    SUMMARY OF FINDINGS 90. Food safety: Many aspects of organic agriculture reduce the risks of pathogens (zoonoses), mycotoxins, bacterial toxins and industrial toxic pollutants, compared to conventional agriculture. However, some other aspects potentially increase them. Reduced resistance to antibiotics in zoonotic pathogens indicates a better prognosis for patients if an infection does occur. For natural plant toxins, the content in plants appears to systematically be 10 to 50 percent higher than in conventional plants. However, this is in a range of concentrations where these compounds have no toxic effect and may even benefit human health. 91. Pesticide poisoning: This is an area where very substantial health problems have been documented, especially among farmers and their families. Pesticide poisoning causes some 20 000 deaths per year globally and an average of 11 days wages lost due to illness, per farmer per incidence, in some areas. Even symptom-free workers often exhibit biomarker changes indicating increased risk of diseases, including Parkinson’s disease. With the present level of knowledge, elimination of such horrible conditions, which can be achieved on a short timescale, is the quantitatively single most important benefit of organic farming in terms of human health. Still, long-term occupational exposure to copper also increases the risk of Parkinson’s disease, but not as much as exposure to synthetic pesticides. 92. Pesticide residues: The levels in organic products are consistently 4 to 5 times lower than in conventional products. However, no definitive causal connection with harm to consumers has ever been demonstrated for food produced in accordance with general (conventional) food safety rules. Errors and accidents can cause contamination with harmful levels of pesticides, but this risk is eliminated when no pesticides are present. Across the different safety risks in both systems, the best managers achieve much better standards than the average producers, and the occurrence of serious hazards is so low that no significant differences have been demonstrated between production systems. 93. Food quality: Consumers generally appreciate that food is authentic and trustworthy and produced with care for them and the environment. So reduced food additives and pesticide residues, good traceability and emphasis on animal welfare all support the perception of organic food as being of high-quality. Differences in taste between organic and conventional food products are strongly affected by interaction with local aspects and therefore show few general trends. Only poultry (broiler) produced according to the organic standards results in a clearly differentiated taste compared with mainstream conventional products. 94. Nutritional adequacy: In developing countries, organic agriculture has several advantages for the provision of nutrients, such as higher Zn/phytate ratio and better amino acid composition in cereals. Also, a more balanced diet due to the greater diversity of organic rotations, including legumes and various types of vegetables, and the need for animals on each farm provide important nutritional benefits. In developed countries, nutritional value is much more difficult to determine. However, the higher levels of plant secondary metabolites and conjugated fatty acids in milk may provide important protection against cardiovascular disease, cancer and other diseases known to be influenced by diet. 95. Human health: Epidemiological studies have shown better health scores among consumers of organic food for immunological characteristics and weight control, and similar benefits have been reproduced in animal studies, supporting a possible causal role of the food production system. 96. Post-harvest operations: Higher activity of plant defense mechanisms in organic plants reduces the losses during transport and storage. The preference for local products and short supply chains also reduce the loss of quality during transport. 97. Pollution of drinking water: Organic farmers have substantially higher economic incentives than conventional farmers to establish and maintain sufficient capacity for collection, composting and incorporation of animal and human wastes as valuable fertilizer. This is particularly important in areas where sanitation is not provided or standards not enforced by the authorities. Such measures will also substantially reduce contamination with nitrates and phosphorus. There is little evidence that these minerals have any harmful effects on humans, if the drinking water is free of pathogens, except by promoting blooms of toxic algae. 98. Pollution of the environment: Persistent pesticides (such as DDT) have damaged wildlife globally and are still being used in many developing countries. Organic agriculture protects the local environment against all types of pesticides and has potential to benefit the global situation if the proportion of land under organic management becomes large enough to reduce the total use. Pollution with nitrate and phosphorus are major causes of eutrophication. Organic farms leach lower levels of phosphorus into drainage water than conventional ones. For nitrate, the loss from organic farms tends to be slightly lower than conventional, except when comparing organic outdoor pig production with conventional indoor production. However, recent data indicate that organically managed soil may be more efficient at denitrification, releasing most of the nitrate into the atmosphere as harmless N2. If this is a general trend, the benefits of organic farming are much larger than previously estimated

