Istnieje coraz więcej danych, że nadciśnienie tętnicze pierwotne u dzieci i młodzieży nie jest
izolowanym zaburzeniem hemodynamicznym, ale złożonym zespołem immunometabolicznym,
a podwyższone ciśnienie tętnicze jest tylko jednym z elementów tego procesu. Istotne
znaczenie w patogenezie tych zaburzeń mają otyłość trzewna i nieprawidłowe relacje
między podskórną i trzewną tkanką tłuszczową. Wczesne, subkliniczne uszkodzenie narządowe
stwierdza się już u około 40% nastolatków z nadciśnieniem tętniczym pierwotnym,
a ryzyko wystąpienia uszkodzenia narządowego jest związane z narażeniem na towarzyszące
zaburzenia metaboliczne i immunologiczne oraz ilością trzewnej tkanki tłuszczowej. Leczenie
hipotensyjne oparte na lekach blokujących układ renina-angiotensyna (RA, renin-
-angiotensin) i równolegle prowadzonym leczeniu niefarmakologicznym zapewnia osiągnięcie
normotensji u 70% młodych osób z nadciśnieniem tętniczym pierwotnym i normalizację
zaburzeń metabolicznych. Jednak głównym predyktorem regresji uszkodzenia narządowego
jest nie tyle obniżenie ciśnienia tętniczego, ale zmniejszenie ilości tłuszczu trzewnego,
względne zwiększenie ilości tłuszczu podskórnego i normalizacja zaburzeń immunologicznych.

syndrome. Visceral obesity and disturbed relationship between subcutaneous and
visceral fat depot, together with elevated blood pressure are regarded as the main cardiovascular
risk factors. 40% of children and adolescents with newly diagnosed primary hypertension
are at risk of target organ damage at the start of diagnosis what is enhanced by metabolic and immunological abnormalities related to visceral obesity. Compilation of standard
pharmacological therapy based on ACE inhibitors with non-pharmacological treatment
leads to normalization of blood pressure and metabolic profile in 70% of hypertensive children
and is potentially beneficial in preventing long-term complications. But instead of blood
pressure lowering, decrease of the ratio between visceral and subcutaneous fat depot is
considered to be the main predictor of regression of target organ damage. (Forum Zaburzen
