Das Ziel dieser Pilotstudie bestand darin, anhand von Untersuchungs- und
Fragebögen die craniomandibulären Befunde bei insgesamt 44 Kindern und
Jugendlichen mit gesicherter juveniler idiopathischer Arthritis im
Therapieverlauf mit einer Äquilibrierungs-schiene (N = 33) oder okklusalen
Aufbissen (N = 11) zu ermitteln. Die Pilotstudie sollte zeigen, ob die
kieferorthopädische Therapie eine Schmerzlinderung und Abnahme der
Kiefergelenkbeschwerden bei JIA-Patienten unter antirheumatischer Therapie
bewirkte. Alle Patienten wurden zuvor nicht kieferorthopädisch behandelt
worden. Von allen Patienten lagen mindestens drei CMD-Screenings (klinische
Untersuchung) und drei der Anamnesebögen (Fragebogen) zur Auswertung vor.
Einmalig beantworteten 33 von 44 Patienten zusätzlich einen Schmerz-Fragebogen
(Graded-Chronic-Pain-Status). Aufgrund der kleinen Patientenzahl wurden neben
der beschreibenden Statistik nichtparametrische statistische Testverfahren
verwendet. Die Auswertung des GCP-Status zeigt, dass die befragten Kinder und
Jugendlichen mit JIA häufig an chronischen Schmerzen leiden. Sowohl die
Therapiegruppe mit Äquilibrierungsschiene als auch die mit okklusalen
Aufbissen geben bei der Auswertung des Anamnesebogens und des CMD-Screenings
eine verringerte Anzahl eingeschränkter Aktivitäten im Therapie-verlauf an.
Eine statistisch signifikante Verbesserung der Symptome vom Beginn bis zum
letzten Untersuchungstag konnte für beide Therapiegruppen nachgewiesen werden.
Ein eindeutiger Behandlungserfolg mittels okklusaler Therapie konnte klinisch
nicht ermittelt werden, da die JIA-Patienten schmerzlindernde Medikamente (z.
B. Ibuprofen) einnahmen. Aufgrund der langen Tragedauer kann angenommen
werden, dass am letzten Untersuchungstag die medikamentöse antirheumatische
Therapie und das hohe Regenerationspotenzial im Wachstum von JIA-Patienten
ebenfalls zu einer Verringerung der CMD-Symptome beigetragen haben. Da bisher
keine einheitlichen Vorgehensweisen existieren, ist die Forderung nach einem
allgemein anerkannten diagnostischen und therapeutischen Protokoll zu stellen.
Es sollte alle sechs Monate eine Kontrolle der Kiefergelenke bei juvenilen
Patienten mit rheumatischer Erkrankung erfolgen. Die vorliegende Arbeit mit
ihrem sehr heterogenen Patientengut hinsichtlich der Altersverteilung stellt
eine Pilotstudie dar und dient als Basis für weitere Untersuchungen in der
okklusalen Therapie bei rheumatischer Erkrankung im Kiefergelenk und einer
eingegrenzten Altersstruktur.The purpose of this pilot study was to determine, on the basis of examinations
and questionnaires, the craniofacial findings in children and adolescents with
juvenile idiopathic arthritis during the course of treatment with occlusal
bite planes (N = 33) or blocks (N = 11). It was aim of this pilot study
determine effectively the occlusal therapy reduced the pain and decreased
craniomandibular disorders will simultaneously being treated by antirheumatic
drugs. All patients had not previously been treated orthodontical. All 44
patients were examined at least three times using CMD-screening (clinical
investigation) and answered least three anamnessis questionnaires. 33 of 44
patients additionally answered pain questionnaires (Graded-Chronic-Pain-
Status) during TMJ consultation. Nonparametric statistical tests were used
because of the small number of patients in addition to descriptive statistics.
The analysis of the GCP-Status that was only answered once shows that the
surveyed children and adolescents with JIA frequently suffer from chronic
pain. The evaluation of the medical history sheet and the CMD-screening show a
reduced number of restricted activities during the course of therapy in both
groups. A statistically significant improvement from the first to the last
examination was observed for both treatment groups. A unique treatment success
by occlusal therapy couldn´t be determined clinically, because all rheumatic
patients had also received medical treatment (i.e. Ibuprofen). Due to the long
period of wear one can assume that on the last day of examination medicinal
antirheumatic therapy and high regenerational potential during the growth
period of JIA patients play a part in the decrease of CMD-symptoms. As there
are as yet no standard procedures, generally accepted diagnostic and
therapeutic protocols should be developed. The results of the present study
strongly suggest that regular monitoring of the temporomandibular joints of
juvenile patients with rheumatic disease should be carried out every six
months. The present paper showing a heterogeneous patient population in terms
of age distribution is a pilot study and serves as a basis for further
investigations into occlusal therapy of JIA patients with specific subgroups
and narrow age corridors
