DURHAM, N.H. -- The Institute on Disability at the University of New Hampshire, in
collaboration with the NH Department of Health and Human Services, will present a
conference – Real Choices for Older Adults: Transforming Long Term Systems of Support on
Tuesday, Oct. 3, highlighting best practices in integrating supports for persons who are aging.
Real Choices for Older Adults: Transforming Long Term Systems of Support
Tuesday, October 3, 8:30 a.m. to 3:45 p.m.
Center of New Hampshire Radisson Hotel, Manchester, N.H.
Registration: $50 professionals; $25 consumers and family members

This conference will highlight federal initiatives, the experience of other states, and best
practices in NH in integrating community-based long term services and supports. Keynote
speakers will include representatives from the Centers on Medicaid and Medicare Services and
the Administration on Aging regarding current federal initiatives, as well as Patrick Flood,
Commissioner at the Department of Disabilities, Aging, and Independent Living in Vermont.
Individual breakout sessions will focus on current initiatives in New Hampshire related to
access to services, quality assurance and improvement, housing and supports, consumer
direction, and community engagement.
The N.H. Department of Health and Human Services, in collaboration with the University of
New Hampshire and numerous community partners, is engaged in a significant initiative to
transform the community-based system of long term support for older adults and adults with
disabilities. This work is being supported through several Real Choice grants awarded through
the Center for Medicare and Medicaid Services (CMS) within the US Department of Health and
Human Services.
This conference is sponsored by NH’s Real Choice Grants: Quality Assurance and Quality
Improvement, Systems Transformation, Service Link Resource Centers, and Home Care
Connections; Division of Community Based Care Services, NH Department of Health and
Human Services; and the Institute on Disability at the University of New Hampshire.
The Institute on Disability at the University of New Hampshire was established in 1987 to
provide a coherent university-based focus for the improvement of knowledge, policies, and

practices related to the lives of persons with disabilities and their families. Its mission is to
