Preparation for airway managementbegins with assessment of the airway and with the identifi-cation of any potentially challenging anatomical variations.However, the currently available clinical tests have quite poorprognostic value in the prediction of difficult intubation [1]and use of a combination of tests is recommended in order togain better diagnostic value [2]. At present, airway evaluationand assessment fails to completely exclude the unexpecteddifficult airway in daily anesthetic practice. Recently, a strongfocus has been placed on imaging methods: magnetic reso-nance imaging (MRI), airway ultrasound, and nasal endo-scopy [3].Various new airway devices have been introduced inorder to improve perioperative airwaymanagement in the lastdecade.These devices vary from videolaryngoscopes to inno-vative supraglottic airway devices, fiber-optic scopes, opticalstylets, optical tracheal tubes, airway exchange catheters, andbougies. The use of most of these novel devices in difficultairway management scenarios is not supported by any robustevidence, and they remain only as devices used mostly inteaching andmannequin settings and, apart fromvideolaryn-goscopes, they are not included in any recent guidelines [4].Difficult or failed intubations inperioperative airwayman-agement are relatively rare and occur much more frequentlyin prehospital medicine, Emergency Departments, or Inten-sive Care Units. However, critical situations such as “cannotintubate, cannot oxygenate” are often poorly managed andmay be associated with significant morbidity and mortality[5].The invention of novel supraglottic airway devices hasbeen driven by efforts to supersede tracheal intubation, evenfor traditional indications such as in obese patients, advancedlaparoscopic procedures, positions other than supine, or con-ditions associated with an increased risk of aspiration of gas-tric contents [6]. These devices should exhibit high orophar-yngeal seal pressures and enable smooth insertion even inthe hands of novice users. Another area of development ofsupraglottic airway devices concentrates on their use in diffi-cult airway management.This special issue of the journal focuses on progress, inno-vations, and controversial issues in perioperative airwayman-agement.The original investigations and review articles coverdifferent angles of this extensive anesthetic subspecialty. Oneclinical study investigated the changes in the intracuff pres-sures in two different types of tracheal tubes depending on thehead and neck positioning. This paper entitled “Comparisonof Pressure Changes by Head and Neck Position betweenHigh-Volume Low-Pressure and Taper-Shaped Cuffs: A Ran-domized Controlled Trial” found that taper cuffs were asso-ciated with a lower rise in intracuff pressures than the otherHindawi Publishing CorporationBioMed Research InternationalVolume 2016, Article ID 1965623, 3 pageshttp://dx.doi.org/10.1155/2016/19656232 BioMed Research Internationaltype during neck extension and flexion. This finding maybe important for postoperative tracheal morbidity: increasedintracuff pressures together with prolonged mechanical ven-tilation have been associated with an elevated incidenceof tracheal ischemic lesions [7]. The clinical performanceof five different supraglottic airway devices with increasedaspiration protection (2nd generation)was investigated in themanuscript “Comparison of Five 2nd-Generation Supraglot-tic Airway Devices for Airway Management Performed byNovice Military Operators.” The authors concluded that theLaryngeal Mask Airway Supreme and i-gel performed betterfor most insertion parameters such as insertion time, ease ofinsertion, and success rate than the other three devices.Simulation studies are an important part of anestheticresearch; however extrapolating their findings into clinicalmedicine is problematic [8]. Mannequin airway studies havebeen repeatedly used as a first assessment tool for new devicesand airway management techniques but their oropharyngealanatomy may differ significantly from real patients [9]. Twostudies in this special issue focus on simulation in airwaymanagement. Gum-elastic bougie improved the success rateof tracheal intubation in an infant model of difficult laryn-goscopy when compared with a standard procedure per-formed with the Miller laryngoscope, in the article entitled“Utility of a Gum-Elastic Bougie for Difficult Airway Man-agement in Infants: A Simulation-Based Crossover Analysis.”Two different types of videolaryngoscopes, McGrath andPentax-AWS Airwayscope, and direct Macintosh laryngo-scope were compared in simulated vomitus and hematemesisscenarios. This article “Comparison of Direct and Indi-rect Laryngoscopes in Vomitus and Hematemesis Settings:A Randomized Simulation Trial” favored Macintosh andMcGrath laryngoscopes in terms of success rate and intuba-tion time.Bedside ultrasound has also been used within the lastdecade in perioperative airway management. In their reviewarticle, entitled “The Role of Airway and EndobronchialUltrasound in Perioperative Medicine,” the authors recom-mend the use of ultrasound for location of the cricothyroidmembrane, confirmation of tracheal tube placement, or loca-tion/biopsy of pulmonary nodules but do not suggest itsroutine use in preoperative prediction of the difficult airway.Some rare disorders may significantly affect airway manage-ment and perioperative ventilation. The paper entitled “Res-piratory Strategies and Airway Management in Patients withPulmonary Alveolar Proteinosis: A Review” summarizes theavailable data on lung isolation techniques for this procedureand recommends step-by-step ventilation strategies includ-ing extracorporeal oxygenation for these patients. Pediatricairway management may be associated with controversialissues such as prediction of difficult laryngoscopy, manage-ment of the difficult airway, selection of cuffed or uncuffedtracheal tubes, timing of extubation, and indications for theuse of supraglottic airway devices. The review article entitled“Controversies in Pediatric Perioperative Airways” concludesthat strong evidence regarding all these controversies ismissing, most data has been extrapolated from adult airwaymanagement, and more randomized controlled trials on thistopic are vitally needed. Supraglottic airway devices are alter-natives to tracheal intubation in elective surgical procedureswithout additional risk of aspiration and may be also used asan alternative airway device in difficult intubation scenarios.The paper entitled “Complications Associated with the Useof Supraglottic Airway Devices in Perioperative Medicine”highlights the incidence and significance of complicationsrelated to these devices: aspiration, nerve injuries, trauma,and postoperative sore throat.The field of airway management is still evolving. Theincidence of life-threatening events and complicated scenar-ios is very low and these are not completely predictable.This fact explains the lack of robust clinical trials in airwaymanagement and it is also a reason why most guidelinesand recommendations are based on expert opinions, con-sensus, and case series rather than on high-quality evidence-based medicine. International groups of experts such as the“Difficult Airway Research Collaboration” (http://www.darc-airway.com/site/) have been established in order to createand coordinate more robust pan-European and worldwideclinical trials in airway management. Future airway researchshould focus on setting up large multicentre studies whichare adequately powered to evaluate the incidence of even veryrare complications, such as aspiration of gastric contents orsignificant damage to anatomical structures caused by airwayinterventions.Pavel MichalekMagboul M.
