The Fournier's gangrene was first described in 1883 by Jean Alfred Fournier. It is defined like a fulminant and progressive infection of genital and perineal regions or even in both of them with a thrombosis of small subcutaneous vessel, which originates gangrene in the overlap skin. The bacterial flora is mixed. The diagnosis must be established in an early stage. The treatment with antibiotics and the surgical debridement increase the survival. It is presented a case of a 60-year old white male that suffered from a Fournier's gangrene with some affectations in the perineal zone, scrotum and inguinal region as well. The patient was treated immediately and submitted in several times to the necrectomía and toillet of mentioned region with a derivative colostomy what left as sequel a deformity of above mentioned region. It was necessary a perineal reconstruction with a plastia of sphincters. The patient evolved adequately and had no difficulties concerning the anal continence.La gangrena de Fournier fue descrita en 1883 por Jean Alfred Fournier, se define como una fulminante y progresiva infección de las regiones perineales y genitales o ambas, con trombosis de los vasos subcutáneos pequeños que origina gangrena de la piel suprayacente. La flora bacteriana es mixta. El diagnóstico debe realizarse tempranamente. El desbridamiento quirúrgico y el tratamiento con antibiótico aumentan la supervivencia. Se presenta un paciente de 62 años de edad, sexo masculino y raza blanca que presentó gangrena de Fournier y afectaciones en la región perineal, el escroto y la región inguinal. Fue tratado de forma inmediata y se le realizaron, en múltiples ocasiones, necrectomía y toillet de dicha zona con colostomía derivativa, todo lo que dejó como secuela deformidad de dicha región.
