    Teen Risk-Taking: Promising Prevention Programs and Approaches

    To help close the knowledge gap and to help program directors, practitioners, and community leaders enlarge the network of effective programs and approaches for at-risk youth, Urban Institute researchers reviewed what is known about successful prevention interventions and their dissemination. They identified 51 problem behavior prevention interventions whose initial effectiveness has been demonstrated through scientific evaluation. A subset of 21 programs was selected on the basis of the rigor of their evaluations or the strength of their results for closer examination of the program elements and/or delivery modes that appeared to be associated with their effectiveness. The researchers also explored with the assistance of experienced prevention scientists and school-based practitioners what might be the essential elements of schools' and other community organizations' readiness to undertake research-based problem behavior prevention programming. This guidebook to promising programs and approaches offers the fruits of that research.     Advancing Patient Safety in the U.S. Department of Veterans Affairs

    As part of a systemwide transformation, the VA formed its National Center for Patient Safety to foster an organizational culture of safety within its nationwide network of hospitals and outpatient clinics. A recent medical team training program designed to improve communication among operating room staff was associated with a reduction in surgical mortality and improvements in quality of care, on-time surgery starts, and staff morale. The program is now being expanded to other clinical units, along with a patient engagement program that prevents errors by facilitating communication relating to patients' daily care plans. A recognition program stimulated facilities to conduct timelier and higher-quality root-cause analyses of reported safety events to identify stronger actions for preventing their recurrence. Other initiatives have reduced rates of health care -- associated infections, patient mortality, and post-operative complications. Success factors include leadership accountability for performance and organizational support for testing, expanding, and adopting improvements

    Characteristics of TQM: Evidence from the RIT/USA Today Quality Cup Competition

    This paper reports the results of a field study examining the use of TQM at 15 firms. The sample is drawn from winners and finalists of the RIT/USA Today Quality Cup. The authors interviewed 75 employees (5 per firm) including 14 executives, 44 middle managers, and 17 front line workers. The interviews elicited information on the motives for adopting TQM, the role of leadership, the use of monitoring, the use of rhetoric, the extent and type of training, the basis for employee evaluation, compensation, and promotion, the use of teams, reallocation of authority, and the results of the TQM program. We use the data to provide a description of how TQM works in practice, including factors that determine patterns of use across firms. A major result is that team-based problem solving is used about twice as frequently as devolution of authority in our sample. We attribute this result to the higher costs of monitoring and corporate change associated with devolution relative to problem solving.

    Eliminating Language Barriers for LEP Individuals: Promising Practices from the Public Sector

    While the focus of this report is on eliminating language barriers for limited English proficient (LEP) individuals, any strategy to improve communications with this population must also include English learning and address the shortage of high-quality English as a Second Language (ESL) courses for adults. State-administered ESL programs currently serve only about a million of the estimated 12.4 million LEP adults in the United States who need language instruction. The underfunding of ESL programs means that large numbers of immigrant adults who wish to learn English are unable to enroll in classes or face overcrowded classrooms. For instance, a 2006 national survey of ESL providers found that 57 percent of these programs maintained waiting lists -- ranging from a few weeks to more than three years -- and could not accommodate the high numbers of immigrants interested in learning English. Policy experts and organizations that work with adult English learners have proposed various strategies to increase the availability of high-quality ESL courses, but lack of political support at the national level -- coupled with the current fiscal crisis -- has weakened efforts to help immigrants improve their English skills
