BisognanoCardiology Division, University of Rochester Medical Center, Rochester, NY, USAA 74 year old experienced transient severechest pain after choking and receiving the Heim-lich maneuver. Troponin T was elevated and anechocardiogram showed an ejection fraction of 30%with anterior hypokinesis. Coronary angiographyrevealed a myocardial bridge in the mid-left ante-rior descending artery. Since stress nuclear imagingrevealed no ischemia, she was treated medically andhad no further episodes of chest pain.While myocardial bridging is not usually clini-cally significant, this case shows an unusual mani-festation of the catecholamine surge associated withchoking. The resultant tachycardia and increase incontractility likely caused coronary blood flow to becritically reduced, resulting in infarction of the an-terior wall [1].Reference1.
