A Case of Staged Surgical Treatment for Acute Aortic Dissection with Lower Limb Ischemia After Abdominal Aortic Graft Replacement

腹部大動脈人工血管置換術後に急性大動脈解離を発症した場合, 人工血管の中枢吻合部で偽腔が盲端となり, 破裂や真腔の圧排による下肢虚血を来すことがまれにある．症例は83歳, 男性．大動脈弁置換術と腹部大動脈人工血管置換術の既往があった．5日前から出現した下肢のしびれ, 背部痛のため前医整形外科に入院となっていたが, CTで急性A型大動脈解離の診断となり, 当院に搬送された．上行大動脈が68㎜に拡大しており, さらに腹部人工血管の中枢側で偽腔が盲端となり, 真腔を著明に圧排し, 下肢虚血を生じていた．転院当日にステントグラフト内挿術を施行し, 腎動脈直下にAortic Extenderを留置することで下肢虚血を解除することができた．翌日に上行大動脈置換術を施行し, 術後14日目に自宅退院となった．術後4年経過しているが, 問題なく経過している．腹部大動脈人工血管置換術後遠隔期に, 急性A型大動脈解離を発症し, 下肢虚血を合併した症例に対し, ステントグラフト併用手術により良好な結果を得られた．An 83-year-old man with a history of aortic valve replacement and abdominal aortic graft replacement was admitted to the orthopedic department of a previous hospital due to lower limb numbness and back pain that had persisted for five days. Computed tomography revealed an acute Stanford type A aortic dissection, and he was transferred to our hospital for surgical management. The ascending aorta was dilated to 68 mm, and the false lumen formed a blind end at the proximal anastomosis of the abdominal graft, causing marked compression of the true lumen and resulting in lower limb ischemia.　 On the day of transfer, endovascular stent grafting was performed, and two aortic extenders were placed just below the renal arteries, successfully relieving the lower limb ischemia.　On the following day, ascending aortic replacement was performed. The patient was discharged home on postoperative day 14. Four years have passed since the surgery, and he remains in good condition without complications. This case illustrates that a staged surgical approach combining endovascular stent grafting and open ascending aortic replacement was effective in treating acute type A aortic dissection complicated by lower limb ischemia in a patient with a prior abdominal aortic graft.departmental bulletin pape

Organizing Evaluation Items and Examining Inter-Rater Agreement to Improve the Quality of OSCE in Nursing Initiatives from 2022 to 2024

令和6年度の看護学OSCEは令和4年度に実施した看護学OSCEの評価を踏まえて領域別課題の評価項目の偏りを是正した。本報告は、これら実施上の改善内容を整理し、令和6年度に実施した看護学OSCEの評価について評価者間一致度の観点から検討することにより、実施上の課題を抽出し検討することを目的とした。令和4年度データの分析から明らかになった課題に対して、次の3つのStepに沿って補正を進めた。Step1：令和4年度看護学OSCE実施の評価項目の傾向に基づく令和6年度の評価項目の調整、Step2：本学Diploma Policy（DP）との対応、Step3：評価者間のばらつきに対する改善。令和6年度看護学OSCE受験者59名について受験者1名につき試験評価者2名が別々に評価したデータを対象とし、その2名の評価者間一致度について、級内相関係数（intra-class correlation coefficients; 以下ICC）を用いて検証した。令和6年度の総スコアにおけるICCは、0.172～0.839と一致度にばらつきが認められ、令和4年度から令和6年度にかけての改善がみられたのは1領域のみであった。また評価項目をStep１～3に基づき分類しκ係数により評価した。看護技術（アセスメント）や看護技術（適切な実施）に関する分類については多くの領域別課題においてある程度の一致度が認められた。しかしながら複数の分類を用いた検討から、出題領域ごとの一致度にはばらつきが認められ、特に評価項目が少ない内容では一致度の検証に限界があった。今後も領域別課題の修正に加え、評価項目や評価基準の明瞭性を高め、評価者間の事前打ち合わせの強化など工夫が必要であると考える。departmental bulletin pape
