視鏡下穿刺吸引生検術（EUS－FNAB）により簡便に
膵生検が可能となった。今回、EUS－FNABにて採取し
た膵癌組織を用いてMUCl、　MU2発現について染色
性ついて検討した。
【方法】対象は当科にて診断され、化学療法施行前に
EUS－FNABを行った切除不能膵癌20例で、全例ヘマ
トキシリン・エオジン（HE）標本にて組織学的に悪性
と診断されている。ホルマリン固定パラフィン包埋し
た生検材料に対して、HE染色とともに抗MUCl抗
体、抗MUC2抗体を用いてSAB法にて免疫染色を
行った。検討項目ははじめに各種画像検査より通常型
浸潤性膵癌かIPMT由来の浸潤癌なのかを診断し、そ
れぞれの診断と得られた生検組織とのMUC1および
MUC2発現との関係を検討した。
【結果】今回の症例は画像診断上は全て通常型膵癌
であった。20例中MACl陽［生、　MAC2陰性であった
のは15例（75％）でMAC2陽性は2例（10％）で
MAClは陰1生であった。MACl、MAC2ともに陰1生は
3例（15％）であった。
【結論】EUS－FNAB材料を用いたムチンコア蛋白発
現の検討は得られた組織が少量であっても上皮成分
が得られれば可能であった。密なる検討も必要と考え
られるが、従来、通常型浸潤性膵管癌と考えられてい
た切除不能膵癌の中にはIPMT由来の浸潤癌も少な
からず含まれ、こうした違いをムチンコア蛋白発現に
より診断できる可能性もある。

Myofibroblasts　eorrelates　with　lymphatic　microves－
sel　density　and　lymph　node　metastasis　in　early－
stage　invasive　colorectal　carcinoma

Background：　Recent　studies　have　shown　that　the　inter－
actions　between　tumor　cells　and　stromal　cells　are　impor－
tant　in　the　development　of　tumor．　We　therefore　inves－
tigated　whether　there　is　a　correlation　between　tumor－
activated　myofibroblasts，　the　main　component　cells　of
tumor　stroma，　and　lymphatic　microvessel　density
（LMVD）　or　other　clinical　parameters　in　carcinoma．
Materials　and　Methods：　lmmunohistochemical　exami－
nation　of　alpha－smooth　muscle　action　and　podoplanin
were　performed　in　83　cases　of　early－stage　invasive　color－
ectal　carcinoma．
　Results：　There　was　a　good　correlation　between　pro－
liferation　of　myofibroblasts　（PMpt）　and　LMVD　（L－
MVDpt）　in　peri－tumoral　area．　（p＝O．oo34）．　lncreased
PMpt　was　also　associated　with　lymphatic　invasion　（P＝
O．oo51）　and　with　lymph　node　metastasis　（p＝　O．Oll）．
However　proliferation　of　myofibroblasts　in　intra－
tumoral　（PMit）　areas　was　not　associated　with　these
clinical　parameters．
　Conclusion：　Proliferation　of　myofibroblasts　in　peri－
tumoral　areas　seems　to　play　an　important　role　in
lymphangiogenesis，　and　is　also　associated　with　lymph
node　metastasis．

Aberrant　promoter　methylation　and　histone
deacetylation　of　the　Cyclooxygenase－2（COX－2）

Background：　lmmunostaining　studies　indicate　frequent
up　regulation　of　COX－2　expression　in　NSCLC　that　is
related　to　increased　invasiveness　and　LN　metastases．　By
contrast，　expression　in　SCLC　is　weak　or　absent．　We
studied　the　mechanism　for　the　differential　expression　of
COX－2　gene　in　lung　cancer　cell　lines．
Methods：COX－2　mRNA　expression　assays　were
examined　by　semi－quantitative　real－time　RT－PCR．
Aberrant　methylation　of　the　promoter　region　of　COX－2
was　studied by　methylation　specific　polymerase　chain
reaction（MSP）．　The　effects　of　the　demethylating　agent
5’一aza－2－deoxycytidine　（5Aza）　or　a　histone　deacetylase
inhibitor，　trichostatin　A　（TSA）　were　tested　in　expression
egative　c ll　lines．
Result ：COX－2　expression　was　present　in　all　18
NSCLC　cell　lines　tested．　However，　expression　was

