Traditional Iranian medicine, based on thousands of years of written experience, has provided treasures of education to researchers in the field of medical sciences. Lime salve is a traditional medicine preparation that has been used by therapists as a topical and effective medication for wound healing from the 9th to 19th centuries. The aim of this study was to evaluate the healing effect of this topical preparation and its underlying mechanisms in a model of deep-seated deep-burn burn-in Wistar rats. Methods: Lime salve was prepared from a combination of refined calcium hydroxide powder, beeswax, and sesame oil and then quality control assessments were performed. Deep second-degree burns were caused by a heated metal plate in 48 male Wistar rats. Then, the rats were randomly assigned into four groups of lime salve (consisting of calcium hydroxide powder, sesame oil, and bees wax), the base of salve (sesame oil and bees wax), silver sulfadiazine (positive control), and saline (negative control). After sacrificing on days 5, 10, 17, and 24, wounds were digitally photographed and skin samples were obtained for qRT-PCR, immunohistochemical staining, and histological examination. Results: Lime salve significantly increased wound healing rate, increased angiogenesis on day 10, and increased collagen formation on days 17 and 24 compared to the control group. In addition, the samples treated with lime salve showed more epithelialization than the other groups. The qRT-PCR showed that on day 10, the VEGF and TGF-β1 genes were significantly higher in the lime-treated group than in the none treated group. MMP-9 and MMP-2 genes were also significantly different on day 17 compared to the control group; a statistically significant decrease occurred in the expression of these two genes on day 24. The expression level of IL-6 and TNF-α genes peaked on day 10 in the lime-treated group and declined in the following days. Conclusion: Lime salve with the properties mentioned in traditional Iranian medicine can accelerate the healing process in deep second-degree burn wounds in rats. Therefore, it is recommended to be considered as a complementary choice for the treatment of burn wounds in future clinical studies.
