    Highlights. Congenital heart disease (CHD) is estimated to occur nearly in one per 100 newborns and a third of these babies are in need of corrective procedures. There is a downward trend in mortality associated with advances in surgical treatment and perioperative care. An increasing number of patients with corrected CHD are accumulating in the population. This article provides a literary review of the current state of long-term outcomes of CHD corrections and possible mechanisms for improving late outcomes.Abstract The article analyzes the literature on long-term results of surgical treatment of congenital heart defects and possible mechanisms for their improvement. The bibliographic method was used. The analysis of domestic and foreign literary sources with a search depth of 20 years is carried out. The criteria for including publications were: access to full-text publications, original research, scientific and review articles. Exclusion criteria: abstracts and summaries of publications. The following search queries were formulated for the review: in English: late outcomes of corrections of congenital heart defects, in Russian: long-term results of corrections of congenital heart defects, long-term results after corrections of congenital heart defects. The used search engines are Science Direct, PubMed, Cyberleninka, E-library. The analysis of the literary data showed that the volume and duration of patients’ observation that underwent surgical correction of congenital heart defects remain controversial. In world and domestic practice modern digital technologies are used for the purpose of patients remote monitoring. Remote monitoring programs in the Russian Federation are used for such socially significant diseases as diabetes mellitus, bronchial asthma, as well as cardiac rehabilitation of adult patients. There are no similar programs for patients with congenital heart defects. There is a problem of patients remote monitoring after congenital heart defects surgical treatment. In Russia today there is no any comprehensive program for remote monitoring of children after congenital heart disease surgical correction. We believe that a universal remote monitoring system for managing this group of patients must be created.Основные положения. Врожденные пороки сердца встречаются у одного из ста новорожденных, причем треть этих детей нуждаются в корректирующих процедурах. Отмечена тенденция уменьшения смертности, связанная с достижениями в хирургическом лечении и периоперационном уходе. В популяции накапливается все больше пациентов с корригированными врожденными дефектами сердца. В статье оценены современное состояние отдаленных исходов коррекций врожденных пороков сердца и возможные механизмы улучшения поздних результатов по данным литературы.Резюме В статье проведен анализ зарубежной и отечественной литературы, посвященной отдаленным результатам хирургического лечения врожденных пороков сердца и возможным механизмам их улучшения. Глубина поиска – 20 лет. Критерии включения источников: доступ к полному тексту, оригинальные исследования и обзорные статьи. Критерии исключения: абстракты публикаций. Для обзора сформулированы следующие поисковые запросы на английском языке: late outcomes of corrections of congenital heart defects, на русском языке: «отдаленные результаты коррекции врожденных пороков сердца», «долгосрочные результаты коррекции врожденных пороков сердца». Поиск литературы выполнен в системах ScienceDirect, PubMed, «КиберЛенинка», eLIBRARY.ru. Анализ данных показал, что остаются спорными вопросы объема и длительности наблюдения пациентов, перенесших хирургическую коррекцию врожденных пороков сердца. В мировой и отечественной практике применяют современные цифровые технологии с целью дистанционного мониторинга больных. В России программы удаленного отслеживания состояния пациентов используют при таких социально значимых заболеваниях, как сахарный диабет, бронхиальная астма, а также для кардиологической реабилитации взрослых больных. Подобные программы для дистанционного наблюдения детей, перенесших хирургическую коррекцию врожденных патологий сердца, на сегодняшний день в нашей стране отсутствуют. Также актуальна проблема отдаленного контроля состояния детей после хирургического устранения врожденных дефектов сердца. Таким образом, необходимо создание комплексной универсальной системы удаленного мониторинга и ведения данной группы больных