weak　or　absent　in　16　（73％）　of　22　SCLC　cell　lines．
Compared　to　normal　bronchial　epithelial　cells，　mean
RNA　expression　in　the　NSCLC　lines　was　7　fold　in－
creased　in　NSCLC　lines　and　10　fold　decreased　in　SCLC
lines．　By　MSP　assay，　methylation　was　absent　in　all　of
the　NSCLC　cell　lines　but　was　present　in　33％　of　the
SCLC　lines．　ln　SCLC　cell　lines　three　patterns　were
identified：　Group　1　consisted　of　expression　negative，
MSP　positive　lines；　Group　2　consisted　of　expression
negative，　MSP　negative　lines；　and　Group　3　consisted　of
expression　positive，　methylation　negative　lines．　ln
methylation　positive　Group　1　and　2，　expression　were
restored　after　treatment　with　5Aza　or　TSA　restored
expresslon．
Conclusions：　COX－2　expression　is　down　regulated　in
all　SCLC　cell　lines　examined　compared　to　NSCLC　cell
lines　and　bronchial　epithelial　cells；　and　the　mechanisms
of　lack　of　expression　were　due　to　aberrant　methylation，
histone　deacetylation　or　by　a　combination　of　both
epigenetic　phenomena．　Our　studies　indicate　a　highly
unusual　dichotomy　of　COX－2　gene　expression　一　up
regulation　in　NSCLC　and　down　regulation　in　SCLC．

胃上皮細胞での皿，一8発現に対するCOX－2・
PGE2　pathwayの関与についての検討

【背景・目的】　胃の炎症においてInterleukin－8（IL－8）
は大きく関与している。特にHelicobαcter　pylori（HP）
感染における胃の炎症において、IL－8の関与が強く指
摘されている。一方、HP感染を含める胃の炎症におい
て、Cyclooxygenase－2　（COX－2）・ProstaglandinE2
（PGE2）pathwayの関与も指摘されている。胃上皮細
胞でのIL－8発現へのCOX－2・PGE2　pathwayの関与に
ついては明らかにされていない。
【方法】　胃癌細胞株MKN45およびヒト線維芽細胞
をHP標準株NTCTl1637と共培養し、それぞれの細
胞でのIL－8、　COX－2　mRNA発現およびIL－8分泌量を
検討した。これらの共培養時にCOX－2選択的阻害剤
（NS398）で前処置を行いIL－8発現量を比較した。ま
た、MKN45におけるPGE2受容体（EP　I、2、3、4）
mRNAの発現についてもHPおよびPGE2刺激前後
で検討した。更にMKN45を濃度別のアラキドン酸お
よびPGE2で刺激し、　IL－8の発現の変化を比較した。
【結果】MKN45はHP共培養下でIL－8、　COX－2発現
量が有意に増加し、NS398前処置でIL－8発現は抑制
された。MKN45はPGE2受容体EP2、4を発現してお
り、この発現はHP刺激およびPGE2添加による変化
を示さなかった。MKN45はPGE2およびアラキドン
酸付加によりIL－8産生が増加した。一方、線維芽細胞
は恒常的にIL－8発現が強く、HP共存およびNS398
前処置により変化は認められなかった。
【結論】胃上皮細胞におけるIL－8発現へのCOX－2・
PGE2　p hwayの関与が示唆された。

胆道疾患に対する内視鏡治療におけるRendez－
vous　techniqueの有用性の検討

　Rendezvous　techniqueとは経乳頭的な胆管または胆
嚢内へのカニュレーションが乳頭や胆管の条件など
で不可能であった場合、経皮経論的な胆嚢、胆管への
アプローチを組み合わせて施行する方法である。今回
我々はRendezvous　techniqueにて胆管、胆嚢ドレナー
ジを施行した症例について有用性と問題点について
検討したので報告する。
【対象】　2001年から2005年3月まで当院にて経乳頭
的ドレナージを施行した症例は37例、経皮経肝的に
胆管、胆嚢ドレナージを施行した症例は22例であっ
た。そのうち、経乳頭的アプローチを試みるも不可能
で経皮経肝的アプローチに変更し、Rendezvous　tech－
nique て経乳頭的にESTや胆管stentを施行した症