    Comparison of Intakes of US Chinese Women Based on Food Frequency and 24-Hour Recall Data

    The objective of this cross-sectional study was to compare dietary reports from a food frequency questionnaire (FFQ) for US Chinese women with 24-hour recall estimates. The subjects were 56 women recruited through organizations in Philadelphia’s Chinese community. Spearman correlations were used to describe FFQ estimates of food servings per month and nutrient intake per day vs estimates from three 24-hour recalls over 1 month. On average, women reported at least weekly consumption of 28 of 96 FFQ food items. The three most frequently consumed were rice (38 times/month), tea (29 times/ month), and dark green, leafy vegetables (18 times/ month). Comparing reported frequencies of the 28 foods to 24-hour recall estimates, the median Spearman correlation was 0.36. For nutrient estimates, correlations were high (r\u3e0.5) for dietary fiber and calcium; moderate (r=0.25 to 0.5) for energy, saturated fat, cholesterol, carbohydrates, protein, folic acid, and iron; but poor (

    Zinc intake, status and indices of cognitive function in adults and children: a systematic review and meta-analysis

    In developing countries, deficiencies of micronutrients are thought to have a major impact on child development; however, a consensus on the specific relationship between dietary zinc intake and cognitive function remains elusive. The aim of this systematic review was to examine the relationship between zinc intake, status and indices of cognitive function in children and adults. A systematic literature search was conducted using EMBASE, MEDLINE and Cochrane Library databases from inception to March 2014. Included studies were those that supplied zinc as supplements or measured dietary zinc intake. A meta-analysis of the extracted data was performed where sufficient data were available. Of all of the potentially relevant papers, 18 studies met the inclusion criteria, 12 of which were randomised controlled trials (RCTs; 11 in children and 1 in adults) and 6 were observational studies (2 in children and 4 in adults). Nine of the 18 studies reported a positive association between zinc intake or status with one or more measure of cognitive function. Meta-analysis of data from the adult’s studies was not possible because of limited number of studies. A meta-analysis of data from the six RCTs conducted in children revealed that there was no significant overall effect of zinc intake on any indices of cognitive function: intelligence, standard mean difference of <0.001 (95% confidence interval (CI) –0.12, 0.13) P=0.95; executive function, standard mean difference of 0.08 (95% CI, –0.06, 022) P=0.26; and motor skills standard mean difference of 0.11 (95% CI –0.17, 0.39) P=0.43. Heterogeneity in the study designs was a major limitation, hence only a small number (n=6) of studies could be included in the meta-analyses. Meta-analysis failed to show a significant effect of zinc supplementation on cognitive functioning in children though, taken as a whole, there were some small indicators of improvement on aspects of executive function and motor development following supplementation but high-quality RCTs are necessary to investigate this further

    Forage legumes as feed for pigs in smallholder production systems in the North of Lao PDR

    This thesis sought to characterise and understand existing smallholder pig production systems in the North of Lao PDR and to investigate the role of forage legumes as alternative protein sources for pigs in smallholder production systems. A survey found that there were three main pig rearing systems, free-scavenging, semi-scavenging and confinement (enclosures and pens), in the study areas. These systems were managed differently depending on intensity of crop production, farmer ethnicity and reason for keeping pigs. Farmers fed their pigs mainly rice bran, distiller's waste, maize, cassava and green plant materials. The growth performance was poor, with an average daily gain (ADG) of around 100 g. The reproductive performance of sows was low, with an average litter size of 8.3 piglets and piglet mortality around 50%. Other constraints in smallholder systems were disease outbreaks. An agronomy trial showed that the forage legumes Stylo composite and porcupine joint vetch produced high DM yields, had high contents of crude protein (CP) and had potential to be used as protein sources for pigs in smallholder systems. Harvesting every 45 days produced higher DM yields than other harvesting intervals. The CP content was highest with the shortest harvesting interval, while the opposite was true for the fibre content (CF, ADF and NDF). Growth trials on the CP requirements of growing local Moo Lath pigs showed that final body weight (BW), ADG, feed intake and feed conversion ratio (FCR) did not improve above a dietary CP level of 181 g kg-1 DM for weaner pigs, 132 g kg-1 DM for grower pigs and 111 g kg-1 DM for finisher pigs. Additional growth trials with Moo Lath pigs showed that replacing soybean CP with forage leaf CP reduced DM and energy intake and ADG, but had no effect on FCR. Increasing the replacement of soybean CP with legume leaf CP had a negative linear effect on DM and energy intake, final BW and ADG, and on all carcass traits except lean meat percentage. Supplementing a traditional diet with soybean meal resulted in higher DM and energy intake, lower FCR and higher final BW and ADG

