find out the effect of OAGB on LFTS and also if a Bilio-Pancreatic Limb(BPL) of 150 cm (OAGB-150) would deliver better outcomes than thatwith a BPL of 200 cm (OAGB-200).Methods: We investigated our prospectively maintained records to ob-tain information on patients who underwent an OAGB-200 or OAGB-150 until 31st July 2018 in our unit.Results: A total of 405 patients underwent an OAGB-200 (n¼ 234) orOAGB-150 (n¼ 171) in our unit between October 2012 -July 2018. Therewere significant improvements in Gamma-Glutamyl-Transpeptidase(GGT) levels at 1 and 2 years after OAGB-200 and significant worseningin the levels of Alkaline Phosphatase (ALP) and albumin at 1 and 2years follow up. There was a significant improvement in GGT levels at1 and 2 years after OAGB-150, and in Alanine Transaminase (ALT) lev-els at 1 year follow up. There was a significant worsening in ALP and al-bumin levels at both follow-up points in this group. There was no sig-nificant increase in the number of abnormal values for any of the testsin either group at both follow-up points. OAGB-150 group had a signifi-cantly lower bilirubin level at 1 year and significantly fewer abnormalALP values at 2 years in comparison with OAGB-200 patients.Conclusion: This study demonstrates the overall safety of OAGB withregards to its effect on LFTs with no remarkable difference betweenOAGB-150 and OAGB-200.P46Effect of the COVID-19 pandemic on hip fracture patients in aTeaching London HospitalShamim Umarji, Rhonda Sturley, Prodromos Tsinaslanidis,Caroline BHingSt George’s University Hospitals NHS Foundation TrustCorresponding Author: Dr. Prodromos Tsinaslanidis (p.tsinaslanidis@nhs.net)Introduction: Hip fractures are the commonest serious injury in olderpeople, the commonest reason for older people needing surgery andthe commonest cause of death following an accident. The NHFD is anational database whereby markers of care are recorded prospectivelyfor patients over 60 years sustaining a hip fracture and requiring inpa-tient care, with a Best Practice Tariff linked to key performance indica-tors. The aim of this study was to review how care varied for patientssustaining a hip fracture during the COVID-19 pandemic compared tothe equivalent time last year.Methods: Using the NHFD we compared data from March – May 2019 tothe same period in 2020 during the pandemic at St George’s Hospital.The study provides a snapshot of care during the pandemic.Results: There were more admissions and more discharges during the pan-demic. There was an increase in time to theatre and subsequent reductionin BPT. There was also an increase in non- operative treatment for thesepatients. 6 patients were confirmed COVID-19 positive, one of whom died.There was a reduction in time to ortho-geriatrician review and also a reduc-tion in delirium review post-operatively. Length of stay was also reduced.Conclusion: The COVID-19 pandemic had an effect on care provided tohip fracture patients. There was an increase in time and a deterioration inorthogeriatric review within 72 hours. The length of stay however im-proved due to improved focus on pathways and discharge processes.There was also a lower threshold to offer non- operative care whereverpossible.P47Evaluating the Relationship of Pre-Assessment Host SystemicInflammatory Response Biomarkers (Neutrophil LymphocyteIntroduction: The host systemic inflammatory response (SIR) is prog-nostic in a wide variety of solid tumours, with a greater degree ofinflammation being associated with a worse clinical outcome.Neutrophil Lymphocyte Ratio (NLR) is an established way to measureSIR in vivo. We aimed to establish whether preoperative NLR correlateswith infective complications in patients undergoing colorectal cancer(CRC) resection.Methods: Data was extracted retrospectively for all CRC resectionsover a 2 year period (2016-2018) at a single district general hospital. NLRwas measured for each patient from bloods at the time of pre-operativeassessment. Post-operative complications were recorded using theClavien-Dindo classification and subsequently classified as infectiveand-talof-P48Otomanagement of acutely symptomatic abdominal wall hernia (ASH) in-cluding groin (GH), umbilical (UH), and incisional hernia (IH). The sur-vey captured respondent characteristics. It explored preferences in themanagement of each type of ASH, including factors related to treat-ment and repair strategy. Surgeons at ST3þ level with a UK practicewere invited to participate through Twitter and collaborative networks.Results: In total, 144 responses were received (response rate 26%). Ofthese, 62 (43.1%) were Consultant Surgeons, 105 (72.9%) did not have a spe-cialist hernia practice, and 95 (66%) did not follow specific guidelines foremergency hernia repair. There was variation in investigations used e.g.CT required for IH (91%) but less for GH (34.7%) and UH (47.2%). Open repairwas preferred in all settings. For GH with ischaemic bowel 76 (52.8%) wouldsuture repair vs. 67 (46.5%) who would use a mesh (64.2% permanent syn-thetic) this is similar for IH, where 59 (41%) chose suture vs. 71 (49.3%)mesh, although 61.9% favoured biologic mesh for IH. Suture repair waspreferred for UH with ischaemic bowel (69.4%).Conclusion: The survey demonstrates variation in treatment of ASH.Areas which require further exploration include use of mesh and lapa-roscopy in the emergency setting.P49Introduction: Social media facilitates the rapid spread of informationand misinformation to the public.
