It contains variables collected through a questionnaire on socio-demographic information, health-related knowledge; intentions, agency, and perceived social support for behaviour change; engagement with health services; prevalence of health conditions and health-related behaviours screened; improvements in previously-diagnosed health and well-being conditions by the time of follow-up, and referral attendance