Recent advances in smartphone and wearable sensor technologies have led to a proliferation of food monitoring applications based on automated food image processing and eating episode detection, with the goal to conquer drawbacks of the traditional manual food journaling that is time consuming, inaccurate, underreporting, and low adherent. In order to provide users feedback with nutritional information accompanied by insightful dietary advice, various techniques in light of the key computational learning principles have been explored. This survey presents a variety of methodologies and resources on this topic, along with unsolved problems, and closes with a perspective and boarder implications of this field

    The characterization of feces and urine: a review of the literature to inform advanced treatment technology

    The safe disposal of human excreta is of paramount importance for the health and welfare of populations living in low income countries as well as the prevention of pollution to the surrounding environment. On-site sanitation (OSS) systems are the most numerous means of treating excreta in low income countries, these facilities aim at treating human waste at source and can provide a hygienic and affordable method of waste disposal. However, current OSS systems need improvement and require further research and development. Development of OSS facilities that treat excreta at, or close to, its source require knowledge of the waste stream entering the system. Data regarding the generation rate and the chemical and physical composition of fresh feces and urine was collected from the medical literature as well as the treatability sector. The data were summarized and statistical analysis was used to quantify the major factors that were a significant cause of variability. The impact of this data on biological processes, thermal processes, physical separators, and chemical processes was then assessed. Results showed that the median fecal wet mass production was 128 g/cap/day, with a median dry mass of 29 g/cap/day. Fecal output in healthy individuals was 1.20 defecations per 24 hr period and the main factor affecting fecal mass was the fiber intake of the population. Fecal wet mass values were increased by a factor of 2 in low income countries (high fiber intakes) in comparison to values found in high income countries (low fiber intakes). Feces had a median pH of 6.64 and were composed of 74.6% water. Bacterial biomass is the major component (25–54% of dry solids) of the organic fraction of the feces. Undigested carbohydrate, fiber, protein, and fat comprise the remainder and the amounts depend on diet and diarrhea prevalence in the population. The inorganic component of the feces is primarily undigested dietary elements that also depend on dietary supply. Median urine generation rates were 1.42 L/cap/day with a dry solids content of 59 g/cap/day. Variation in the volume and composition of urine is caused by differences in physical exertion, environmental conditions, as well as water, salt, and high protein intakes. Urine has a pH 6.2 and contains the largest fractions of nitrogen, phosphorus, and potassium released from the body. The urinary excretion of nitrogen was significant (10.98 g/cap/day) with urea the most predominant constituent making up over 50% of total organic solids. The dietary intake of food and fluid is the major cause of variation in both the fecal and urine composition and these variables should always be considered if the generation rate, physical, and chemical composition of feces and urine is to be accurately predicted

A major challenge is the accurate estimation of portion sizes for traditional foods/dishes. Objective: To systematically review published records up to 2014 describing a portion size estimation element (PSEE) applicable to dietary assessment of UK-residing ethnic minorities. Data sources, selection, extraction: Electronic databases, internet sites, and theses repositories were searched generating 5683 titles from which 57 eligible full-text records were reviewed. Data analysis: Forty-two publications aimed at minority ethnic groups (n=20) or autochthonous populations (n=22) were included. The most common PSEE (47%) were combination tools (e.g. food models and portion size lists); followed by portion size lists in questionnaires/guides (19%); image-based and volumetric tools (17% each). Only 17% PSEE had been validated against weighed data. Conclusions: When developing ethnic-specific dietary assessment tools it is important to consider customary portion sizes by sex and age; traditional household utensil usage and population literacy levels. Combining multiple PSEE may increase accuracy but such tools need validating

