Orientador: Elizabeth de Leone Monteiro SmekeTese (doutorado) - Universidade Estadual de Campinas, Faculdade de Ciencias MedicasResumo: Trata-se de um estudo sobre o processo de transformação da assistência ao doente mental, ocorrido no município de Campinas, SP., no período de 1989 a 1991, quando foi proposta a criação de uma nova política para a área. Analisando os princípios que a orientaram, destacam-se temas e questões levantados pelos Movimentos Sociais precedentes - o da Reforma Sanitária e da Luta Antimanicomial-, e pelas experiências que se encaminharam na perspectiva da desospitalização, desinstitucionalização e das ações regionalizadas de atenção ao doente mental. A particularidade da experiência ocorrida em Campinas foi resgatada a partir da análise de documentos e de depoimentos de seus atores sociais, indicando os conflitos e contradições surgidos naquela trajetória. Considerando a importância das transformações das subjetividades dos atores para que pudessem se envolver ativa e criativamente naquele processo, assinalou-se a relevância e pertinência de alguns fatores e acontecimentos - aqui estudados como dispositivos analisadores , que instigaram mudanças necessárias para a implementação daquela política. Destacou-se também a oportunidade e adequação da incorporação do instrumental teórico-prático da Terapia Ocupacional na lida com o doente mental - objetivando sua melhor qualidade de vida, nas inovações efetivadas nas práticas já existentes e na criação de novos serviços. Considerou-se, por fim, que a Terapia Ocupacional, enquanto profissão e área de conhecimento, é elemento essencial para a efetivação de transformações dos paradigmas relacionados à atenção à pessoa em cuidado - como sujeito da sua própria cura, e um recurso importante no processo mesmo de reorientação de relações intra-institucionaisAbstract: This study is about the changing process of the assistance of the mentally ill people, which happened in the city of Campinas, São Paulo, during the period of 1989-1991, when there was suggeted the creation of new policies for the area. Analysing the principIes that oriented that proposal, there were found themes and issues brought up by former Social Movements, sush as the Sanitary Reform and the fight against traditional psychiatric hospitals. There also have been found similarities with other experiences that went in the direction of dehospitalization, deinstitutionalization and regionalized actions of attention to the mentally ill. The particularity of that study was recovering the history of the experience happened in Campinas, through the ana1ysis of documents and testimonies of their social actors, indicating the conflicts and contradictions that appeared along that trajectory. It was focused the relevance and proper manner of some events happened (here studied as ana1ysers devices), that functioned as provoking factors of the changes in the subjectivity of the actors involved. This was the major contribution for the creation and implementation of new mental health policy in Campinas. It was emphasized the opportunity and suitable incorporation of the theoretical and practical tooIs of occupational therapy to deal with the mentally sick. This was done with the purpose of improving their quality. of life and well-being, including new procedures to the already existing practices and also by the development of new services.
