Such plaques,which cannot be detected by commonly-used testssuch as a treadmill examination or even coronaryangiography, are suspected to be the cause of mostsudden cardiac deaths and non-fatal heart attacks.The InfraReDx LipiScan Imaging System usesinfrared imaging to detect lipid core-containing pla-ques of interest and assess a patient’s coronaryartery lipid core burden index. The LipiScan Coro-nary Imaging System is a catheter-based device thatuses laser light to detect how much fat and othersubstances are contained in a plaque.Near infrared (NIR) diffuse spectroscopy isa technique based on the absorption of light in the NIRspectrum, in a specific manner, by organic molecu-les. NIR spectroscopy has demonstrated the abilityto identify plaques with lipid pools through blood [1].The device works by placing a catheter equip-ped with a fiber-optic laser into the artery. The de-vice shines the near infrared light through the blo-od to the artery wall and measures the light reflec-ted back from it, a technique called spectroscopy.The reflected wavelengths vary depending on howmuch fat and other substances are in the plaque inthe illuminated portion of the wall.The identification of the chemicals present isbased on the differential absorption of light in theNIR spectrum by different molecules. An importantfeature of near-infrared light is that it can penetra-te tissue and can therefore identify a tissue despitethe presence of blood between the detector and thetarget. This is an important advantage for imagingwithin the human coronary artery.The system consists of a laser light source, anautomated pullback and rotation device and a smallfiber-optic catheter. While the catheter is similarin size and ease of use to an intravascular ultraso-und catheter, the information it provides is quite dif-ferent since it is based on an optical rather that anultrasonic signal (Fig. 1).The NIR system obtains signals from patientsthat are analyzed with algorithms validated by com-parison to tissue histological findings in ex-vivocoronary specimens. It is therefore possible to per-form a pullback in a patient’s artery and provide animage of the NIR signals which indicate the presen-ce of lipids and other chemicals of interest. It isexpected that these images, which are called Intra-vascular Chemograms™, will provide information forinterventional cardiologists to help in the care ofpatients already undergoing cardiac catheterizationfor coronary events. The identification of the che-mical composition of coronary plaques is of value tocardiologists in the selection of medical, stenting orsurgical therapy for coronary lesions. The device isalso expected to be of value to the pharmaceuticalindustry as a means to assess the effect of novel anti-atherosclerotic agents on lipid core plaque burden.387S.
