Empatía y humanización en enfermería, clave del cuidado holístico: imperativo ético en la atención integral del paciente con VIH/SIDA

Abstract

Nursing care for people living with HIV has evolved significantly since the beginning of the epidemic, shifting from a biomedical approach to a person-centered paradigm. However, although biomedical and pharmacological advances have made HIV a chronic condition that is manageable, barriers related to stigma, discrimination, and dehumanized care still persist, undoubtedly affecting treatment adherence and quality of life. This essay critically analyzes the integration of empathy and humanization as imperatives and essential components of holistic nursing care for people with HIV. Through a reflection based on the available evidence, it problematizes the tension between the effectiveness of the technical dimension and the human dimension of care, questioning whether clinical competence without empathy is sufficient to achieve optimal health outcomes. It argues that empathy is not an optional attribute but a fundamental ethical requirement that directly influences adherence to highly active antiretroviral therapy (HAART), the reduction of internalized stigma, and mental health outcomes. Systemic barriers such as work overload and burnout syndrome are recognized, and finally, strategies are integrated to combine humanization and efficiency in the daily practice of holistic care.El cuidado de enfermería para las personas que viven con VIH/SIDA ha evolucionado a desde los inicios de la epidemia de manera significativa, han transitado de un enfoque biomédico hacia un paradigma centrado en la persona. Sin embargo los avances biomédicos y farmacológicos que si bien es cierto han convertido al VIH en una condición que encaja en la cronicidad y esta si es manejable, aún persisten barreras relacionadas con estigma, discriminación y cuidado deshumanizado que sin duda inciden en la adherencia al tratamiento y en la calidad de vida. En este ensayo se analiza críticamente la integración de la empatía y la humanización visto como un imperativo, claves o componentes esenciales del cuidado holístico de enfermería en personas con VIH. A través de una reflexión fundamentada en la evidencia disponible, se problematiza la tensión entre la eficacia de la dimensión técnica y la dimensión humana del cuidado, cuestionando si la competencia clínica sin empatía es suficiente para lograr resultados óptimos en salud. Se argumenta que la empatía no es un atributo opcional sino un requisito ético fundamental que influye directamente en la adherencia al tratamiento antirretroviral altamente activo (TARAA), la reducción del estigma internalizado y los resultados en salud mental. Se reconocen las barreras sistémicas como la sobrecarga laboral y el síndrome de burnout, por último se integran estrategias para integrar humanización y eficiencia en la práctica diaria del cuidado holístico
