Antecedent infections and anti-glanglioside antibodies in Guillain-Barré syndrome. Their role in pathogenesis and heterogeneity

The Guillain-Barre syndrome (GBS) is the most common form of acute neuromuscular
paralysis in developed countries, but the pathogenesis is still largely unknown.
The major clinical features of the syndrome were first united by J-B.O. Landry in 1859
(1). The syndrome was named after G. Guillain and J.A. Barre, two French army
neurologists, who in 1916 described, together with A. Strohl, the typical findings in the
cerebrospinal fluid (2). This 'dissociation albumino·cytologique', the combination of
elevated protein and normal cell count, was in those days an important diagnostic
feature that further distinguished the syndrome from other neurological diseases, like
poliomyelitis. GBS was early recognized as a postinfectious disorder, although the
infectious agents were not identified. Since then, major efforts have been made to
elucidate the pathogenesis of GBS. In the last years, research in this field was dominated
by the discovery that infections with the Gram negative curved rod Campy/abaeter
jejunifrequently precede GBS and by the finding of antibodies against various peripheral
nerve gangliosides in serum from GBS patients. This introduction will focus on these
infections and antibodies, and will provide a background for understanding the studies
described in this thesis

De klinische en electrodiagnostische manifestaties bij patiënten met het syndroom van Guillain-Barré (GBS) worden deels bepaald door de antece-dente infecties en de aanwezigheid van anti-ganglioside antistoffen. Dit proefschrift. 2. De lipopolysacchariden van Campylobacter je juni vertonen een sterke moleculaire gelijkenis met gangliosiden uit perifere zenuwen. Infecties met Campylobacter je juni kunnen derhalve leiden tot de vorming van kruisreagerende antistoffen tegen deze gangliosiden. Dit proefschrift. 3. GBS-patiënten met Campylobacter je juni-infecties en anti-GM l-antistof-fen vertonen een beter herstel na intraveneuze toediening van immuno-globulinen dan na plasmaferese. Dit proefschrift. 4. Anti-GQ1 b-antistoffen zijn sterk geassocieerd met het Miller Fisher syn-droom en met het optreden van ophthalmoplegie bij patiënten met GBS. 5. Het onderzoek naar antiganglioside-antistoffen wordt sterk bemoeilijkt door het ontbreken van een gestandaardiseerde, internationaal aanvaarde methode voor het aantonen van deze antistoffen. 6. Het 'experimental autoimmune neuritis' model, waarbij proefdieren worden geïmmuniseerd met zenuwweefsel, heeft slechts een minimale bijdrage geleverd aan het begrijpen van de etiologie en pathogenese van GBS. 7. Een primaire deficiëntie van de normale immunosuppressie in het centrale zenuwstelsel speelt waarschijnlijk een belangrijke rol bij het ontstaan van multipele sclerose. 8. De omvang van het geneesmiddel onderzoek is omgekeerd evenredig met de kans dat het middel een bepaalde patiënt zal helpen en evenredig met het aantal patiënten dat behandeld gaat worden zonder duidelijk nut, maar dat daarentegen wel blootgesteld wordt aan de risico's en bijwer-kingen van de therapie. Om co-auteurs voldoende scherp te houden bij het beoordelen van een manuscript, verdient het aanbeveling hierin enkele kleine maar opzichtige fouten op te nemen. 11.
