60  MONITORIZAREA AMBULATORIE HOLTER ECG 24 ORE LA COPII CU ARTRITĂ JUVENILĂ IDIOPATICĂ Bogonovschi Livia (Conducător ştiinţific: Revenco Ninel, dr. hab. şt. med., prof. univ., Departamentul de pediatrie) Introducere. Implicarea ţesutului cardiac în artrita juvenilă idiopatică (AJI) cuprinde şi componente ale reglării vegetative, declanşând un dezechilibru simpatovagal. Echilibrul dintre sistemul nervos simpatic şi cel parasimpatic reflectă variaţia frecvenţei cardiace normale. Holter ECG permite determinarea activităţii funcţiei nervoase simpatice şi parasimpatice asupra cordului. Scopul lucrării. Aprecierea unor parametri ai funcţiei vegetative asupra ritmului cardiac la copiii cu artrită juvenilă idiopatică. Material şi metode. În studiul clinic prospectiv au fost incluşi 55 de copii: 25 de copii cu AJI şi 30 de copii sănătoşi. Repartiţia după gen a fost aceiaşi: fetiţe – 60%, băieţei – 40%.Vârsta copiilor incluşi în studiu a 6-18 ani, media vârstei a constituit 13 ± 0,5 ani. Holter ECG 24 de ore a fost efectuat la toţi copiii, cu aprecierea indicilor variabilităţii ritmului cardiac (VRC): SDNN, SDANN, rMSSD, pNN50. Rezultate. Analiza traseelor ECG a evidenţiat la copiii cu AJI tendința crescută a tahicardiei sinusale: 83,2% vs 62%  și a extrasistolelor supraventriculare în 47% vs. 26%. În urma analizei parametrilor de timp ai VRC, s-a determinat o scădere semnificativă a acestor indici în AJI. Astfel, valori scăzute ale SDNN s-au înregistrat la 81,5 % în AJI, comparativ cu copiii sănătoşi (68 %), SDANN – 46,5% comparativ cu 27 % la copiii sănătoși. Aceiaşi tendinţă s-a notat şi pentru rMSSD: 56% vs. 32 % şi pNN50 94% vs. 74% la copiii sănătoși. Concluzie. Pacienţii cu AJI dezvoltă modificări semnificative ale parametrilor de timp ale VRC, ceea ce denotă o reducere a funcţiei parasimpatice şi o creştere a acţiunii simpatice asupra ritmului cardiac, contribuind la apariţia dereglărilor de ritm cardiac. Involvement of cardiac tissue in juvenile idiopathic arthritis (JIA) encompasses both autonomic components and results in sympathovagal imbalance. Balance between sympathetic and parasympathetic nervous system reflects the variation of normal heart rate. Holter ECG determines the sympathetic and parasympathetic nerve function activity on the heart. Objective of the study. To assess autonomic function parameters of heart rate in children with juvenile idiopathic arthritis  Material and methods. The prospective clinical study included 55 children of which 25 children with JIA and 30 healthy children. The gene distribution was the same: 60% girls, 40% boys. The age of children included in the study was 6-18 years, the average age was 13 ± 0.5 years. Holter ECG 24 hours was performed in all children, appreciating the indices of heart rate variability (HRV): SDNN, SDANN, RMSSD, pNN50. Results. The analysis of ECG in children with JIA emphasized the increasing tendency of sinus tachycardia: 83.2% vs 62% and premature supraventricular beats 47% vs. 26%. Following the analysis, time parameters of HRV determined a significant decrease in these indices in children with JIA. Thus, low values of SDNN were recorded in 81.5% in AJI compared to healthy children (68%), SDANN – 46.5% compared to healthy children in 27%. The same tendency was noted in rMSSD values: 56% vs. 32% and pNN50 94% vs. 74% to healthy children. Conclusions. JIA patients develop significant changes in HRV parameters, which shows a reduction of parasympathetic function and an increase in sympathetic action on heart rate, leading to the development of disorders of cardiac rhythm.
