Kinderen die worden geboren met een complexe aangeboren hartafwijking, waarbij slechts één hartkamer goed ontwikkeld is en de andere hartkamer onderontwikkeld of afwezig is, hebben meestal al op jonge leeftijd een operatie nodig. De Fontan procedure is ontwikkeld om patiënten met een dergelijk functioneel univentriculair hart (één-kamer-hart) te behandelen. Deze procedure resulteert is een unieke, niet-fysiologische circulatie, waarin slechts één hartkamer zowel de pulmonale als de circulatoire circulatie ondersteunt. Achteruitgang van de Fontan circulatie, meest waarschijnlijk gerelateerd aan een on-fysiologische karakteristieken van de circulatie, is een opkomend probleem waarmee een groeiende groep patiënten geconfronteerd wordt. In dit proefschrift hebben wij aangetoond dat de uitkomst na de Fontan operatie verbeterd is in de afgelopen decennia. We hebben echter ook geïdentificeerd dat de Fontan circulatie nog steeds meerdere nadelige gevolgen heeft, zoals een beperkt inspanningsvermogen, verminderde hart- en longfunctie, en longvaatremodelering. Verder hebben wij met de hulp van een nieuw toegepaste magnetische resonantie (MRI) techniek gezien dat het ontwikkelen van leverziekte bij Fontan patiënten veel vaker voorkomt dan voorheen werd gedacht. Tenslotte hebben wij gedemonstreerd dat de behandeling van uiteindelijke falen van de Fontan circulatie is geassocieerd met een aanzienlijke mortaliteit, en dat de ene chirurgische ingreep geen voordelen toont boven de andere. Concluderend is de overleving na de Fontan operatie toegenomen in de afgelopen decennia, en zouden de inspanningstolerantie, een biomarker genaamd NT-proBNP en de ontwikkeling van leverziekte potentiële markers kunnen zijn voor een achteruitgang van de Fontan circulatie in de toekomst.Children who are born with a complex congenital heart disease, with only one well-developed ventricle and one underdeveloped or absent ventricle, usually require a surgical intervention at a young age. The Fontan procedure is designed to treat patients with this so-called functionally univentricular heart (one-chamber-heart) and who are not suitable for a biventricular (two-chamber) repair. The procedure results in a unique, unphysiologic circulation, in which only one ventricle sustains both the pulmonary and the circulatory circulation. Attrition of the Fontan circulation, most likely related to the unphysiologic characteristics, is an emerging problem currently faced by a growing cohort of patients. In this thesis, we demonstrated that the outcome after the Fontan operation has improved over the past decades. However, we also identified that the Fontan circulation is still associated with a multiple sequelae, including limited exercise tolerance, restricted cardiac and pulmonary function, and pulmonary vascular remodeling. Furthermore, with the help of a newly applied magnetic resonance technique, we recognized that the development of liver disease is far more common in Fontan patients than previously thought. Finally, we demonstrated that the surgical treatment for eventual failure of the circulation is associated with considerable mortality, and does not favor one surgical option above another. In conclusion, survival after the Fontan procedure has improved over the past decades, and the exercise tolerance, a biomarker called NT-proBNP and the development of liver disease may all be potential markers of Fontan attrition in the future
