39ª SEMANA CIENTÍFICA DO HOSPITAL DE CLÍNICAS DE PORTO ALEGRE              100  Clin Biomed Res 2019; 39 (Supl.)     Results: During the 24 months of analysis, 1206 patients comprised the dataset. In this series, there were 43 postoperative deaths (3.56%). The presence of complications was evaluated by POMS scale on postoperative day 3 and 7. A total of 485 (40.4%) patients suffered at least one complication at day 3 and 208 (17.3%) at day 7. Postoperative morbidity occurred more frequently in patients with higher probability of death. The relative risk of any complication on the 3rd postoperative day increased significantly at a greater SAMPE risk class. Reliable confidence intervals related to renal, infectious and gastrointestinal domains of POMS scale confirmed the increased risk of complications in higher levels of SAMPE risk classes. Therefore, a post-surgery care pathway called Extended Care in High-Risk Surgical Patients (EXCARE) in the form of coordinated multiprofessional actions (patient-centered risk assessment, specialized care in the post-anesthetic care unit, and also in the surgical wards performed by the nursing, anesthesia, clinic and surgery teams) dedicated to this population, with the aim of improving the postoperative outcomes, was created.  Beyond clinical effectiveness, it was found necessary to evaluate costs associated to this intervention. In these context, the time-driven activity-based costing (TDABC) model allows measurement of the real cost of care following patient flow and identifying all resources (professional, structure or technology) consumed. It combines process mapping from industrial engineering with the most modern approach for accurate and transparent patient-level costing. Methods: Study population includes analysis of 430 consecutive patients, submitted to surgeries in Hospital de Clínicas de Porto Alegre (HCPA) and considered to be at high-risk (>5%) of 30-day death by the SAMPE Risk Model. Cost analysis will use the TDABC model for each complication defined by the Postoperative Morbidity Survey( POMS) identified in the sample. Control group costs will be estimated in a retrospective analysis by the TDABC method guidelines considering the time that each professional spent in direct patient care and to record this time, interviews with health professionals will be conducted. By associating the resources and their respective times, costs-per-activity and total costs will be calculated. With cost-per-patient information, an assessment of the economic impact resulting from the implementation of standards of care by risk class will be conducted. The costs of drugs and materials will be collected through Querys requested to the HCPA information technology department. Variables of comparative cost between classes are: cost with drugs and both medical and nursing professionals in PACU, intensive care and wards. The project costs analysis will be used as the basis for the development of a platform for the measurement of health technology costs using TDABC.  eP2493 Anestesia e crack: um relato de caso Valentina Morel Côrrea Rodríguez; Cibelle de Abreu Evaldt; Nathália Paludo; Vicente Vieira Damiani UFRGS - Universidade Federal do Rio Grande do Sul  INTRODUÇÃO: A interação entre crack e anestesia apresenta escassas referências bibliográficas. Haja vista a elevada prevalência do uso dessa substância, faz-se necessário documentar o desfecho de uma indução anestésica em paciente usuário de crack para solidificar o conhecimento já existente sobre esta combinação. DESCRIÇÃO DO CASO: Paciente 59 anos é internada para realizar histerectomia abdominal eletiva por apresentar lesão expansiva pélvica sugestiva de neoplasia de ovário. Foi internada na semana anterior à cirurgia para certificar o não uso de drogas ilícitas que pudessem interferir com a técnica anestésica. Optou-se pela realização de técnica anestésica combinada com anestesia geral e loco-regional. A raquianestesia foi realizada com agulha Quincke 27G, nível L3-L4, punção mediana, utilizando bupivacaína pesada 0.5% 15 mg e morfina 0,1 mg. Na indução anestésica, foi usado propofol 2mg/kg, fentanil 3 mcg/kg e succinilcolina 1mg/kg. A manutenção se deu através de sevoflurano, com fração expiratória média do agente de 1.7, e 10mg de atracúrio. A monitorização foi realizada com oximetria de pulso, cardioscopia, pressão arterial invasiva, temperatura corporal central e monitor de profundidade anestésica (BIS). O procedimento foi bem-sucedido, tendo como única intercorrência picos de hipotensão que foram corrigidos com fármacos vasopressores. Uma vez que o crack é a forma alcalinizada da cocaína, os efeitos já descritos na literatura em relação à esta podem ajudar no entendimento da interação do crack com a anestesia. A cocaína inibe a recaptação pré-sináptica de monoaminas neurotransmissoras, causando vasoconstricção intensa, vasoespasmo e picos hipertensivos. Na anestesia geral, são descritas complicações como infarto agudo do miocárdio, arritmias, dissecção aguda de aorta, acidente vascular cerebral, insuficiência renal e complicações pulmonares. Indução com propofol e tiopental parece ser mais seguro, e o bloqueio neuromuscular com succinilcolina parece ser maior. Na anestesia regional, ocorre hipertensão, hipotensão desencadeando arritmias e resistente à efedrina, comportamento combativo e percepção alterada da dor. Dessa forma, é recomendado, no mínimo, intervalo de uma semana sem o uso da droga antes da realização de procedimentos eletivos.
