    Coronary spiral CT

    This dissertation describes the use and clinical potential of ECG gated multislice spiral computed tomography in patients with coronary artery disease. First the use of other non invasive cardiac imaging, i.e. the previously mentioned electron beam CT and magnetic resonance imaging is reviewed (chapter 2.1). Part 3 contains studies related to the characteristics of multislice spiral CT for the imaging of the heart and coronary arteries, and the diagnostic potential of ECG gated spiral CT coronary angiography to detect and visualize obstructive coronary artery disease in symptomatic patients, using conventional coronary angiography as the standard of reference. Part 4 is focussed on the feasibility and characteristics of coronary wall imaging by CT, including the assessment of non calcified atherosclerotic plaque material. The usefulness of contrast enhanced multislice spiral CT in symptomatic patients who previously underwent coronary artery bypass grafting and percutaneous coronary intervention with stent implantation is discussed in part 5. Finally, the first results with the latest generation 16 slice computed tomography scanners and patient preparation with ~ receptor blockers are described in part 6, including a review and future outlook on the continuing development and clinical use of non invasive coronary angiography with spiral computed tomography

    Gadofosveset (Vasovist®, Bayer Schering Pharma AG, Berlin/Germany) is the first intravascular contrast agent approved for use with magnetic resonance angiography in the European Union, Switzerland, Turkey, Canada, and Australia. Gadofosveset reversibly binds to albumin providing extended intravascular enhancement compared wth existing extracellular magnetic resonance contrast agents. Prior to approval, gadofosveset underwent extensive testing to evaluate the safety and efficacy of the drug; the clinical trials show that gadofosveset-enhanced magnetic resonance angiography (MRA) is safe and well tolerated in patients with vascular disease and effective for the detection of vascular stenosis and aneurysms gadofosveset has the potential to open new horizons in diagnostic MRA by increasing the spatial resolution and the robustness of MRA examinations and facilitating the examination of multiple vascular beds

    AbstractIndications for and experience with placement of endovascular stent grafts in the thoracic aorta are still evolving. Recent advances in imaging technologies have drastically boosted the role of pre-procedural imaging. The accepted diagnostic gold standard, digital subtraction angiography, is now being challenged by the state-of-the-art computed tomography angiography (CTA), magnetic resonance angiography (MRA) and trans-oesophageal echocardiography (TEE). Among these, technological advancements of multidetector computed tomography (MDCT) have propelled it to being the default modality used, optimising the balance between spatial and temporal resolutions and invasiveness. MDCT angiography allows the comprehensive evaluation of thoracic lesions in terms of morphological features and extent, presence of thrombus, relationship with adjacent structures and branches as well as signs of impending or acute rupture, and is routinely used in these settings.In this article, we review the current state-of-the-art radiological imaging for thoracic endovascular aneurysm repair (TEVAR), especially focusing on the role of MDCT angiography. After analysing the technical aspects for optimised imaging protocols for thoracic aortic diseases, we discuss pre-procedural determinants of candidacy, and how to formulate interventional plans based on cross-sectional imaging
