Aspects related to oral impact on the daily activities of adolescents, adults and elderly in the State of São Paulo

Orientador: Antonio Carlos PereiraDissertação (mestrado profissional) - Universidade Estadual de Campinas, Faculdade de Odontologia de PiracicabaResumo: O reconhecimento dos problemas bucais como causadores de impacto negativo na qualidade de vida é uma crescente, onde além de dor e sofrimento podem causar privações sociais e constragimentos psicológicos. O presente estudo teve por objetivo analisar o impacto bucal na qualidade de vida associado aos aspectos sociodemográficos, de acesso e clínicos em uma amostra representativa de base populacional. Trata-se de um estudo transversal com dados secundários relativos ao Estado de São Paulo obtidos da Pesquisa Estadual de Saúde Bucal - Projeto SB São Paulo 2015, do qual participaram 16.776 pessoas divididas em 3 grupos etários: adolescentes de 15-19 anos, adultos 35-44 anos e idosos com 65 anos ou mais. A variável desfecho estudada foi o impacto oral nas atividades diárias obtida através do instrumento OIDP - Oral Impacts on Daily Performances e as variáveis independentes analisadas foram divididas em três blocos adaptado ao modelo de Peres, et al 2013. Nos blocos 1 e 2 foram incluídos os aspectos sóciodemográficos - bloco 1: gênero, faixa etária e grupo étnico, bloco 2: renda familiar e no terceiro bloco os aspectos clínicos e de acesso aos serviços de saúde - bloco 3: dor de dente, visita ao dentista, tipo de serviço utilizado, motivo da consulta e índice CPOD. Para a análise dos dados avaliou-se a associação entre o OIDP e as variáveis independentes por meio de modelo de regressão logística múltipla hierarquizado.. Foram testadas no modelo de regressão logística múltipla as variáveis com p?0,20 de cada bloco, permanecendo no modelo aquelas que continuaram associadas ao OIDIP com p?0,05 após o ajuste para as variáveis do mesmo bloco e para as hierarquicamente superiores. Foi possível concluir que os aspectos sociodemográficos, juntamente aos aspectos clínicos e de acesso aos serviços odontológicos influenciaram no impacto bucal de forma negativa na qualidade de vida dos indivíduosAbstract: Recognition of oral problems as causing negative impact on quality of life is a growing problem, where pain and suffering can cause social deprivation and psychological constraints. The present study aimed to analyze the oral impact on the quality of life associated to sociodemographic, access and clinical aspects in a representative population - based sample. This is a cross-sectional study with secondary data related to the State of São Paulo obtained from the State Oral Health Survey - Projeto SB São Paulo 2015, in which 16,776 people were divided into 3 age groups: adolescents aged 15-19 years, adults 35 -44 years old and aged 65 and over. The variable endpoint studied was the oral impact on the daily activities obtained through the OIDP instrument - Oral Impacts on Daily Performances and the independent variables analyzed were divided into three blocks adapted to the model of Peres, et al 2013. In blocks 1 and 2 were included the Socio-demographic aspects - block 1: gender, age group and ethnic group, block 2: family income and in the third block clinical aspects and access to health services - block 3: toothache, visit to the dentist, Query reason and CPOD index. For the analysis of the data the association between OIDP and the independent variables was evaluated through a hierarchical multiple logistic regression model. The variables with p?0.20 of each block were tested in the multiple logistic regression model, remaining in the Model those that continued to be associated with OIDIP with p?0.05 after adjusting for the variables of the same block and for the hierarchically superior ones. The presence of oral impact was observed in 28% of the study participants, and a significant association was found with the female, adult and elderly individuals who belonged to the ethnic group of blacks / browns, those with a family income up to R 1.500.00, individuals who reported feeling toothache in the last six months, those who reported attending the public service and who said that the reason for seeking the service was to perform dental treatment had a significant association with a greater impact on OIDP.
