The project of comprehensive care program for patients with heart failure (HF), arguments for it, its goals, logistics and the description of the pilot phase have been shown in the article. The proposed model of care in patient with heart failure is the next example of the "disease management" program, after the comprehensive care program for patients with myocardial infarction and other health programs of the National Health Fund implemented in 2017. Due to the large number of patients and the multimorbidity accompanying heart failure, it is necessary to conduct a pilot phase prior to full program implementation. The final outcome of the pilot phase should be to determine how changes associated with the new program may affect the frequency of rehospitalization of HF patients, their quality of life and finally their survival.W artykule przedstawiono projekt programu kompleksowej opiekinad chorymi z niewydolnością serca (NS), jego uzasadnieniei cele, rozwiązania organizacyjne oraz założenia fazy pilotażowej.Proponowany model opieki nad pacjentem z niewydolnościąserca jest kolejnym przykładem programu „zarządzania chorobą”,po wdrażanym w 2017 roku programie koordynowanejopieki w zawale serca oraz innych programach zdrowotnychNarodowego Funduszu Zdrowia. Ze względu na dużą liczbęchorych oraz towarzyszącą NS wielochorobowość i trudnościw długotrwałym leczeniu, konieczne jest przetestowanieomawianego modelu w formie pilotażu, którego ostatecznymwynikiem powinno być rozpoznanie, w jakim stopniu związanez programem zmiany pozwolą na zmniejszenie częstościhospitalizacji, poprawę jakości życia i w końcu przełożą się nawydłużenie życia chorych z NS
