The first cycle of the Canadian Incidence Study of Reported Child Abuse and Neglect (CIS-1998) was the first Canada-wide study to examine the incidence of reported child maltreatment and the characteristics of children and families investigated by Canadian child welfare services. Comparisons between the CIS-1998 and CIS-2003 will provide the opportunity for the first time in Canada to examine trends in child maltreatment such as the rates of child welfare services, maltreatment types, substantiation outcomes, and severity of maltreatment.
Canadian Incidence Study of Reported Child Abuse and Neglect (CIS-2003) is designed to:
• Produce national estimates of the incidence of investigated maltreatment in Canada in 2003;
• Document changes from 1998 to 2003 and explore the reasons for change;
• Examine variations in types and severity of maltreatment; and,
• Explore the role of selected child and family characteristics on the incidence of maltreatment and on key service outcomes.

TheCIS-2003 team at the University ofToronto and the University of Calgary iscurrently in the stage of data analysis andreport writing. The first cycle of theCanadian Incidence Study of ReportedChild Abuse and Neglect (CIS-1998)was the first Canada-wide study toexamine the incidence of reported childmaltreatment and the characteristics ofchildren and families investigated byCanadian child welfare services.Comparisons between the CIS-1998and CIS-2003 will provide theopportunity for the first time in Canadato examine trends in child maltreatmentsuch as the rates of child welfare services,maltreatment types, substantiationoutcomes, and severity of maltreatment.All provinces and territories participatedin both the CIS-1998 and CIS-2003.The CIS-2003 is funded by HealthCanada, provincial and territorialgovernments of Canada, Bell Canada,the Centre of Excellence for ChildWelfare, and the First Nations Childand Family Caring Society. Theprimary objective of the CIS-2003 is toprovide reliable estimates of the scopeand characteristics of reported childabuse and neglect across Canada.Specifically, the study is designed to:• Produce national estimates of theincidence of investigatedmaltreatment in Canada in 2003;• Document changes from 1998 to 2003and explore the reasons for change;• Examine variations in types andseverity of maltreatment; and,• Explore the role of selected child andfamily characteristics on theincidence of maltreatment and onkey service outcomes.MethodologyData Collection InstrumentInformation was collected using a three-page data collection instrument(Maltreatment Assessment Form)completed by child welfare workers. TheMaltreatment Assessment Form gathersinformation about the investigated childmaltreatment, severity of maltreatment,child and caregiver functioning,substantiation, and other relatedoutcomes. The form was kept as shortand simple as possible to minimize theburden of filling it out and to ensure ahigh completion rate. Completion of theMaltreatment Assessment Form waslinked to the written assessments thatworkers recorded upon the completionof intake investigations. Given that someinvestigations can take many months tobe completed, workers were asked tocomplete the Maltreatment AssessmentForm at the same time as their firstassessment report, regardless of whetherthe entire investigation was complete.As a result of the feedback received fromworkers on the CIS-1998, a few keychanges were made to the MaltreatmentAssessment Form for the 2003 cycle ofdata collection. For example, the CIS-1998 definition of maltreatment included22 types of maltreatment subsumedunder four categories: physical abuse,sexual abuse, neglect, and emotionalmaltreatment. After the pilot testing, the22 kinds of maltreatment were increasedto 25. Specifically, the definitions ofphysical abuse were changed in order tobetter understand the inappropriatepunishment category, which was the mostinvestigated and most substantiatedtypeof maltreatment in the CIS-1998.The physical abuse categories nowinclude: 1) Shake, push, grab or throw; 2)Hit with hand; 3) Punch, kick or bite; 4)Hit with object; and, 5) Other physicalabuse. The sexual abuse definitions werealso changed to clarify some forms (e.g.,“sexual activity completed” was changedto “penetration”).SampleThe CIS-2003 team randomly sampled62 agencies across all provinces andterritories including eight First Nationsagencies. Three provinces (Alberta,Ontario, and Prince Edward Island) andone territory (the Northwest Territories)elected to oversample to provide enoughcases to produce a provincial/territorialreport and provincial/territorial estimatesof child maltreatment. At most sites, allcases opened for service between October1, 2003 and December 31, 2003 wereselected for inclusion in the study. As theCIS collects information about maltreat-ment reported to child welfare authori-ties, it does not include undetected casesthat are not reported to child welfareauthorities, investigations conductedonly by police, cases screened out prior toan investigation being carried out orinvestigations on already opened cases.Data CollectionThe CIS-2003 collected approximately12,000 instruments (data collectionforms) that provided information aboutthe child’s family, child functioning, typeof maltreatments and levels ofsubstantiation, harm to the child, andoutcomes of investigation. Theinformation was collected directly froman estimated 1,000 child welfare workersin 62 agencies across Canada (excludingQuebec). Twenty-five site researcherswere assigned to coordinate 120 half-daysite training sessions. The trainingincluded a standard PowerPointpresentation through which participatingchild welfare workers were introduced tothe CIS, key findings from the CIS-1998, the CIS-2003 sampling and caseselection, data collection, and theMaltreatment Assessment Forms. Siteresearchers visited the CIS-2003 sites ona regular basis to collect forms, respondto questions, and monitor studyprogress. The forms were verified bothon site and by either the University ofToronto or University of Calgary officeand scanned into an electronic file.Currently, the