report signs and symptoms – which has neverbeen part of their working role in the past –and use a smartphone to communicate withspecialist palliative nurses, providing them withinformation the nurse would collect during avisit and delivering the care plan developedand managed by the nurse. Working within regulations, the nurse monitorsand delegates appropriate interventions in realtime to the PSW or to a visiting district nursewhere appropriate, ensuring safety andenhancing the quality of the care. This approachleverages the skills of the palliative carespecialist, enabling them to care for fourpatients at the same time when traditionallythey would only be able to care for one.EnComPaSSA recent survey of 200 sets of case notes in thecity of Sheffield, UK, identified possiblealternatives to hospital-based end of life care for40% of cases. The potential savings in acutebed expenditure is £4.5m (~€5.7m) per annum,equivalent to a national saving of £450m. Asimilar, smaller UK study indicated that 52% ofadmissions might have been avoided. According to the Sheffield study: “In terms ofwhere patients had been admitted from, only thenursing home residents showed an aboveproportion who might have avoided admission:17 out of 29 admissions from nursing homes(59%) might have been avoided. In most casesit was felt that the patient could have remainedin the nursing home, or at least returned thereafter suitable assessment, in some cases withadditional specialist or palliative nursinginput.”The study concluded that approximately600 people a year in Sheffield who die in acutecare are admitted from a nursing home.Following alternative care pathways could raisethe percentage of people able to die in theirnursing home from 61% to 80%, thereby almost The majority of healthcare in the future will be provided in thecommunity, and rapidly growing ageing populations arechallenging community care delivery systems. Innovative modelsof collaboration between communities, healthcare providers andbusinesses are required to enable optimal health service delivery and‘ageing in place’ in an economical way. With the ageing of populationsworldwide, living and dying with dignity in one’s own home has becomea global priority. The benefit is twofold: enhanced quality of life forindividuals and substantial cost savings for healthcare systems. Currently, too many people nearing the end of their lives continue toexperience preventable hospitalisation and struggle to access theservices and support they need in a fragmented healthcare system. The goal of the Sam Katz Community Health and Aging ResearchProgram, funded by the Canada Foundation for Innovation (CFI) anddirected by Dr Kloseck, is to develop and test innovative models ofcollaboration between communities, healthcare providers andbusinesses to enable optimal ageing at home, in an economical way,for frail, older individuals. The Enhanced Community Palliative Support Services (EnComPaSS)model in the UK, a consortium of international care providers, researchers,healthcare funders and industry partners led by Dr Fitzsimmons,addresses this significant gap in community-based palliative care service provision.A novel approachIn Canada, a new approach has been developed and is being used todeliver specialist care in the homes of palliative patients. Personalsupport workers work at the bedside in the home of patients requiringpalliative care. Following extensive stakeholder engagement, our community partnershave identified that finding a way to provide high quality, yet affordable,palliative care in the community is of key importance to them, and in ourstakeholder meetings they have identified that the EnComPaSS approachmay well fill this gap.halving the number of people dying in acute carefollowing admission from a nursing home.Supported through the National Institute ofHealth Research (NIHR) Collaboration forLeadership in Applied Health Research andCare (CLAHRC) and the Yorkshire and HumberAcademic Health Science Network (Y&H AHSN),the Ontario approach is being tested in the UKnursing home setting. To provide EnComPaSS in the UK, specialistpalliative nurses employed by a hospice willmanage and mentor trained care assistantsworking in several nursing homes acrossSheffield. Within each study site nursinghome, any residents requiring palliative carewill be cared for by care assistants trained inobservation and reporting, using theCanadian technology to communicate withthe specialist nurses. The trained care assistants will be supported bynurses employed by the nursing homes who willprovide care acts beyond the remit of the careassistants at the request of the remote specialistpalliative nurse.
