[Abstract] There are many studies of a main railroad, Sanyo Railway Co.Ltd. which existed in the Meiji period. They are specific studies on history of management, economic history, political history, historical geography, industrial archaeology of the Railway. However, few studies are systematized. The aim of this paper is to introduces articles that have been pubicized about Sanyo Railway Co.Ltd. and to demonstrate the assignments yet to be done(hereafter), checking my own business historical and entrepreneurial studies against others

    publisher[目次] 1. はじめに 2. 医療供給の現状 3. 関西における医療の生産性 3.1. 関西の医療の現状 3.2. 関西の生産性について 4. おわりに[要旨]本稿では, 関西地域における医療の生産性の現状を, 病院の規模と機能分化が進んでいるかという2点で検討をおこなった. 本稿でわかったことは以下のとおりである. 第1に, 日本において, 90年代前半から現在にいたるまで医療供給体制は基本的な構造変化はなされていない. 病床規制の緩和によって民間病院の新規参入と非効率な病院の淘汰や整理統合によって病院の生産性を高めることが求められる. 第2に, 関西は, 全国, 関東, 中部と比較して医師数と病床数が多いことから, 医療供給としての潜在力の高さが窺える. しかし病院の機能分化は全国と同様に, 関西でも進んでいない. これは日本の医療の生産性が低い要因の1つであり, 関西においても病院と病床の整理・統合, 淘汰は進んでいない. 第3に, 関西は全国と比較して人口あたりの一般病床数が多く, 療養病床数が少ない. 関西では, 病床利用率の観点から, 一般病床の活用の余地があり, 一般病床のより効率的な利用を進め, 医療分野での生産性を高める余地があることがわかった. [Summary] In this paper, I examined the present conditions of the medical productivity in the Kansai area from a standpoint whether the scale and functional differentiation of hospitals have made progress. What this paper found are as follows. First, the Japanese medical supply system has not changed the basic structure from the early 90s to the present. The new entry of private hospitals may be expected by the relaxation of the sickbed regulation. The productivity of hospitals may also be enhanced by the selection and consolidation of inefficient hospitals. Second, in Kansai, the number of medical doctors and hospital beds is greater in comparison with those of national, Kanto and Chubu levels. These indicate that the Kansai area has great potential as medical supplier. However, the functional differentiation of hospitals has not advanced in Kansai just like other areas of the country, either. In this regard, it may be concluded that the Kansai area has not yet made much progress in the field of integration and liquidation of hospitals, which is identified as one of the factors causing low medical productivity in Japan. Third, there is a more general hospital bed capacity per capita in Kansai than the national level, but it is found that there are a fewer hospital beds for medical treatment. Kansai should take some measures to make more effective use of general sickbeds. From the viewpoint of sickbed availability rate, there is room for general sickbeds to be made good use of. Japan has more room to enhance the productivity in the medical field

    〈Case Reports〉Catheter Ablation of Dual-Loop Macro-Reentrant Tachycardia under Electroanatomical Mapping Guidance Using Multiple-Electrode Sampling Catheter in Enlarged Right Atrium after Repair of Tetralogy of Fallot : A Case Report

    publisher〈Abstract〉We describe a 47-year-old woman who underwent tetralogy of Fallot repair at the age of 6 years and radiofrequency catheter ablation of macro-reentrant right atrial tachycardia (rt-AT) 41 years after surgery. A three-dimensional electroanatomical mapping system helped to determine the macroreentrant circuit. Although the right atrium was enlarged by the burden on the right side of the heart, multiple samplings on contact bipolar electrograms generated using a multipolar catheter enabled the production of high-density activation and voltage maps (429 points) within 30 minutes. The activation map revealed a dualloop reentrant circuit surrounding the superior and inferior vena cava, and clarified a critical isthmus located in the low-voltage area on the postero-lateral side of the right atrium. The rt-AT was terminated by ablation of the isthmus without damaging the nearby right phrenic nerve

    publisher〈Abstract〉Stellate ganglion block was performed on a 54-year-old Japanese man who had been undergoing outpatient therapy for a chief complaint of pain in the right shoulder girdle. The patient developed dyspnea approximately 45 min after the procedure, but his life was saved by tracheal intubation and artificial respiratory management. Based on the clinical course, Quincke's edema was diagnosed. Patients undergoing SGB need to be instructed to immediately contact a medical institution if any abnormalities manifest

[Abstract] Using a three-axis acceleration sensor, a micro-computer together with a PC, I experimentally devised a wearable system specialized in boxing. This system is characterized by high portability and high-cost-performance. By measuring acceleration in 3 axial directions using this system, it is possible to measure hitting rhythm, a boxer's physical strength and an average of blows and a kind of blows in boxing

    Novel method to induce chondrio-regeneration using fine-cut, block-shaped microcartilage in vivo

    publisher[抄録] 軟骨組織の再生修復能は極めて低く, かつ軟骨欠損部への軟骨片移植の臨床成績はいまだ不良である. 近年では幹細胞を用いた軟骨再生誘導法が開発され臨床応用されつつある. しかし幹細胞を単離する従来法にも手技上のコストや手技に伴う細胞障害等の諸問題が存在している. また以前の研究結果より, 幹細胞を含む軟骨細胞塊を用いた軟骨再生が可能であることが分かっている. そこで本研究では従来の方法を発展させ, より実際的な新規軟骨再生誘導法の開発を試みた. まず軟骨を低侵襲的に微細加工する装置を作製し, 再生誘導に適したマイクロ軟骨サイズの検討を行った. その結果, 細胞利用率が最も高いマイクロ軟骨の至適サイズは, 100~400μm前後であると推測された. 次に塩基性線維芽細胞増殖因子(basic fibroblast growth factor/bFGF)徐放化システムを軟骨移植に併用し, 本サイトカインが移植後軟骨片に及ぼす影響を評価した. その結果, 自家軟骨移植時にbFGF徐放化システムを併用することで, SOX5活性化を伴う軟骨細胞増殖が促進されることが示された. さらにマイクロ軟骨を吸収性足場材料に接着させ, マイクロ軟骨・足場材料・bFGF徐放化システムを組み合わせた移植を行い, 細胞培養行程を介さない新規軟骨組織再生誘導を試みた. その結果, bFGF徐放化システムによるbFGF拡散の程度はマイクロ軟骨のサイズに依存し, 比較的小型の100μm群においてSOX5発現亢進を介した軟骨再生が最も効率よく行われることが示唆された. 以上の結果より, 微細加工装置により作製したマイクロ軟骨にサイトカインを併用することで軟骨組織の分化増殖を促しうることが示され, 培養行程を介さない新規軟骨再生誘導法の可能性が示唆された
