Thrombus in atrial transit is an atypical presentation of pulmonar thromboembolism, occurring mainly in the right cavities. We present the clinical case of an 86-year-old man who came to the Emergency department due to deterioration in his general condition, edema in his lower limbs, and worsening of his baseline dyspnea in the context of intermediate-risk pulmonary thromboembolism. In this case, an echocardiography was important, as it revealed a thrombus in transit in the right cavities, indicating a high risk of recurrent pulmonary thromboembolism and clinical worsening.El trombo en tránsito auricular es una forma de presentación atípica de tromboembolismo pulmonar, presentándose mayoritariamente en las cavidades derechas. Presentamos el caso clínico de un varón de 86 años que acudió a la Urgencias por deterioro del estado general, edemas en miembros inferiores y empeoramiento de su disnea basal en contexto de TEP de riesgo intermedio. Fue importante en este caso la realización de ecocardioscopia, donde se objetivó un trombo en tránsito en cavidades derechas lo que indicaba un elevado riesgo de TEP de repetición y empeoramiento clínico