Body weight, height and waist circumference measurements were taken by trained health nurses according to a standard procedure (WHO, 1998). Body Mass Index (BMI) was determined among 33,055 eligible adults, while risk of abdominal obesity based on waist circumference (WC) was obtained from 32,900 individuals. In addition, peripheral muscle wasting was determined using calf circumference (CC) among 4,282 older individuals aged 60 years and above. The national prevalence of normal BMI (BMI 18.5-24.9 kg/m2) was 48.4% (47.7–49.0)*. The prevalence of underweight (BMI <18.5 kg/m2) was 8.5% (8.2–8.9), being higher in rural areas {9.8% (9.2–10.4)} than in urban areas {7.8% (7.4–8.3)}. Meanwhile, 29.1% (28.6–29.7) of the adults were overweight (BMI 25.0-29.9 kg/m2). Among the ethnic groups, Indians had the highest prevalence of overweight followed by Malays and Chinese. In addition, 14.0% (13.6–14.5) of the adults were obese (BMI >30.0 kg/m2). Women had higher obesity prevalence at 17.4% (16.7–18.0) than men at 10.0% (9.5–10.5). In terms of ethnic groups, Indians and Malays had higher obesity prevalence than other groups. By occupation category, housewives showed the highest prevalence of obesity at 20.3% (19.4–21.3). The prevalence of abdominal obesity (WC for women >88 cm and men >102 cm) was 17.4% (16.9-17.9), with women showing higher prevalence {26.0% (25.2-26.8)} than men {7.3% (6.7-7.6}. The prevalence of older persons with peripheral muscle wasting (calf circumference for women < 27.3 cm and men < 30.1 cm) was 19.9% (18.5–21.6), with a higher prevalence of men {23.7% (21.6-25.9)} than women {16.8% (15.1-18.5)} at risk of malnutrition according to this indicator. In comparison, the present national prevalence of underweight of 8.5% was three times lower than that (25.2%) reported in the NHMS II in 1996. In contrast, the prevalence of overweight hasof 14.0% in this study and 12.7% in the MANS were approximately three times the level of 4.4% found in 1996 (NHMS III). This alarming trend calls for serious re-examination of public health programmes for more effective reduction of obesity among Malaysian adults. increased from 16.6% in the NMHS II to 29.1% in the present study. The latter is comparable with the finding of 27.4% reported in another nationally representative sample involving over 5,000 adults namely, the Malaysian Adults Nutrition Survey (MANS) conducted in 2003. It is noted that obesity prevalence of 14.0% in this study and 12.7% in the MANS were approximately three times the level of 4.4% found in 1996 (NHMS III). This alarming trend calls for serious re-examination of public health programmes for more effective reduction of obesity among Malaysian adults

    The nutritional transition and diet-related chronic diseases in Asia

    The nutritional transition currently occurring in Asia is one facet of a more general demographic/nutritional/epidemiological transition that accompanies development and urbanization, marked by a shift away from relatively monotonous diets of varying nutritional quality toward an industrialized diet that is usually more varied, includes more preprocessed food, more food of animal origin, more added sugar and fat, and often more alcohol. This is accompanied by shift in the structure of occupations and leisure toward reduced physical activity, and leads to a rapid increase in the numbers of overweight and obese. The accompanying epidemiological transition is marked by a shift away from endemic deficiency and infectious diseases toward chronic diseases such as obesity, adult-onset diabetes, hypertension, stroke, hyperlipidaemia, coronary heart disease, and cancer. Obesity is now a major public health problem in Asia. Obesity is a problem of the urban poor as well as the rich, and the urban poor have the added predisposing factors associated with low birthweight. Costs of chronic disease are estimated for China and Sri Lanka. Diet-related chronic disease is projected to increase and dietary factors (principally overweight) will account for an increased share of chronic disease, and childhood factors will decline in significance. Few program and policy options to address these issues have been undertaken in Asia. Agricultural policy is important, and the relatively cheap availability of vegetable oil may have had dramatic (adverse) dietary effects in Asia. Price policy has considerable potential, in particular the pricing of oils. Promoting a traditional diet has been quite helpful in holding down fat intake and obesity in Korea. Health promotion efforts in Mauritius succeeded in reversing several adverse trends contributing to coronary heart disease. Thailand has successfully used mass media for other health promotion efforts and is moving to pilot schemes in the area of chronic disease.
