Expressions of brain-derived neurotrophic factor 
（BDNF） in cerebrospinal ﬂuid and plasma of chil-
dren with meningitis and encephalopathy

（Background and aims）　Many reports in the field of 
childhood brain disorders have shown that brain-derived 
neurotrophic factor （BDNF） affects central nervous 
system (CNS) functions.　In this clinical study, BDNF 
levels were evaluated in association with pediatric CNS 
infections, especially acute meningitis and ence phalo-
pathy.
（Methods）　BDNF levels were measured in the serum 
and cerebrospind ﬂuid (CSF) of 42 patients admitted due 
to CNS infections over a 5-year period using ELISA.　
Control samples were collected from 108 patients with 
non-CNS infections.
（Results）　BDNF levels were below the sensitivity of 
the ELISA in most CSF samples from the control group, 
but were elevated in patients with bacterial meningitis 
and inﬂuenza-associated encephalopathy （IAE）.　Serum 
BDNF levels were elevated in all subgroups of patients 
with CNS infections, particularly those with bacterial 
meningitis.　In subgroups of encephalopathy, BDNF 
levels in the serum and CSF were higher in a subgroup 
with cytokine storm than in others, and most patients in 
the subgroup had neurological sequelae.
（Conclusions）　BDNF levels were elevated in the 
serum and CSF of pediatric patients with CNS infections, 
particularly those with bacterial meningitis and IAE.　
Monitoring changes in the serum and CSF levels of 
BDNF may allow for the differential diagnosis of speciﬁc 
CNS infections.

VBM画像を用いたMSA-C画像病期分類の検討

【はじめに】　我々は小脳萎縮をきたす疾患の病型分
類に VBM（Voxel-based morphometry）画像が有効
であることを報告している。今回MSA-Cにおいて
萎縮部位の進展を調べる目的で、VBM画像を画像
病期別に分類した。
【方法・対象】　対象は 2007年以降に VBM画像で
の評価をおこなったMSA-C 24例・計 40回分。発
症年齢は 48-77歳（中央値 59歳）、男性 7例女性
17例、VBM画像撮影時の罹病期間は 6-129か月（中
央値 37か月）であった。MSA-Cの診断は、Gilman
らの診断基準で probableまたは possibleの症例で小
脳症状が前景に立つものとした。VBM画像解析に
は VSRAD advanceを用い、MRI脳画像の灰白質
（GM）と白質（WM）を分離・抽出し、得られた
Z-score画像をもとに GMを 4段階に、WMを 5段
階に grade分けをおこなった。vbSEE解析をおこな
い各 grade別の平均画像を作成した。脳幹・小脳・
大脳の各領域における Z-score2以上の異常座標を
示したボクセル数と、その領域の Z-score平均値を
もとめた。また罹病期間・歩行能力（SARA : 歩行
点数）との関連性についても検討した。
【結果】　罹病期間と GM および WM の grade、
Z-score値、歩行点数は高い相関がみられた。GM
においては、各小脳半球に Z-score（以下 Z）2-3の
分布をみとめ（GM1）、Z4へと拡大進行し（GM2）、
片側が Z5-6へと障害され（GM3）、最終的には両
側半球が Z6へ高度に障害された（GM4）。小脳 GM
の Z-score値と高い相関を示した。WMにおいては、
中小脳脚より始まり（WM1）、橋へ萎縮が波及 
（WM2）。片側の中小脳脚が Z5-6と高度となり
（WM3）、やがて両側中小脳脚へ拡がり（WM4）、
最終的には橋・両側中小脳脚が Z6へ高度に障害さ
れた（WM5）。小脳および脳幹WMの Z-scoreと高
い相関を示した。WM grade分けは視覚的に伸展様
式を把握しやすく、GM grade分けは罹病期間・歩
