Journalof the College of Physicians and Surgeons Pakistan, 27(10), 669.Available at: https://ecommons.aku.edu/pakistan_fhs_mc_women_childhealth_paediatr/622Pediatric Critical Care Medicine is a well-establisheddiscipline in developed and many developing countries,but in very early stages in Pakistan.1 The provision ofpediatric intensive care medicine is associated with highsurvival rates.2 Many acutely ill conditions are potentiallyreversible, if recognized in the early stages and treatedpromptly. There is no need of high-cost intervention ortherapies in all cases. The provision of basic intensivecare services is, therefore, essential for life-saving.3 Inlieu of severe shortage of trained pediatric intensive carephysicians and a relatively high mortality rate of childrenunder five (87/1000 live births in 2015) in Pakistan, wedesigned a "Basic Pediatric Intensive Care Course"(BPICC) for pediatricians and pediatric residents (front-line physicians) who are actively involved in the care ofacutely ill children. The goal of this course was tostandardize the care of acutely ill children and bringintensive care reflexes outside the pediatric intensivecare unit (PICU). BPICC is a two-full-day course withpre-test, post-test, and feedback form. The programconsisted of didactic lectures [How to diagnose and howto treat] on common acute critical illnesses of children inthe morning session along with a video on intensive careunit (ICU) procedures like central venous cannulation orendotracheal intubation each day followed by casesimulation in small groups (3-5 participants/group) in theafternoon after lunch. We conducted 13 BPICC in fourpediatric residency training hospitals in collaborationwith Department of Continuing Medical Education of TheAga Khan University Hospital from July 2014 toDecember 2016. The aim of this course was to preparethe frontline pediatricians in the early recognition andprompt appropriate initial management of acutely ill orinjured children to meet immediate short-term need.Of total 456 participants, the median participants percourse were 40 (20-56). Ninety-one percent wereresidents and the rest of them were consultants ofpediatrics in these teaching hospitals. More than 30%gain was seen in post-test score. We preparedfacilitators from these courses for future courses,increased the capacity of team from 3 to 25 over fewyears and few of them have chosen pediatric intensivistas a future career. The perceptions of most of theparticipants were knowledge enhancement, and gainingself-confidence on approaching a sick child. Theavailability of pediatric intensive care services isregarded as a reflection of quality of a country's pediatricmedical care.4 Focused educational intervention, likeBPICC, may improve our basic pediatric intensive careservices and ultimately improve survival of our childrenfrom acute, potentially-reversible illnesses.REFERENCES1.
