rosemary which are widely grown in Turkey by using novel extraction methods. The essential oils obtained from different extraction methods were compared with respect to yield and composition. In addition, the antimicrobial and antioxidant properties of the essential oils obtained using different extraction methods were determined. In this study, solvent free microwave extraction (SFME) for oregano and laurel and microwave assisted hydrodistillation (MAHD) for rosemary were used. In addition, extractions were performed using supercritical carbondioxide extraction (SFE) method. Essential oils were also obtained using hydrodistillation method as control. SFME and supercritical carbondioxide extraction offered significantly higher oregano essential oil yield as compared to hydrodistillation. While no significant difference was obtained in the compositions and consequently antioxidant and antimicrobial activities of oregano essential oils obtained by SFME and hydrodistillation, the composition was quite different in the case of SFE. Conventional process time was reduced by 80% in SFME. No significant differences were obtained between SFME and hydrodistillation with respect to both yield and antioxidant and antimicrobial activities of laurel oil. The process time was reduced by 55-60% when SFME was used. The highest yield was obtained in SFE while the concentration of oxygenated compounds was the highest in SFME. In the extraction of essential oils from oregano (Origanum vulgare L.) and laurel (Laurus nobilis L.) SFME gave good results but it couldn’t be applied for the extraction of essential oil from rosemary. Therefore, in the case of rosemary, MAHD was used. MAHD reduced process time by 65 % as compared to hydrodistillation. Significantly higher amounts of oxygenated compounds were detected in the essential oils extracted by MAHD than by hydrodistillation and SFE. Rosemary essential oils obtained by hydrodistillation and MAHD were not different with respect to their antioxidant and antimicrobial activities.