database for the CIS-2003is being developed by the University ofToronto and University of Calgaryresearch teams. This database willcontain close to 400 variables.ConclusionFor the first time in Canadian history, thenational incidence of reported child abuseand neglect will be compared across twopoints in time (CIS-1998 and CIS-2003)which will reveal important trends inchild welfare services across Canada. InOntario, three points in time will beavailable to compare (i.e., OIS-1993,OIS-1998, and OIS-2003). Similar tothe CIS-1998 data, which Alberta used toinform and implement a child welfareAlternative Response Model, the CIS-2003 and subsequent comparison reportswill allow for evidence-based practice thatwill influence and inform child welfarepolicy and research. With this knowledge,we hope to facilitate prevention andintervention efforts and help to end thecycle of violence.The CIS-2003 team will present keyfindings to participating agencies uponrelease of the CIS-2003 Final Report. Acomparison report, fact sheets, andjournal articles will be published. Thesewill be available at www.cecw-cepb.ca. Inaddition, Health Canada has agreed tofund some secondary analysis of this richdatabase, which will allow for furtherexamination of key findings. Le Formulaired’évaluation des mauvais traitementsrecueille de l’information sur les mauvaistraitements enquêtés, particulièrement, letype de mauvais traitements et leur gravité,le niveau de fonctionnement de l’enfant etde l’aidant, la corroboration, ainsi qued’autres renseignements connexes. Nousavons cherché à faire en sorte que leformulaire soit aussi concis et simple quepossible afin de minimiser le travail requispar les intervenants pour le remplir et pourassurer un taux d’achèvement élevé. Lestravailleurs devaient remplir le Formulaired’évaluation des mauvais traitements unefois terminée la rédaction du rapport del’enquête préliminaire. Étant donné quecertaines enquêtes peuvent s’échelonnersur des mois, nous avons demandé auxtravailleurs de remplir le formulaire enmême temps que le rapport de l’évaluationpréliminaire, même lorsque l’enquêten’était pas encore terminée.En réaction aux commentaires destravailleurs au sujet de l’ÉCI – 1998,quelques changements importants ont étéapportés au Formulaire d’évaluation desmauvais traitements pour le volet 2003 de la collecte de données. Par exemple,lors de l’ÉCI – 1998, la définition de« mauvais traitements » était composée de22 différentes formes divisées en quatrecatégories : violence physique, violencesexuelle, violence psychologique et négli-gence. Après la période d’essai, nousavons rajouté à la définition de mauvaistraitements trois autres formes. Plusparticulièrement, nous avons modifié ladéfinition de « violence physique » afin de mieux refléter la catégorie « punitionsinappropriées », qui était la forme demauvais traitements relevée le plusfréquemment à l’ÉCI – 1998. Les catégories de violence physique incluentmaintenant : 1) secouer, pousser, saisir oulancer; 2) frapper avec la main; 3) frapperd’un coup de poing ou de pied, mordre;4) frapper avec un objet; 5) toute autreforme de violence physique. À noter que l’ÉCI recueille des données à partir de cas de mauvaistraitements signalés aux services deprotection de l’enfance; font donc l’objetd’exclusion les cas non signalés aux servicesde protection de l’enfance, les enquêtesmenées exclusivement par la police, les caséliminés lors de la pré-enquête ou lesenquêtes réalisées sur des cas déjà actifs.Collecte de donnéesDans le cadre de l’ÉCI – 2003, près de12 000 formulaires de collecte de donnéesont été recueillis; ces formulaires ont fourni des renseignements sur la famille del’enfant, le niveau de fonctionnement del’enfant, les types de mauvais traitements,les niveaux de corroboration, les tortscausés à l’enfant ainsi que les résultats de l’enquête. Ces renseignements ont été obtenus directement par environ1000 travailleurs en protection del’enfance de 62 agences de partout auCanada, sauf au Québec. On a demandé à25 chercheurs de se rendre sur place afind’organiser 120 ateliers d’une demi-journée. L’atelier comportait une présen-tation uniformisée en format PowerPointvisant à familiariser les travailleursparticipants à l’ÉCI, aux constations clésde l’ÉCI – 1998, à l’échantillonnage et à lacollecte de données de l’ÉCI – 2003 ainsiqu’au Formulaire d’évaluation des mauvaistraitements. Les chercheurs ont visitérégulièrement les sites de l’ÉCI – 2003 afin de recueillir les formulaires, derépondre aux questions des participants et de surveiller l’évolution de la recherche.Les formulaires ont été examinés sur place,ainsi qu’à l’Université de Toronto ou àl’Université de Calgary et ensuite numé-risés en format électronique. À l’heureactuelle, la banque de données de l’ÉCI –2003 est alimentée par les équipes dechercheurs de l’Université de Toronto et del’Université de Calgary; à la fin du projet,elle contiendra près de 400 variables.ConclusionPour la première fois dans l’histoire duCanada, cette étude nationale permettrade comparer à deux moments précis, soit1998 et 2003, les signalements de mauvaistraitements et de négligence afin de révélerles tendances importantes dans les servicesde protection de l’enfance au Canada.Toutefois, il sera possible en Ontario decomparer trois périodes : 1993, 1998 et2003. L’ÉCI – 1998 avait permis àl’Alberta de concevoir l’Alberta ResponseModel (ARM) pour les services deprotection de l’enfance; nous prévoyonsque l’ÉCI – 2003 et les rapports compara-tifs qui en découleront serviront égale-ment à alimenter les pratiques fondées surles preuves et influenceront les politiqueset la recherche en protection de l’enfance.À l’aide de ces connaissances, nous espé-rons appuyer les efforts de prévention etd’intervention et de contribuer à freiner lecycle de la violence.L’équipe ÉCI – 2003 présentera les prin-cipales constatations aux agences ayantparticipé à la recherche après le dépôt durapport final. Des documents comparatifs,des feuillets d’information et des articles derevue seront également publiés. On pourrales consulter au site www.cecw-cepb.ca.
