AJNR Am J Neuroradiol. 26: 2200-2206，2005. 研究成果脳の拡散テンソル(Diffusion-Tensor、DT)磁気共鳴(MR)画像法は、脳組織内の水分子の拡散の大きさと方向性を非侵襲的に定量測定することで脳の発達、加齢、病変等を評価可能な手段として広く臨床応用されている。拡散テンソルMR画像法によって得られるいくつかのパラメーターの中の一つに異方性(水分子の拡散に一定の方向性が認められるもの)の指標がある。正常の脳梁をはじめとした白質は多くの場合のこの異方性を有しているが、ヒト新生児期脳の白質内水分子拡散異方性の増加が髄鞘化と時期的な関連を示す事から、白質内髄鞘構造がこの異方性に大きく寄与する事が考えられている他、軸索及びその走行形態の寄与も示唆されている。このことに着目し、脳白質内での水分子拡散の方向性をprobeとして用いることにより脳内神経束、神経路構造を可視化する拡散テンソルMRtractographyが提案され、生体脳において脳内の各部分の連結性を調べる手段として研究が進んでいる。これらの研究のなかに脳腫蕩性病変と周囲の神経東との関連を調べる試みが含まれており、大きくは神経束が腫療により押し退けられる(displacement)、腫蕩中を通り抜ける(infiltration)、腫蕩部分で途絶している(disruption)3つの状態が存在することが観察され、腫蕩の悪性度との関係、も含めて議論がなされている。しかし、これらの臨床研究中で使用されている数mmレベルの解像度の画像で可視化されている神経束と考えられる腫蕩周囲の構造が実際は何を反映しているかという点についてはいまだ明らかではない。本研究では、 ENU(N-ethyl-N -nitrosourea)誘発gliomaを持つラットの標本脳拡散テンソルMR画像から各画素内の水分子拡散の方向性とその大きさを可視化した高分解能vectorマップを作成し腫蕩と周囲脳組織の方向性を持つ構造との関係を描出した後に同じ断面の組織標本を作成し比較検討することで、vectorマップの示す構造が組織学的にどのような構造に起因しているかを、特にneuro-五lamentsとの関係、に着目して明らかにすることで、 DTMR画像法がN-エチノレ・N-ニトロソウレア(ENU)誘発ラット脳腫蕩について、腫療と白質内神経線維構造との関係を評価可能かどうか検討した。材料及び方法実験動物経胎盤性にENU(N-ethyl-N -nitrosourea)を投与し生後脳神経勝腫を発生させたFisher-334ラット 6匹。DTMR画像撮像用標本ENU誘発脳腫蕩ラットを生後231・258日目にparaformaldehydein 0.1 M phosphate buffer (pH 7.2)で経心性に潅流した。取り出した脳のうち腫蕩が含まれた部分を 5mm厚に切り出し0.1M phosphate bufferで満たした 10mm径自作撮像用チェンパーに封入し撮像した。組織標本作製DTMR撮影後40Cに保った国定液中で3日間固定後パラフィン包埋し、Hematoxylin -eosin(HE)染色及び免疫組織染色(neurofilament)を施した。当実験に実験動物を使用すること及び実験操作に伴う動物の取り扱いに関しては滋賀医科大学動物実験委員会承認を受けた(2003・11・7)。DTMR画像撮像(心撮像装置Horizontal 7T/40 cm magnet (Fig.1a) (Japan Superconductor Technology Inc.， Kobe， JAPAN) 12cm gradient set up to 400mT/m (Magnex Scientific Ltd.， Abbingdon， uK) UNITYINOVA NMR system (VARIAN Inc， Palo Alto， CA， USA) a handmade coil with an inner diameter of 12mm (Fig.1b) (b)収集画像及び撮像条件(1)diffusion weighted images with diffusion sensitization ofb-value = 1027 sec/mm2 in six different noncolinear directions (2)T2-weighted (bO) image without diffusion sensitization 2000/36/12・14(TR msec 1 TE msec 1 averaging) 24 mm field of view， 0.5 mm slice thickness 256 x 256 scan matrix (0.094 x 0.094 x 0.5 mm3) 画像解析(a)テンソル計算6組のDW画像と T2強調画像データからボクセルごとの拡散テンソルの要素を多変量線形回帰をもちいて計算しこれを対角化することで 3つの固有値と 3つの固有ベクトルを得た。(b)Color map作成各ボクセルの 3つの固有値のうち、最も大きい国有値に関連した固有ベクトルが局所のベクトルの方向をあらわすと仮定した。異方性の指標としてfractionalanisotropy (FA)を使用し、 Colormapをこの FA値(輝度)と上記の最も大きい固有値に関連した固有ベクトノレの3つの要素から作成した。