Endoscopic Pancreatic Duct Stents Reduce the In-
cidence of Post-Endoscopic Retrograde Cholangi-
opancreatography Pancreatitis in High-Risk 
Patients -RANDOMIZED CONTROLLED MUL-
TICENTER TRIAL-

　BACKGROUND & AIMS : Pancreatitis is the most 
common and potentially serious complication of post-en-
doscopic retrograde cholangiopancreatography （ERCP）.　
Post-ERCP pancreatitis （PEP） is caused mostly by post-
procedural papillary edema and retention of pancreatic 
juice.　We conducted a randomized controlled trial to 
determine whether placement of a temporary-type, pan-
creatic duct stent prevents PEP and to identify risk fac-
tors for PEP.
　METHODS : We analyzed data from 426 consecutive 
pa- tients who underwent ERCP-related procedures at 37 
endoscopic units.　The patients were assigned randomly 
to groups that received stents （S group, n=213） or did 
not （nS group, n=213）.　The stent used was temporary, 
5F in diameter, 3 cm long, and straight with an unÀanged 
inner end.
　RESULTS : The overall frequency of PEP was 11.3%.　 
The frequencies of PEP in the S and nS groups were 
7.9% and 15.2%, respectively ; the lower incidence of 
PEP in the S group was statistically signi¿cant based on 
the full analysis set （P=.021）, although there was no sta-
tistically significant differences in an intention-to-treat 
analysis （P=.076）.　There were significant differences 
in PEP incidence between groups in multivariate analysis 
for the following risk factors : pancreatography first, 
nonplacement of a pancreatic duct stent after ERCP, pro-
cedure time of 30 minutes or more, sampling of pancre-
atic tissue by any method, intraductal ultrasonography, 
and difficulty of cannulation （15 min）.　Patients with 
more than 3 risk factors had a signi¿cantly greater inci-
dence of pancreatitis.
　CONCLUSIONS : Placement of a pancreatic duct 
stent reduces the incidence of PEP.　Several risk factors 
are associated with PEP.

T4食道癌に対する Docetaxel, CDDP, 5-FUを用
いた化学放射線療法の治療成績

【緒言】　T4食道癌に対して従来より FPを用いた根
治的化学放射線療法が行われているが、その治療成
績は極めて悪く新たな治療戦略が望まれる。
【目的】　2007年 4月より StageIV食道癌に対する治
療成績向上を目指し、FPに Docetaxelを加えた 3剤
による化学放射線療法（DCF-R）を開始し、良好な
成績が得られているため報告する。
【対象】　DCF-R を行った StageIV症例のうち T4食
道癌 14例を対象にその治療成績を検討した。
【方法】　DCFによる化学療法 2コースに放射線
60 Gyとした。SD以上の症例に対しては維持療法
として 4カ月毎に 1コースの DCF療法を行った。
治療に伴う腫瘍の縮小により大動脈破裂の可能性が
高い症例には、治療前に大動脈内にステントグラフ
トを留置した。
【成績】　14例中 N4は 3例、M1は 1例であった。
治療寛遂率は 100%で、関連死亡は認めなかった。
11例に Grade 3以上の白血球減少を認め、うち 4例
は Grade 4であった。また Grade 3以上の SIADHに
よる電解質異常を 3例に認めた。治療の 1次効果は
CR : 7例、PR : 5例、PD : 2例であった。観察期
間は 127～790日（中央値 : 522日）、生存期間は
127～790日（中央値 : 436日）で、14例中 10例が
生存中である。
【結論】　DCF-Rは T4食道癌に高い治療効果を示
し、新たな治療法として極めて有用であると考えら
れた。
