In der Therapie von malignen B-Zell Lymphomen ist die humorale Immunochemotherapie mit Anthrazyklin-basierter Chemotherapie und mit monoklonalen Antikörpern gegen das Differenzierungsantigen CD20 die Basis der Primärtherapie. Die hier vorgelegte Doktorarbeit fasst Ergebnisse zusammen, die zeigen, dass aus Lymphomzellen gebildete Exosomen Anti-CD20-Antikörper binden, Komplementfaktoren aufbrauchen und somit die malignen Tumorzellen vor Komplementattacken schützen können. Weitere Versuche unter Anwendung des CAM-in-vivo-Modells zeigten zudem, dass die zytostatische Aktivität von Chemotherapeutika vom Typ der Anthrazykline mit Modulatoren der zytoplasmatischen Sequestration gesteigert wurde. 

Durch die Verwendung der Substanzen Rapamycin, Indometacin und U18666A, die mit der Sekretion von Exosomen interagieren, wurde in Zelllinienmodellen eine Inhibition der exosomenbasierten Resistenzmechanismen gegen humorale Immunochemotherapie bei aggressiven Lymphomen gezeigt. 
Mittels differentieller Ultrazentrifugation wurden eine Methode optimiert, mit der Exosomen aus dem Überstand von Lymphomzellenkulturen angereichert wurden. Die Exosomen der Lymphomzelllinien waren durch die Expression der Exosomenmarker CD63, Flotillin-2 und Alix gekennzeichnet. Weiterhin wurde auf den Exosomen das Antigen CD20 nachgewiesen, sowie die Komplement-regulierenden Proteine CD46, CD55 und CD59. 
Es wurde funktionell gezeigt, dass durch die Komplement-regulierenden Proteine die Komplementattacke begrenzt, Komplementfaktoren verbraucht und damit die zytolytische Effektivität des Anti-CD20-Antikörpers Rituximab reduziert wurde. Basierend auf der Bedeutung des ABC-Transporters A3 für die Exosomensekretion wurde die Bedeutung von ABCA3 Inhibitoren wie Rapamycin, Indometacin und U18666A auf die Exosomensekretion überprüft. Inhibitoren von ABCA3 reduzierten die Exososomensekretion und verstärkten den zytolytischen Effekt des Anti-CD20 Antikörpers Rituximab. Weiterhin verminderte eine genetische Suppression der ABCA3-Expression mittels spezifischer shRNA-Konstrukte die Exosomenbiogenese und erhöhte die CDC-Aktivität des Antikörpers Rituximab. Umgekehrt verminderte eine verstärkte Exosomenbiogenese die spezifische CDC-Aktivität des Antikörpers. Weiterhin wurde gezeigt, dass durch die Hemmung der Exosomensekretion mittels Indometacin auch die Sekretion von Anthrazyklinen aus der Zelle vermindert, und damit die Retention der Zytostatika im Kern erhöht wurde.During the therapy of malignant B-cell lymphomas the humoral immunochemotherapy with anthracycline-based chemotherapy and with monoclonal antibodies against the differencing antigen CD20 is the basis of the primary therapy. This medical thesis summarizes results that show that exosomes from lymphoma cells can bind anti-CD20 antibodies, consume complement factors and hence can protect malignant tumor cells against complement attacks. Additional experiments by using the CAM-in-vivo-model showed that the cytostatic activity of chemotherapeutic agents (type anthracyclines) can be increased by modulators of the cytoplasmatic sequestration. By using the substances rapamycin, indomethacin and U18666A that interact with the secretion of exosomes, an inhibition of the exosome-based resistance mechanisms against humoral immunochemotherapy in aggressive lymphomas could be demonstrated in cell line models. Using differential ultracentrifugation, a method was optimized to accumulate exosomes from the supernatant of lymphoma cell cultures. The exosomes of the lymphoma cell lines were characterized by the expression of the exosome marker CD63, flotillin-2 and alix. Furthermore, the antigen CD20 and the complement-regulatory proteins CD46, CD55 and CD59 were detected on the exosomes. It was functionally shown that the complement-regulatory proteins limit the complement attack, consume complement factors and hence, the cytolytic effectivity of the anti-CD20 antibody rituximab was reduced. Based on the importance of the ABC-transporter A3 for the exosome secretion, the role of ABCA3 inhibitors like rapamycin, indomethacin and U18666A were checked on exosome secretion. Inhibitors of ABCA3 reduced the exosomes secretion and enhanced the cytolytic effect of the anti-CD20 antibody rituximab. Moreover, a genetic suppression of the ABCA3 expression using specific shRNA-constructs reduced the biogenesis of exosomes and increased the CDC activity of the antibody rituximab. In addition, it was demonstrated that the inhibition of the exosome secretion using indomethacin also resulted in a reduced secretion of anthracyclines from the cell and thus, the retention of cytostatic drugs in the nucleus was increased.2020-12-3
