Coronary or myocardial muscular bridging is a congenital anomaly that describes the intramyocardial segment of the coronary artery, called a tunneled artery. Myocardial bridging can occur in any artery, however, 67-98% occurs in the anterior descending artery. Diagnosis is usually incidental at angiography or autopsy; most individuals are unaware that they are carriers of this congenital anomaly until they present clinical manifestations that may suggest an acute coronary syndrome. We present the case of a patient with ST-elevation acute coronary syndrome who was incidentally diagnosed with myocardial bridging, successfully treated with calcium antagonist.El puente muscular coronario o miocárdico es una anomalía congénita que describe el segmento intramiocárdico de la arteria coronaria, denominada arteria tunelada. El puente miocárdico puede presentarse en cualquier arteria, sin embargo, el 67-98 % ocurre en la descendente anterior. El diagnóstico suele ser incidental en la angiografía o la autopsia, la mayoría de los individuos no saben que son portadores de esta anomalía congénita hasta que presentan manifestaciones clínicas que pueden sugerir un síndrome coronario agudo. Presentamos el caso de un paciente con síndrome coronario agudo con elevación del ST al que se diagnosticó incidentalmente un puente miocárdico, tratado con éxito con antagonista del calcio.
