AT製剤、プロテアーゼインヒビター等）を行った。
本症例に対し、早期より活性型第 VII因子製剤を投
与したがその効果は充分とはいえなかった。その理
由として、低フィブリノゲン血症、低体温、および
血小板数低下などが存在していたためと考えられ
る。そのため外科的処置が行われるまでに時間を要
した点は反省すべきであったかもしれない。集学的
治療により救命し、合併症なく第 8病日に一般病棟
へ転床となった。

Fas/Fas-L and Bax/Bcl-2 systems in germ cell 
death in experimental autoimmune orchitis in-
duced by immunization with syngeneic testicular 
germ cells alone

　Experimental autoimmune orchitis （EAO） is charac-
terized by T cell-dependent lymphocytic inflammation 
and damage to seminiferous tubules, causing death of 
testicular germ cell （TGC）.　The aim of the present 
study is to investigate the role of Fas/Fas-L and Bax/Bcl-
2 systems in the TGC death in EAO induced by immuni-
zation with syngeneic TGC alone in mice, using real time 
RT-PCR and immunostaining.　The results showed that 
the many Tdt mediated dUTP nick end labeling positive 
TGC were found at the active EAO stage and persistently 
observed in the seminiferous epithelium until the post-
active EAO stage.　Intra-testicular mRNAs expression 
of both Fas and Bax increased at the active EAO stage 
and dramatically decreased at the post-active EAO stage.　
In contrast, intra-testicular the mRNAs expression of 
both Fas-L and Bcl-2 did not show signi¿cant changes at 
the active EAO stage but extremely increased at the post-
active EAO stage.　Immunohistochemically, some Fas 
or Bax positive TGC were detected at the active EAO 
stage and hardly found at the post-active EAO stage.　In 
contrast, some Fas-L or Bcl-2 positive TGC were found 
at the active EAO stage but many of them were observed 
at the post-active EAO stage.　These results indicate 
that TGC death of TGC-induced EAO is mediated by 
Fas/Fas-L and Bax/Bcl-2 systems at the active EAO 
stage but not at the post-active EAO stage.

合成レチノイド Am80による C. albicans water 
soluble fraction誘導血管炎の抑制効果の検討

　レチノイドは、レチノイン酸受容体に結合し、生
理活性を示す化合物の総称である。All-trans retinoic 
acid （ATRA）は急性前骨髄球性白血病の治療薬とし
て用いられる一方、Th1、Th17分化を抑制するなど
の免疫調節作用が知られている。Am80は合成レチ
ノイドの一種で、ATRAより安定性が高く副作用が
少ないという特徴があり、炎症性疾患の治療薬とし
ての可能性も検討されている。 一方、血管炎治療
は免疫抑制薬や副腎皮質ステロイド投与が中心であ
るが、治療抵抗例や感染症等の副作用が問題となる
場合があり、より有効で安全な治療法の開発が求め
られている。我々は、CAWS （C. albicans water solu-
ble fraction）の投与によりマウスの冠動脈に血管炎
を惹起するモデルを用いて、合成レチノイド Am80
による血管炎抑制効果を解析した。Balb/cマウスに
CAWSを 5日間腹腔内投与し血管炎を誘発したマ
ウスに、Am80を経口投与し、病理学的に血管炎発
症抑制及び治療効果を検討した。さらに、血管炎の
病態に重要な役割を担うと考えられる好中球、血管
内皮細胞に対する Am80の作用を in vitroで検討し
た。CAWS投与開始時と同時に、または CAWS投
与開始 1週後（血管炎発症後）より、Am80 4 mg/
kgを経口投与し、6週目に冠動脈を観察したところ、
ともに病理学的に血管炎の程度を表す血管炎スコア
の抑制効果を認めた。またヒト末梢血好中球は、
