대한소화기학회지 2007;49:267-269 □ IMAGE OF THE MONTH□증례: 43세 남자 환자가 내원 1주 전부터 시작된 발열을주소로 내원하였다. 과거력과 가족력에서 특이 소견은 없었다. 내원 당시는 급성병색을 보였으나, 의식은 명료하였고발열, 오한, 우하복통을 호소하였다. 그 밖에 기침, 객담, 흉통, 심계항진, 호흡곤란, 설사 등의 증상은 없었다. 신체검사에서 혈압 110/60 mmHg, 맥박수 95회/분, 호흡수 20회/분,체온 38.0oC였다. 피부 감촉은 정상이었으며, 흉부 청진에서수포음이나 심잡음은 청진되지 않았다. 우하복부에 압통과반발통이 있었으며 간, 비장, 신장과 기타 복부에 촉지되는종괴는 없었다. 말초혈액검사에서 백혈구 9,000/mm3 (중성구분획 80.1%), 혈색소 14.0 g/dL, 혈소판 113,000/mm3였다.혈청생화학검사에서 공복 혈당 102 mg/dL, Na/K/Cl/tCO2137/3.9/99/23 mmol/L, AST/ALT 80/100 IU/L, 총 빌리루빈4.1 mg/dL, alkaline phosphatase 216 IU/L, BUN 27.4 mg/dL,creatinine 0.8 mg/dL, 아밀라아제 32 IU/L, 리파아제 32 U/L,총 단백 5.7 g/dL, 알부민 3.7 g/dL, CEA 0.65 ng/mL, CA19-9 12 U/mL이었다. 소변 검사에서 이상소견은 없었고, 흉부 단순방사선 검사 및 복부 단순방사선 검사에서도 이상소견은 없었다. 복부 통증의 원인 감별을 위해 복부 전산화단층촬영을 시행하였다. 진단은?연락처: 이상길, 120-752, 서울시 서대문구 신촌동 134번지연세대학교 의과대학 내과학교실Tel: (02) 2228-1991, Fax: (02) 393-6884E-mail: sklee@yumc.yonsei.ac.krCorrespondence to: Sang Kil Lee, M.D.Department of Internal Medicine, Institute of GastroenterologyYonsei University College of Medicine, 134, Sinchon-dong, Seo-daemun-gu, Seoul 120-752, KoreaTel: + 82-2-2228-1991, Fax: + 82-2-393-6884E-mail: sklee@yumc.yonsei.ac.kr급성충수염에 동반한 상장간정맥 혈전정맥염연세대학교 의과대학 내과학교실, 소화기병연구소김범경·이상길A Case of Thrombophlebitis in Superior Mesenteric Vein due to AppendicitisBeom Kyung Kim, M.D. and Sang Kil Lee, M.D.Department of Internal Medicine, Institute of Gastroenterology, Yonsei University College of Medicine, Seoul, KoreaFig. 1. CT scan shows a filling defect (thrombus) in superior me-senteric vein.268 대한소화기학회지: 제49권 제5호, 2007진단: 급성충수염에 동반한 상장간정맥 혈전정맥염복부 전산화단층촬영에서 체액으로 충만한 충수와 충수주변 염증 소견이 관찰되었다(Fig. 1). 또한 상장간정맥과 그가지에 혈전이 관찰되었으며, 동맥기에서 간 실질에 불균등한 조영증강이 보였다(Fig. 2, 3). 이상의 영상의학 소견으로급성충수염에 속발한 상장간정맥 혈전정맥염과 간염으로진단하였다. 내원 후 환자는 경험적인 광범위 항생제(cefo-taxime 1g t.i.d., amikacin 500 mg q.d., metronidazone 500 mgt.i.d.)를 경정맥 투여 받았으며, 동시에 heparin을 이용한 항응고요법을 받았다. 입원하여 시행한 혈액 세균 배양 검사에서 동정된 균주는 없었다. 내원 10일째 복강경하 충수절제술을 시행받고, 이후 warfarin으로 항응고요법을 현재까지 지속하고 있다. 퇴원 1개월 후 시행한 복부 전산화단층촬영에서 상장간정맥 혈전은 더 이상 관찰되지 않았으며,환자는 현재까지 별 다른 증상 없이 외래 경과 관찰 중이다(Fig. 4).간 문맥이나 상장간정맥의 패혈 혈전정맥염은 매우 드문질환으로, 일반적으로 문맥순환계통 감염으로 발생하며, 즉각적인 항생제 치료를 시작하지 않을 경우 사망률이 높게는30%까지 보고된다.1,2 특히 충수나 회장의 정맥계는 간 문맥으로 연결되기 때문에 복부 또는 골반부 염증이 혈전정맥염외에도, 추후에 다수의 간 농양 등을 일으키기도 한다.1,3,4가장 흔한 원인 질환은 게실염이며, 그 밖에 충수염, 췌장염, 담낭염, 골반감염 등이 있다. 드물게 응고인자 결핍으로발생하는 특발 혈전정맥염도 있다.4,5 복강 내 감염으로부터혈전정맥염이 발생하는 기전은 정확히 알려져 있지 않지만,감염 물질과 혈관 내피세포 간의 상호작용이 혈액 내 응고과정을 촉진하여 혈전을 형성하며, 특히 혐기 세균 감염은fibrin 형성과 heparin 분해를 촉진시켜 혈전을 더 잘 생성한다.6 패혈 혈전정맥염은 전형적인 증상이 없기 때문에 진단자체가 지연되는 경우가 많으며, 임상적으로 발열, 복통, 오심, 황달 등의 비특이 증상이 동반된다. 혈액 배양 검사에서세균이 동정되는 경우가 많으며, 흔한 균주로는 Bacteroidesfragilis, Eschericia coli, Proteus mirabilis, Klebsiella pneu-moniae, Enterobacter species 등이 있고, 드물게 Salmonella,Campylobacter, Yersinia 등에 의한 혈전정맥염이 있다.1,2 진단은 일반적으로 환자의 증상과 전산화단층촬영과 같은 영상의학 방법으로 가능하며, 이를 통해 감염 부위와 이와 동반된 혈전까지 비교적 쉽게 확인할 수 있다.6 그 밖에 초음파나도플러 검사, 그리고 자기공명영상도 진단에 유용하다.7패혈 혈전정맥염을 의심하는 경우 즉각적으로 광범위 항생제 투여를 시작하고, 감염 부위 확인 후 수술 또는 그 외의 방법으로 제거하야 한다. 혈전에 대한 항응고요법의 효능에 대해서는 아직 이견이 있으나, Condat 등8은 항응고요법을 통하여 90% 이상에서 혈류가 재개통된다고 하였다.1,6일반적으로 항응고요법은 내장의 허혈상태를 방지하고, 혈류를 유지하기 위해 권장한다.8,9 만약 적절한 항응고요법에도 불구하고 장의 허혈상태가 지속될 경우, tissue plasmi-nogen activator 등의 혈전용해제를 카테터를 통해 주입하기도 하며, 외과적으로 혈전제거술을 병행할 수 있다.9이번 증례는 우하복부통과 발열로 급성충수염에 동반한패혈 혈전정맥염으로 진단받은 환자가 충수절제술과 더불어 경험적인 항생제 투여와 항응고요법 유지로 성공적으로치료한 예이다. 급성충수염과 동반된 혈전정맥염은 드문 질환이나, 신속한 진단과 함께 즉각적인 항생제 및 항응고요법이 시작되어야 사망률과 이환율을 감소시킬 수 있다.Fig. 3.
