123  PERFUZIA MIOCARDICĂ ŞI HEMODINAMICA INTRACARDIACĂ LA BOLNAVII CU CARDIOMIOPATIE DILATATIVĂ Ana Moroşan (Conducător ştiinţific: Tudor Branişte, dr. hab. şt. med. prof. univ., Departamentul Medicină Internă: Disciplina de medicina internă-semiologie) Introducere. Cardiomiopatia dilatativă rămâne neelucidată, în special, în ce priveşte patogenia. Drept cauză studierea vascularizaţiei miocardului apare ca un deziderat, ce se impune cu necesitate în patologia enunţată. Scopul lucrării. Analiza hemodinamicii şi perfuziei miocardice cu definirea semnificaţiei şi a caracterului informaţional al metodelor exploratorii în diagnosticul afecţiunilor miocardice intrinseci, cardiomiopatii dilatative (CMD), inflamatorii (CMI) şi ischemice (CMPI). Material şi metode. S-a efectuat analiza descriptivă a perfuziei miocardului şi a procentului de includere a taliului, după numărul impulsurilor radioactivităţii, în miocardul VS. Acuitatea inflamatorie a CMI s-a studiat după criteriile Dalas. Rezultate. Tomografia computerizată cu emisie de pozitroni, cu Tl-201, a detectat defecte de perfuzie a VS în 81,1% în CMD, 82,5% în CMPI şi 100% în CMI (p<0,001). Concluzii. (1) Circulaţia sanguină coronariană şi intramiocardică, la bolnavii cu CMD, a evidenţiat tulburări semnificative la toate nivelurile vascularizaţiei miocardului şi indică prezenţa hipoxiei cronice a miocardului ca fiind una din cauzele ce produce leziuni majore în miocardul bolnavilor cu cardiomiopatie dilatativă. Dilated cariomiopathy remains unsolved, especially, regarding pathogenesis. As cause studying myocardial vascularization appears as a desideratum, which is required in this pathology. Objective of the study. Analysis of myocardial perfusion and hemodinamycs and defining importance and informational character of exploration methods in diagnostics of intrinsic myocardial diseases, dilated cardiomyopaties (CMD) inflammatory (CMI) ind ischemic (CMPI). Material and methods. It has been performed myocardial perfusion and analysis of thalium inclusion by the number of radioactive impulses, in the myocardium of LV. Inflammatory acuity of CMI was studied by the Dalas criteria. Results. Computer Tomography by positron emission, with Tl-201, detected perfusion defects of LV in 81.1% in CMD, 82.5% in CMPI and 100% in CMI (p<0.001). Conclusions. (1) Coronary and intramiocardial blood circulation, at patients with CMD, revealed semnificative disturbances at all levels of myocardial vascularization and indicates the presence of chronic hypoxy of myocardium as one of the causes which lead to major lessions of myocardium at patients with dilated cardiomiopathy.
