    Estudi de la seguretat i privadesa dels historials mèdics en format electrònic

    El PFC s'emmarca dins de l'àrea de seguretat informàtica. D'acord amb la legislació, la informació de caràcter personal ha de ser protegida ja que es tracta d'informació molt sensible. Cal aplicar mesures que garanteixin la seguretat i privadesa de la informació. En el cas específic de la informació relativa a les dades de salut de les persones, el nivell de protecció ha de ser encara més elevat. A més, en el cas dels historials mèdics electrònics la informació es transmet per xarxes de comunicacions per la qual cosa cal aplicar mesures addicionals de seguretat per tal de garantir la seguretat i privadesa de la informació. L'objectiu d'aquest PFC és estudiar la legislació actual i extreure els requeriments de seguretat i privadesa exigits, per tal de determinar el grau de compliment d'aquests requeriments per part de les implementacions existents.El PFC se enmarca dentro del área de seguridad informática. De acuerdo con la legislación, la información de carácter personal debe ser protegida ya que se trata de información muy sensible. Hay que aplicar medidas que garanticen la seguridad y privacidad de la información. En el caso específico de la información relativa a los datos de salud de las personas, el nivel de protección debe ser aún más elevado. Además, en el caso de los historiales médicos electrónicos la información se transmite por redes de comunicaciones por lo que hay que aplicar medidas adicionales de seguridad para garantizar la seguridad y privacidad de la información. El objetivo de este PFC es estudiar la legislación actual y extraer los requerimientos de seguridad y privacidad exigidos, para determinar el grado de cumplimiento de estos requerimientos por parte de las implementaciones existentes.The objective of this PFC is to study existing legislation and remove the security and privacy requirements required to determine the degree of compliance with these requirements from existing implementations

    A Research Platform for Artificial Neural Networks with Applications in Pediatric Epilepsy

    This dissertation established a state-of-the-art programming tool for designing and training artificial neural networks (ANNs) and showed its applicability to brain research. The developed tool, called NeuralStudio, allows users without programming skills to conduct studies based on ANNs in a powerful and very user friendly interface. A series of unique features has been implemented in NeuralStudio, such as ROC analysis, cross-validation, network averaging, topology optimization, and optimization of the activation function’s slopes. It also included a Support Vector Machines module for comparison purposes. Once the tool was fully developed, it was applied to two studies in brain research. In the first study, the goal was to create and train an ANN to detect epileptic seizures from subdural EEG. This analysis involved extracting features from the spectral power in the gamma frequencies. In the second application, a unique method was devised to link EEG recordings to epileptic and non-epileptic subjects. The contribution of this method consisted of developing a descriptor matrix that can be used to represent any EEG file regarding its duration and the number of electrodes. The first study showed that the inter-electrode mean of the spectral power in the gamma frequencies and its duration above a specific threshold performs better than the other frequencies in seizure detection, exhibiting an accuracy of 95.90%, a sensitivity of 92.59%, and a specificity of 96.84%. The second study yielded that Hjorth’s parameter activity is sufficient to accurately relate EEG to epileptic and non-epileptic subjects. After testing, accuracy, sensitivity and specificity of the classifier were all above 0.9667. Statistical tests measured the superiority of activity at over 99.99 % certainty. It was demonstrated that 1) the spectral power in the gamma frequencies is highly effective in locating seizures from EEG and 2) activity can be used to link EEG recordings to epileptic and non-epileptic subjects. These two studies required high computational load and could be addressed thanks to NeuralStudio. From a medical perspective, both methods proved the merits of NeuralStudio in brain research applications. For its outstanding features, NeuralStudio has been recently awarded a patent (US patent No. 7502763)

    Symbolic Dynamics Analysis: a new methodology for foetal heart rate variability analysis

    Cardiotocography (CTG) is a widespread foetal diagnostic methods. However, it lacks of objectivity and reproducibility since its dependence on observer's expertise. To overcome these limitations, more objective methods for CTG interpretation have been proposed. In particular, many developed techniques aim to assess the foetal heart rate variability (FHRV). Among them, some methodologies from nonlinear systems theory have been applied to the study of FHRV. All the techniques have proved to be helpful in specific cases. Nevertheless, none of them is more reliable than the others. Therefore, an in-depth study is necessary. The aim of this work is to deepen the FHRV analysis through the Symbolic Dynamics Analysis (SDA), a nonlinear technique already successfully employed for HRV analysis. Thanks to its simplicity of interpretation, it could be a useful tool for clinicians. We performed a literature study involving about 200 references on HRV and FHRV analysis; approximately 100 works were focused on non-linear techniques. Then, in order to compare linear and non-linear methods, we carried out a multiparametric study. 580 antepartum recordings of healthy fetuses were examined. Signals were processed using an updated software for CTG analysis and a new developed software for generating simulated CTG traces. Finally, statistical tests and regression analyses were carried out for estimating relationships among extracted indexes and other clinical information. Results confirm that none of the employed techniques is more reliable than the others. Moreover, in agreement with the literature, each analysis should take into account two relevant parameters, the foetal status and the week of gestation. Regarding the SDA, results show its promising capabilities in FHRV analysis. It allows recognizing foetal status, gestation week and global variability of FHR signals, even better than other methods. Nevertheless, further studies, which should involve even pathological cases, are necessary to establish its reliability.La Cardiotocografia (CTG) è una diffusa tecnica di diagnostica fetale. Nonostante ciò, la sua interpretazione soffre di forte variabilità intra- e inter- osservatore. Per superare tali limiti, sono stati proposti più oggettivi metodi di analisi. Particolare attenzione è stata rivolta alla variabilità della frequenza cardiaca fetale (FHRV). Nel presente lavoro abbiamo suddiviso le tecniche di analisi della FHRV in tradizionali, o lineari, e meno convenzionali, o non-lineari. Tutte si sono rivelate efficaci in casi specifici ma nessuna si è dimostrata più utile delle altre. Pertanto, abbiamo ritenuto necessario effettuare un’indagine più dettagliata. In particolare, scopo della tesi è stato approfondire una specifica metodologia non-lineare, la Symbolic Dynamics Analysis (SDA), data la sua notevole semplicità di interpretazione che la renderebbe un potenziale strumento di ausilio all’attività clinica. Sono stati esaminati all’incirca 200 riferimenti bibliografici sull’analisi di HRV e FHRV; di questi, circa 100 articoli specificamente incentrati sulle tecniche non-lineari. E’ stata condotta un’analisi multiparametrica su 580 tracciati CTG di feti sani per confrontare le metodologie adottate. Sono stati realizzati due software, uno per l’analisi dei segnali CTG reali e l’altro per la generazione di tracciati CTG simulati. Infine, sono state effettuate analisi statistiche e di regressione per esaminare le correlazioni tra indici calcolati e parametri di interesse clinico. I risultati dimostrano che nessuno degli indici calcolati risulta più vantaggioso rispetto agli altri. Inoltre, in accordo con la letteratura, lo stato del feto e le settimane di gestazione sono parametri di riferimento da tenere sempre in considerazione per ogni analisi effettuata. Riguardo la SDA, essa risulta utile all’analisi della FHRV, permettendo di distinguere – meglio o al pari di altre tecniche – lo stato del feto, la settimana di gestazione e la variabilità complessiva del segnale.