    The Internet of Things (IoT) and cloud technologies have been the main focus of recent research, allowing for the accumulation of a vast amount of data generated from this diverse environment. These data include without any doubt priceless knowledge if could correctly discovered and correlated in an efficient manner. Data mining algorithms can be applied to the Internet of Things (IoT) to extract hidden information from the massive amounts of data that are generated by IoT and are thought to have high business value. In this paper, the most important data mining approaches covering classification, clustering, association analysis, time series analysis, and outlier analysis from the knowledge will be covered. Additionally, a survey of recent work in in this direction is included. Another significant challenges in the field are collecting, storing, and managing the large number of devices along with their associated features.     IoT and its business impact on remote monitoring of patients with chronic diseases in Germany

    The goal of this dissertation is to determine the business impact of IoT and remote monitoring on patients with chronic diseases in Germany. Therefore, expert interviews with representatives of the four major affected stakeholder groups were conducted. These four groups consist of statutory health insurance companies, businesses, doctors and patients. The aim of these interviews was to assess the current status of IoT and remote monitoring in the German healthcare system and to find out about the main obstacles that currently keep the business impact at a low level. Although only representatives from the first three groups could be interviewed all interviewees agreed that IoT is in its early stages in Germany. The main obstacles impeding a significant growth of IoT and remote monitoring in Germany are identified as technological, regulatory, and cultural ones. Additionally, the self-governing structures of the German healthcare system and the multidisciplinary approach of already ongoing IoT projects complicate the diffusion of IoT solutions. Despite these barriers the interviewed experts are convinced that IoT and remote monitoring will prevail in Germany sooner or later.O objetivo desta dissertação é determinar o impacto económico da Internet das Coisas (IoT – Internet of Things) e monitorização remota dos pacientes com doenças crónicas na Alemanha. Portanto, foram conduzidas entrevistas com representantes dos quatro maiores grupos de intervenientes afetados. Estes quatro grupos consistem em seguradoras de saúde, negócios, médicos e pacientes. O objetivo destas entrevistas foi aferir o estado atual da IoT e monitorização remota no sistema de saúde Alemão e averiguar os principais obstáculos que mantêm atualmente um baixo nível de impacto económico. Apesar de apenas os representantes dos primeiros três grupos terem sido entrevistados, todos concordaram que a IoT está na sua fase inicial na Alemanha. Os maiores obstáculos que impedem um crescimento significativo da IoT e monitorização remota na Alemanha foram identificados como sendo tecnológicos, regulatórios e culturais. Para além do mais, as estruturas autónomas do sistema de saúde Alemão e a abordagem multidisciplinar dos projetos de IoT já em curso complicam a difusão de soluções IoT. Não obstante estas barreiras, os especialistas entrevistados estão convencidos que a IoT e monitorização remota vai prevalecer na Alemanha mais tarde ou mais cedo

    Systematic Approaches for Telemedicine and Data Coordination for COVID-19 in Baja California, Mexico

Our model is based on human-centric design factors and cross disciplinary collaborations for scalable data-driven enablement of smartphone, cellular, and video Teleconsul-tation technologies to link hospitals, clinics, and emergency medical services for point-of-care assessments of COVID testing, and for subsequent treatment and quar-antine decisions. A multidisciplinary team was rapidly created, in cooperation with different institutions, including: the Autonomous University of Baja California, the Ministry of Health, the Command, Communication and Computer Control Center of the Ministry of the State of Baja California (C4), Colleges of Medicine, and the College of Psychologists. Our objective is to provide information to the public and to evaluate COVID-19 in real time and to track, regional, municipal, and state-wide data in real time that informs supply chains and resource allocation with the anticipation of a surge in COVID-19 cases. RESUMEN Proporcionamos un modelo para la implementación sistemática de la telemedicina dentro de un gran centro de evaluación de COVID-19 en el área de Baja California, México. Nuestro modelo se basa en factores de diseño centrados en el ser humano y colaboraciones interdisciplinarias para la habilitación escalable basada en datos de tecnologías de teleconsulta de teléfonos inteligentes, celulares y video para vincular hospitales, clínicas y servicios médicos de emergencia para evaluaciones de COVID en el punto de atención. pruebas, y para el tratamiento posterior y decisiones de cuarentena.
