대한간학회지제 7 권 제 1 호 2001 ; 109 - 111BRIEF HISTORY68세 남자 환자가 복부 종괴를 주소로 내원하였다. 20년 전 만성 B형 간염으로 진단 받았으며, 10년 전 간의 결절이 관찰되어 방사선 및 세침흡인검사 결과 이형성 결절이 의심되었으나 환자가 치료를 거부한 과거력이 있다. 이후 환자는 정기적인검사를 받지 않았다. 혈청 검사 상 AST/ ALT128/ 39 IU/ L, alkaline phosphatase 38 IU/ L, totalbilirubin 1.3 mg/ dL, fetoprotein 27.5 IU/ mL,HBsAg (+), Anti- HBs (- )였다. 간엽절제술을 시행하였다.GROSS FINDING절제된 간조직은 14×10×9 cm , 610 gm이었으며, 절단면에서 6개의 병변이 비교적 뚜렷하게 관찰되었다(사진 1). 가장 큰 종괴(a )는 장경 5 cm으로 경계가 뚜렷하며 팽창형의 육안소견을 보였다.두 번째로 큰 병변은 장경 3 cm으로 2개의 팽창형종괴(b , c)가 관찰되었으며, 2 cm 크기의 팽창성종괴(f)도 관찰되었다. 그 밖 작은 크기의 병변으로서 1.5 cm 크기의 결절(e)과 1.2 m 크기의 결절(d)이 관찰되었다. 주위 간조직은 간경변의 소견을 보였으며, 주변 간 조직의 재생결절과 비슷한 거대재생결절이 소수 관찰되었다.MICROSCOPIC FEATURE10년 전 시행한 세침 흡인의 소견은 세포밀도가다소 높으며, 간세포를 닮은 세포들이 얇은 육주형 배열을 하고 있었다. 핵대 세포질의 비가다소 증가되어 있었으나, 간세포암종 (hepat o-cellular carcinoma, HCC)의 진단에는 부족한소견이었다(사진 2).병변 a, b , c, f 는 분화가 좋은 HCC의 소견을보였다. 병변 a, c, f 는 주로 1- 2층의 세포로 이루어진 육주형으로 간혹 위선형의 구조가 관찰되었고, 지방변화가 있었다. 병변 b 는 핵대 세포질의 비가 증가된 종양세포들이 주로 육주형배열을 보였다(사진 3).병변 d 는 이형성 결절 저등도의 소견을 보였으며, 병변 e 는 이형성 병변 고등도의 소견을보였다.DIAGNOS ISMulticentr ic occurren ces of hepat ocellular car -cinom a간병리(5)간세포암종의 다중심성 발생연세대학교 의과대학 병리학교실박영년 ·박찬일Multicentric Occurrences of Hepatocellular CarcinomaYoung Nyun Park, M.D., Chanil Park, M.D.Department of Pathology, Yonsei University College of MedicineKey Words : Neoplasm/ Liver/ Hepatocellular carcinoma, Multicentric occurrences109The Korean Journal of Hepatology : Vol. 7. T he cell density is slightly increased (Papanicolaou).전이된 병변을 확진하기는 어려우나, 적어도 다음과 같은 소견을 보이는 병변은 거의 대부분 전이를 하지 않으므로 그 위치에서 발생하여 증식한 것으로 간주할 수 있다. 이러한 병변에는 이형성 결절에서 분화가 매우 좋은 HCC의 병소가발생된 경우, 분화가 매우 좋은 HCC, 분화가 매우 좋은 HCC에서 분화가 좀더 나쁜 HCC 결절이 발생된 경우가 있다. 따라서 2개 이상의 병변이 관찰될 때 주 병변 외의 병변이 위와 같은병리소견을 보이는 경우는 다중심성 HCC로 간주할 수 있다.1,2 또한 이러한 소견을 보이는 병변들이 다발성 기원임이 분자생물학적 연구보고들에 의하여 밝혀졌다.3최근 간이식시 적출된 146개의 간 조직에 대한연구 보고에 의하면 55%에서 HCC가 다발성으로 발생되었으며, 그 중 30%에서는 이형성 결절에서 발생된 HCC 병소(carcinoma in s itu)가 관찰되었다.4 또한 이형성 결절이 발생된 간경변30예에서 대한 연구에서는 이형성이 다발성으로발생된 경우가 33%이었으며, 이형성 결절의 수는 평균 3.5개였다. 그 중 60%의 이형성 결절에서 HCC의 발생이 관찰되었고, 80%에서 HCC가동반되어 있었다.5 ,6 따라서 조기 간암발생단계에서도 적지 않은 수가 다발성 발생을 보일 수110Young Nyun Park, et al.
