Post-transplant, a bile leak occurred and demanded reoperation to convert biliary anastomosis to a Roux-en y hepaticojejunostomy. There were no vascular complications, and the patient was discharged home in good health. Conclusion: Spontaneous portosystemic shunts are common findings in patients with PV thrombosis and/or PV cavernomatous transformation. Whenever there is PV thrombosis, an attempt to remove the entire thrombus is mandatory. If removal of the thrombus is not feasible, the shunt itself may be used for an anastomosis with donor PV. An alternative option is the creation of a renoportal bypass. After thrombectomy or whenever there is not PV thrombosis, all spontaneous large shunts should be ligated during LT. In the absence of PV thrombosis, ligation of the shunt should enable an increase in the PV flow and pressure, as reported herein. Alternatively, PV may be atrophic and thus portal flow may not react adequately to shunt ligation. In such instances, the creation of a jump graft between SMV and PV may be necessary. Thus, preoperative study of PV and the shunts is essential to identify spontaneous PV shunts and enable successful LT.  eP2140 O impacto dos inibidores da 5-Alfa-Redutase sobre mortalidade em um cenário de quimioprevenção primária para câncer de próstata Pedro Glusman Knijnik; Pietro Waltrick Brum; Eduardo Tosetto Cachoeira; Artur de Oliveira Paludo; Antonio Rebello Horta Gorgen; Lucas Medeiros Burttet; Jeruza Lavanholi Neyeloff; Brasil Silva Neto UFRGS - Universidade Federal do Rio Grande do Sul  Introdução: Um método seguro e efetivo para a  quimioprevenção do câncer de próstata (CaP) é desejável devido a sua alta incidência, impacto na morbimortalidade e custos para sociedade. Nesse contexto, há evidências na literatura de que os inibidores da 5-alfa-redutase (5ARI) podem potencialmente reduzir o risco de CaP. Uma metanálise de 2010 demonstrou que o uso de 5ARI reduziu a incidência de diagnóstico de CaP, mas não conseguiu avaliar o efeito na sobrevida específica ou global. Nosso objet ivo foi atualizar essa meta-análise e, o mais importante, incluir, em nossa análise, dados de estudos observacionais. Objetivos: nosso objetivo primário residiu em avaliar o impacto dos 5ARI na mortalidade geral e câncer-específica em um cenário de quimioprevenção primária de CaP. Como objetivos secundários, foram analisadas a incidência geral de CaP, assim como a incidência das neoplasia de baixo, moderado e alto grau. Métodos: realizamos uma busca sistemática nas bases de dados MEDLINE, EMBASE, Cochrane Library, ClinicalTrials.gov até abril de 2018 a fim de indentificar ensaios clínicos randomizados (ECR) e coortes. Nós incluimos artigos que apresentavam dados de mortalidade ou incidência de CaP em homens expostos aos 5ARI previamente ao diagnóstico de CaP. Cada artigo foi avaliado de maneira independente por dois revisores tanto para a seleção dos estudos e extração de dados, como para a avaliação da qualidade dos estudos e do risco de viéses. Utilizamos o modelo de efeitos randômicos associado ao método de Mantel-Haenszel para calcular o sumário dos efeitos observados. A heterogeneidade foi avaliada pelo teste de Higgins. Resultados: 3432 artigos foram incluidos das bases de dados supracitadas (após exclusão das duplicatas). 179 artigos foram selecionados para revisão na íntegra. Foram incluidos 18 artigos nessa revisão sistemática. A exposição aos 5ARI previamente ao diagnóstico de CaP não aumentou a mortalidade geral ou câncer-específica, nem a incidência dos cânceres de alto grau. Nós identificamos uma redução do risco relativo de 24% em CaP de moderado e baixo grau, assim como uma redução de 26% no diagnóstico geral de CaP no subgrupo dos ECR. Conclusão: nosso estudo não identificou efeito da exposição aos 5ARI na mortalidade ou na incidência dos cânceres de alto grau. This study proposes the creation of a post-surgery care pathway called Extended Care in High-Risk Surgical Patients (EXCARE) in the form of coordinated multiprofessional actions dedicated to high-risk non-cardiac surgical patients with the aim of improving the postoperative outcomes. The pathway comprises actions that include individual patient-centered risk assessment (assessed by the SAMPE Risk Model, a 30-day probability of death institutional model), specialized care in the post-anesthetic care unit (PACU) and in the surgical wards performed by the nursing, anesthesia, internal medicine and surgery teams. Methods: Quasi-experiment including 1720 patients, 430 consecutive non-cardiac patients >16 years who underwent elective or non-elective procedures in Hospital de Clínicas de Porto Alegre, referred to post-anesthesia care unit after surgery and classified as high-risk (probability of death 5% in 30 days by the SAMPE Risk Model) in the intervention group and 1290 controls (matched by propensity score in a 1:3 ratio) in which the clinical effectiveness of EXCARE is being analyzed using a before-and-after comparison.  Primary outcome is a compound of in-hospital 30 day-mortality and complications by Postoperative Morbidity Survey (POMS). Secondary outcomes include number of Rapid Response Team (RRT) calls, length of stay, reoperation, readmission. Procedures performed outside the main surgical unit (outpatients, diagnoses, performed under local anesthesia); patients submitted to cardiac surgery with extracorporeal circulation and / or referred to the cardiac ICU postoperatively; organ transplantation; low risk patients classified as <5% probability of death within 30 days by SAMPE Risk Model were excluded. Results: The preliminary analysis included 402 patients (151 intervention and 251 controls) from January to June 2019, with overall high adherence to protocol by health professionals. EXCARE group presented higher number of calls to RRT within 7 days, more reoperations and lower mortality within 30 days (p<0,05). It has proved itself to be feasible as an institutional project, supported by Medical Vice-Presidency. This is the first study to evaluate the implementation of an institutional postoperative care bundle involving nursing and medical teams, and can be consolidated as a future standard of care.