(c)Vector map作成color map作成時の最も大きい固有値に関連した固有ベクトルの要素のうち、表示2次元画像の 2軸に関連した 2要素を用いてピクセル毎にベクトルの方向を線分で表示したvectormapをFA値0.45以上のピクセルについて作成し、 T2強調画像上に重ね合わせて表示した。これらの画像作成に必要な計算や画像表示はプログラム開発用ツール IDL(Interactive Data Language; Research Systems)を用いて作成した自作アプリケーションを使用した(Appendix)。検討項目vector mapの線分が腫療によるneuro宣lament(NF)構造の改変を反映しているかどうかをNF免疫染色を施した組織標本と視覚的に比較することで検討した。また、 T2強調画像での信号変化との関係も検討した。結果(1) vector mapでは、組織標本で認められる白質内の整列した太いNF東を反映すると考えられる整列した線分群の描出が可能で、あった(Fig.2a，2b)。(2) T2強調画像で正常白質の信号が失われている部分でも、組織標本で NF束が保たれている部分はvectormapで残存NF束を反映すると考えられる線分が認められた(Fig.3a，3b，4a，4b)。(3)組織標本で認められる腫蕩・白質境界部のNF東の膨隆、途絶はvectormap で線分の連続性、整列性の途絶として明瞭に描出可能であった(Fig.5a，5b)。(4)腫療中に散在性に残存する微細なNF東はvectormapでは十分描出されなかった(Fig.2a，2b)。考察MR画像は従来、コントラストとしてプロトン密度、 Tl緩和やT2緩和の差、流れの情報などを画像化してきたが、様々な因子が複雑にTl緩和やT2緩和に寄与するため、これらの画像から組織内に生じている病理学的な微細な構造変化を特定する事やさらに腫蕩の悪性度について確かな情報を提供することは容易で、はなかった。ヒト脳腫療における DTMR画像法の利用法として、治療計画のための神経路と腫蕩との関係の評価、腫療部や腫蕩周囲の等方性拡散、異方性拡散を示す指標を計測することでtissuecharacterizationの試みや転移性腫蕩と神経鯵腫の鑑別の可否などが検討されている。しかし疾患の性格上、画像と組織標本との直接の比較検討は困難であることが多い。そこでENU経胎盤投与により誘発したラット脳腫蕩について組織標本と DTMR画像法で得られた計算画像とを用いて、腫療と白質の神経線維構造との関係を評価可能かどうか検討した。T2強調画像での白質内異常信号域は必ずしも単一の病理学的変化を示すのではなく、浮腫や腫蕩浸潤などの多様な変化を反映しているものと考えられているが、 DTMR画像法で得られた計算画像を用いて白質内線維構造を反映していると考えられる異方性の改変を評価することで、 T2強調画像では区別できないこの多様な変化を捉えられる可能性が示唆された。 vectormapは比較的まとまった NF束の描出は可能と考えられたが、組織標本と計算画像の厚さに差がある上、断面の角度も完全には一致しておらず微細な NF構造描出の詳細な比較は不可能で、あった。高分解能vectorマップと組織標本とを詳細に比較することで、従来のMR画像法が提供できなかった腫蕩の悪性度に関する情報や腫蕩の周囲組織についての情報が得られるようになれば、治療の選択や外科的切除に際しての切除範囲の検討の際に新たな有用な情報を提供しうる可能性がある点で意義あるものと考えられる。DTMR画像法で得られたvectormapは白質神経線維の様相を反映した画像を提供することで T2強調画像では捉えられない脳腫蕩進展範囲の評価に利用できる可能性が示された。Fig.la MR unit Fig.lb homemade coil T2WI FAmap Color map Vector map Fig.2a;組織標本で白質内に僅かに腫蕩細胞が認められ白質構造は軽度膨隆しているが(拡大部)NF束の構造はほぼ保たれている。 vectormap及び、colormapで、も白質内に線分の乱れは認めない。，?，???Fig.2b NF; 脳腫瘍及び周囲組織の高分解能拡散テンソル磁気共鳴画像:免疫組織化学標本との比較に関する総合的研究

科学研究費補助金研究成果報告書研究種目: 基盤研究(C)研究期間: 2003～2004課題番号: 15591267研究代表者: 井藤 隆太（滋賀医科大学・医学部・講師）研究分担者: 中州 敏（滋賀医科大学・医学部・助教授）脳の拡散テンソル(Diffusion-Tensor)磁気共鳴(MR)画像法で脳腫瘍と腫瘍周囲白質の神経束をはじめとした微細構造がどのように描出されるかを、免疫組織化学標本と比較検討することで明らかにした。  Fisher-334ラットに対し経胎盤性N-ethyl-N-nitrosourea投与により生後脳腫瘍を誘発し脳腫瘍モデル6匹を得た。脳を灌流固定後、実験用7T-MR装置を用い6方向に拉散検出用磁場を印加した拡散強調画像と検出用磁場を印加しないT2強調画像を収集した  (2000/36/12-14(TR msec/TE msec/averaging),24mm field of view,0.5mm slice thickness,256 x 256 scan matrix, b-value 1027sec/mm^2)。これらの画像からcolor map及びvector mapを作成した。また画像収集後病理組織切片を作成しneurofilament染色を施した。組織標本のneurofilament成分を基準にcolor map及びvector mapが腫瘍周囲の腫瘍浸潤に関する情報を提供するかどうかをT2強調画像と視覚的に比較し以下の結果を得た。  (1)vector mapでは、組織標本で認められる白質内の整列した太いneurofilament(NF)束を反映すると考えられる整列した線分群の描出が可能であった。  (2)T2強調画像で正常白質の信号が失われている部分でも、組織標本でNF束が保たれている部分はvector mapで残存NF束を反映すると考えられる線分が認められた。  (3)組織標本で認められる腫瘍-白質境界部のNF束の膨隆、途絶はvector mapで線分の連続性、整列性の途絶として明瞭に描出可能であった。  (4)腫瘍中に散在性に残存する微細なNF束はvector mapでは十分描出されなかった。We investigated fiber structures in the peripheral zone of N-ethyl-N-nitrosourea (ENU)-induced rat brain glioma by comparison between diffusion tensor MR (DT-MR) images and neurofilament (NF) immunostained histological sections.  In this study, six Fisher-334 rats, which had been treated transplacentally with ENU, underwent in vitro MR imaging. All procedures met institutional Laboratory Animal Care and Use Committee approval. The experiments were performed on a horizontal 7T/40 cm magnet equipped with a 12cm gradient set up to 400mT/m interfaced to an experimental NMR system. The fixed brain was put into the tube filled with 0.1M phosphate buffer for in vitro DT-MR imaging. Acquisition parameters were 2000/36/12-14 (TR msec/TE msec/averaging), 24mm field of view, 0.5mm slice thickness, 256x256 scan matrix (0.0938x0.0938x0.5mm^3). A data set of diffusion-weighted images with b-value=1027sec/mm^2 in six different noncolinear directions and T2-weighted images was obtained. From the diffusion-tensor data, DT-MR imaging-based color maps and vector maps were generated. After DT-MR imaging, neurofilament immunohistochemistry was performed using the avidin-biotin-peroxidase complex method. We visually evaluated peritomoral white matter architecture described by the vector maps in comparison with corresponding neurofilament (NF) immunostained histological sections.  We obtained the following results. (1)In normal white matter, the vector map could depict well-ordered vector bars corresponding ordered NF structures in immunostained histological sections. (2)The vector map could clearly show peritumoral widening and disruption of the vector bars along the border between normal white matter and tumor, which were confirmed by the histological sections. (3)The vector maps showed well-ordered vector bars in the peritumoral area with abnormal hyper intensity on T2-weighted images, where histology revealed normal well-ordered NF bundles without tumor cells in the area. (4)The vector map could not fully describe scattered residual NF bundle pieces in the tumor
