Nursing narratives and reflective practice: a theoretical review

Three themes emerged that related to the simulation ses-sions: briefing, simulation exercise, and debriefing. The nur-sing students experienced “briefing: preparation is important,”“simulation exercise: doing is better than listening,” and“debriefing: methods to communicate.”The findings enrich our understanding of how nursing stu-dents develop communication skills in a simulation setting.This study provides valuable information on designing inter-professional simulations to enhance team communication.Nurses’ Perceptions of Difficult Patients andCoping Strategies Used During theManagement of Difficult Patients in Medical–Surgical WardsHui-Chen Chen, Alice Lee Centre for Nursing Studies,National University of SingaporeChoo Jie Ling, Raffles HospitalA “difficult patient” is one who engages in undesirable actionswhich include (i) aggressive, verbal and nonverbal; (ii) unco-operative; and (iii) demanding behaviours. Encounters withdifficult patients can negatively affect nurses’ emotionalhealth, affecting their work performance, leading to impairednurse–patient relationships and patient care. While there havebeen few programs to equip hospital staff for such situations, asencounters with difficult patients, especially aggressive ones,increase over the years, it is imperative to explore and betterunderstand the experience of nurses with difficult patients andcoping strategies that have been utilized. Such informationwould be valuable in improving existing coping strategies,which could be shared amongst other nurses and facilitate theirmanagement with difficult patients. This study aims to explorenurses’ perceptions of difficult patients and coping strategiesused during the management of difficult patients in medical–surgical wards in Singapore. A face-to-face semi-structuredindividual interview was conducted. Ethical approval wasobtained. Purposive sampling technique was adopted todevelop the sample by selecting 16 registered nurses. Thisstudy has revealed the perceptions of nurses on difficultpatients, difficult patients’ impact on nurses, nurses self-copingstrategies, their perceived support systems available, and chal-lenges faced and suggestions for improvement. There wereseveral aspects which were lacking in terms of organizationalsupport for nurses; hence, improvements could be made tofacilitate nurses’ management of difficult patients in the future.Nursing Narratives and Reflective Practice:A Theoretical ReviewAna Choperena, University of NavarraBegoña Errasti-Ibarrondo, University of NavarraIn the context of a theoretical review, the aim of this study wasto explore the literature regarding how nursing narratives hadbeen used to enhance reflective practice. A literature searchfrom 1990 to 2017 was conducted in PubMed, CINAHL, andPsycINFO databases. After applying the selection criteria, 13studies were identified. The quality of articles was evaluated.Three themes were identified as the main components of anongoing narrative process based on looking back to past clin-ical experiences, creating spaces for dialogue and bringing theworlds of theory and practice closer together. In conclusion,this review provides a forum for exploring the use of narrativesto enhance reflective practice, which may lead to the acquisi-tion of professional competences.Colorectal Cancer Patients’ and Caregivers’Views on Self-Management in the Post-Treatment Period: A Qualitative StudyColleen Cuthbert, University of CalgaryBrenda Hemmelgarn, University of CalgaryWinson Cheung, University of CalgaryResearch has highlighted that self-management (SM) has thepotential to help with the complex challenges of survivorship.Little is known about colorectal cancer (CRC) patients’ views orpreferences for SM in the post–cancer treatment period. Toaddress this, we conducted a qualitative study with CRC patientsand their caregivers who had completed treatment within the last5 years. Data were gathered using focus groups and individualinterviews which were recorded and transcribed. Interpretivedescription was used to guide data analysis and interpretationof results. We included n ¼ 19 patients and n ¼ 3 caregivers.The mean time since diagnosis was 3.2 years. Results of the studyrevealed that participants were keen to engage in SM behaviorsas they saw it as being proactive for their health, yet they neededmore support as evidenced in the following themes: (1) Survivorsneed more structured ways to engage in SM, (2) finding infor-mation and support was often haphazard, (3) information andsupport to manage the long-term physical and psychologicalconsequences of treatment are needed, (4) peer support wouldhelp normalize survivorship, (5) sexual health and bowelchanges are particularly challenging, (6) health care providersshould encourage healthy behaviors, and (7) information andaccess to programs for healthy behaviors are needed.
