    Comparison of oral and gut microflora in octogenarians and centenarians in relation to independence

    松本歯科大学博士(歯学)2022甲第251号application/pdfABSTRACT This study compared the differences in salivary and fecal bacterial flora between 80- and 100-year-old people in different independent states to provide a reference for the bacterial composition of the digestive tract in older people of different ages. Saliva and fecal samples were collected from eight centenarians and 10 octogenarians whose Barthel index (BI) is less than 60,which means poor independence, as well as 10 independent octogenarians from nursing homes.A dentist counted the number of teeth, and evaluated the denture and nutritional status. Bacterial DNA was extracted and multiplex 16S rRNA (V3–V4)sequencing was performed using the Illumina MiSeq platform.Composition and diversity were analyzed by LEfSe,student’s t-test, weighted PcoA distance, and Anosim analysis. Older people who were more independent had significantly better nutritional intake than those who were less independent, and this difference was not reflected in age.The digestive tract of well independent seniors has more normal flora,which also reflects the link between general health and gut flora.While the saliva of less independent seniors contains more pathogenic bacteria,which may be a result of their inability to effectively clean their oral hygiene due to their poor functional independence.In addition, Akkermansia, a genus of bacteria commonly considered to be probiotics,was more frequently found in the stools of the less independent eighty-year-olds.There was no significant difference in the intestinal microbiota of 80-year-old and 100-year-old individuals with low functional independence in the β diversity that constitutes the microbiota.On the other hand,there was a significant difference in β diversity in the salivary flora.The reason for this was thought to be the difference in the number of remaining teeth between 80-year-old and 100-year-old subjects.In addition, there was a significant difference in β diversity in both the intestinal and salivary microbiota between the 80-year-old group with low functional independence and the normal 80-year-old group.From the above, it was clarified that functional independence affects the diversity that constitutes the salivary and gut microbiota.In addition, it was suggested that functional independence affects the diversity of salivary and intestinal microbiota more than age.doctoral thesi

    application/pdfSummary We encountered a case of double inferior vena cava (DIVC) in a 76 year old male cadaver during anatomy practice at Matsumoto Dental University in 2015. The left and right internal iliac veins drained into the interiliac vein ran horizontal in front of the cape angle and joined the left and right external iliac veins. Then the inferior vena cava on each side originated.The width of the origin of the inferior vena cava were 13.7 mm and 11.6 mm on the right and left side, respectively. Based on the previous DIVCʼs classifications, we attempted to classify the DIVC and iliac venous system by combining the direction of the internal iliac veins (lateral (l), vertical (s), and medial (m)), the inclination of the interiliac veins (lower left to upper right (L to R), horizontal (H), and lower right to upper left (R to L)),and the width of the right and left IVC (RIVC>LIVC, RIVC=LIVC, and RIVCLIVC, H, l-l. In this case, hemi-azygos vein was not found, and the height of the left kidney was lower than the right one. We therefore investigated the co-existence of the venous system and the height of the kidney to the 53 cases and referred to the embryological relationship between them. The present classification provides basic knowledge for the clinical anatomy and surgery of the pelvic region.journal articl

    経管栄養の要介護高齢者の口蓋細菌叢に与える剥離上皮膜の影響

    松本歯科大学博士(歯学)2021甲第242号application/pdf[緒言]経管栄養法は経口摂取が困難な要介護高齢者の栄養維持のために広く用いられており,日本の要介護高齢者における経管栄養の実施割合は,介護療養病床で62.2%,病院で63.3%とされている1).経管栄養で意識障害が伴うような非経口摂取者は,常時開口がみられ,舌の低活動により,舌を介して唾液が口腔粘膜に付着せず,口腔粘膜が乾燥する.その結果,口腔粘膜や歯面に剥離上皮膜が形成されることがある2,3).経管栄養患者において,口蓋で40.0%,舌背は42.5%,歯面は20%,頬粘膜は17.5%の者に剥離上皮膜が観察されたとする報告がある2,3).さらに口腔に剥離上皮膜がある者のうち70%の者が咽頭粘膜上に剥離上皮膜が観察されている4).剥離上皮膜は,重層扁平上皮由来の角質変性物と唾液のムチンが主体となり,部分的に炎症性細胞や細菌の集積を認め,痰や痂疲とは異なる5,6).剥離上皮膜の一部は,口腔粘膜と結合しており,剥離上皮膜の除去時に出血する可能性がある7).また剥離上皮膜が咽頭・喉頭に落下し,気道を閉塞させ,呼吸抑制や窒息となる危険性がある8,9).さらに剥離上皮膜形成者に有意に発熱が認められ10),経管栄養と肺炎との関連が報告11-13)されていることから,発熱原因として,除去時の出血部位からの局所感染や頭・喉頭に落下した剥離上皮膜を介する口蓋粘膜細菌の呼吸器感染が考えられる.経管栄養者の口腔内細菌叢として,舌苔からCorynebacterium 属,Peptostreptococcus 属,Fusobacterium 属が経口摂取者より有意に多く検出されたとする報告14)や,口蓋・舌背・咽頭からStreptococcus 属,Rothia 属,Neisseria属が経口摂取者と比べ有意に多く検出されたとの報告15)がある.経管栄養者の口腔内細菌叢は経口摂取者と異なり14,15),経口摂取の有無が細菌叢に影響を及ぼすとされる15).しかし,剥離上皮膜が存在する経管栄養者の口腔粘膜には,う蝕原因菌,歯周疾患関連菌,全身へ影響する細菌の存在は,明らかにされておらず,細菌学的為害性は不明である.今回,経管栄養の要介護高齢者の口腔内のうち,剥離上皮膜が認められることの多い口蓋を対象として,次世代シークエンス16s rRNAメタゲノム解析にて口蓋細菌叢を検索するとともに,経管栄養者の口蓋細菌叢に関連する因子について検討した.さらに剥離上皮膜の有無で口蓋細菌を比較検討した.doctoral thesi
