A white elderly patient, with a history of hypertension, gout and type 2 diabetes mellitus was admitted with severe cough, dyspnea, marked asthenia and hematemesis. She evolved unfavorably presenting hemoptysis, basic acid imbalances and died in irreversible shock with a clinical diagnosis of Goodpasture syndrome; at necropsy, it was concluded to be a microscopic polyangiitis.Paciente anciana, blanca, con antecedentes de hipertensión arterial, gota y diabetes mellitus tipo 2 que ingresó con tos intensa, disnea, hematemesis y astenia marcada, evolucionó desfavorablemente con hemoptisis, desequilibrios ácido básicos y falleció en choque irreversible con el diagnóstico clínico de síndrome de Goodpasture; en la necropsia se concluyó poliangeítis microscópica
