Prognostic value of tumoral and stromal expression of matrix metalloproteinases 2, 9, 14, tissue inhibitors of matrix metalloproteinase 1, 2 and vascular endothelial growth factor A for cervical cancer patients

As expressões das metaloproteases de matriz 2, 9 e 14 (MMP-2, MMP-9, MMP-14), dos inibidores teciduais de metaloproteases 1 e 2 (TIMP-1, TIMP-2) e do fator de crescimento endotelial vascular A (VEGF-A) no microambiente tumoral estão envolvidas na invasão tumoral e metástase pela degradação da matriz extracelular e angiogênese. Uma associação destas proteínas com o prognóstico poderia indicá-las como alvos terapêuticos para esta patologia. Objetivo: Verificar se a expressão das proteínas MMP-2, MMP-9, MMP-14, TIMP-1, TIMP-2 e VEGF-A em células tumorais e estromais do câncer do colo do útero têm valor prognóstico. Materiais e Métodos: É uma coorte retrospectiva cuja amostra provém das 76 pacientes com carcinoma invasivo do colo do útero do estudo: "Expressão das proteínas MMP-2, MMP-9, MMP-14, TIMP-1, TIMP-2 e VEGF-A na NIC 3 e no carcinoma invasor do colo do útero", conduzido no Hospital da Mulher Prof. Dr. José Aristodemo Pinotti da Universidade Estadual de Campinas. Das 76 mulheres, 64 foram incluídas. As expressões proteicas foram anteriormente obtidas por imunohistoquímica nas células tumorais e estromais de biópsias. A expressão foi definida pela porcentagem de células coradas e categorizada, para este estudo, entre alta e baixa usando pontos de corte definidos individualmente por curva ROC. Coletou-se dados clínicos e morfológicos para prosseguir com a análise de sobrevida doença-específica (DSS). Para tal, coletou-se as informações de tratamento e seguimento desde o diagnóstico até a última visita clínica. Os status clínicos foram categorizados em: viva livre de doença, viva com doença, óbito por causa desconhecida e óbito pela doença. Óbito pela doença foi associado com estádio do tumor, grau histopatológico, e com as expressões tumorais e estromais de MMP-2, MMP-9, MMP-14, TIMP-1, TIMP-2 e VEGF-A por modelos de regressão de Cox univariada e multivariada testadas por Hazard Ratio (HR) com intervalo de confiança 95% (IC95%). Para as associações significantes da univariada (p<0,05) foram construídas curvas de Kaplan-Meier e comparadas por Log-rank para análise de DSS. Resultados: Em 50% dos estádios avançados (II-IV) houve maiores expressões de MMP-9 no estroma, enquanto 89,5% em estádio I apresentou expressões altas de MMP-9 no estroma. Risco relativo de óbito pela doença foi associado à altas expressões de: MMP-2 estromal (HR; IC95%: 4.3; 1.3-14.2), MMP-14 estromal (HR; IC95%: 8.8; 1.1-69.6), TIMP-2 estromal (HR; IC95%: 5.3; 1.1-24.7) e VEGF-A tumoral (HR; IC95%: 3.8;1.1-13.6). Altas expressões de MMP-9 estromal (HR; IC95%: 0.2; 0.05-0.54) e tumoral (HR; IC95%: 0.2; 0.03-0.8) foram associadas como um fator protetor. Altas expressões de MMP-9, tanto tumoral e estromal, revelaram melhor DSS, enquanto altas expressões de MMP-2, MMP-14 e TIMP-2 no estroma e VEGF-A no tumor associaram-se a uma pior DSS. Conclusões: MMP-9 no estroma e no tumor associaram-se a uma proteção contra óbito pela doença e, continuamente, altas expressões foram associadas a maior DSS. Altas expressões estromais de MMP-2, MMP-14, TIMP-2 e tumorais de VEGF-A foram associadas a um maior risco de óbito pela doença, apresentando-se inversamente proporcionais a DSSAbstract: Introduction: Cervical cancer is one of the most common in incidence and mortality. Expressions of matrix metalloproteases 2, 9 and 14 (MMP-2, MMP-9, MMP-14), tissue inhibitors of metalloprotease 1 and 2 (TIMP-1, TIMP-2) and vascular endothelial growth factor A (VEGF-A), in tumor microenvironment, are involved in the tumor invasion and metastasis by the degradation of the extracellular matrix and angiogenesis. An association of these proteins with prognosis could indicate them as therapeutic targets. Objective: To verify if the expression of MMP-2, MMP-9, MMP-14, TIMP-1, TIMP-2 and VEGF-A in tumor and in the adjacent stroma has a prognostic value for cervical cancer patients. Materials and Methods: It is a retrospective cohort whose sample comes from the 76 patients with invasive cervical carcinoma from a previous study: "Expression of the proteins MMP-2, MMP-9, MMP-14, TIMP-1, TIMP-2 AND VEGF-A in the CIN 3 and cervical cancer" that was conducted at the Women's Hospital Prof. Dr. José Aristodemo Pinotti of the State University of Campinas. From the 76 women, 64 were included. Protein expressions were previously obtained by immunohistochemistry from biopsies containing both tumor and stroma tissue. The expression was defined by the percentage of stained cells and categorized, for this study, in high and low according to the cut-off points defined, individually, by ROC curve. Clinical and morphological data were collected to perform the disease-specific survival (DSS) analysis. The follow-up was collected from the diagnosis to the last clinical visit. The clinical statuses were categorized into: alive without disease, alive with disease, death due to other causes, and death from the disease. MMP-2, MMP-9, MMP-14, TIMP-1, TIMP-2, and VEGF-A expression were associated with tumor stage, histopathologic grade using Cox univariate and multivariate regression models, tested by Hazard Ratio (HR) in a 95% confidence interval (CI95%). For the significant univariate associations (p<0.05) Kaplan-Meier curves were constructed and tested by Log-rank for DSS analysis. Results: In 50% of the advanced stages (II-IV) there were higher MMP-9 expressions in the stroma, while 89.5% in stage I showed high MMP-9 expression in the stroma. The relative risk of death from the disease was associated with high expression of stromal MMP-2 (HR, CI95%: 4.3, 1.3-14.2), stromal MMP-14 (HR, CI95%: 8.8, 1.1-69.6), TIMP-2 stromal (HR, CI95%: 5.3, 1.1-24.7) and tumor VEGF-A (HR, CI95%: 3.8; 1.1-13.6). High expression of stromal MMP-9 (HR, CI95%: 0.2, 0.05-0.54) and tumoral (HR, CI95%: 0.2; 0.03-0.8) was associated with a protective effect. High MMP-9 expression, both tumoral and stromal, revealed better DSS, whereas high MMP-2, MMP-14 and TIMP-2 expression in the stroma and VEGF-A in the tumor were associated with a worse DSS. Conclusions: MMP-9 in the stroma and in the tumor were protective against death from the disease, and consequentely, high expressions were associated with higher DSS.
