It commonly develops when there is disruption of the lymphatic system due to traumatic injury or obstruction. Chylous ascites is diagnosed when ascetic fluid appears milky and contains a triglyceride content above 200 mg/dl. Patients identified as suffering with chylous ascites are treated to correct the specific underlying cause. Therapeutic paracentesis and administration of diuretics are recommended for patients with underlying cirrhosis. Appropriate nutritional support for chylous ascites, including a low-fat diet and medium-chain triglyceride supplementation, is of paramount importance. (SMN 2011;2:39-40)Key Words: Chyle, Management, DiagnosisChyle Leaks   유미 누출(chyle leak)은 복부, 흉부, 경부 수술이나 사고로 인한 림프관의 손상으로 나타날 수 있는 합병증으로 수술 후 약 1%에서 발생하는 것으로 알려져 있다. 복강 내 유미 누출 발생시 치료 방법으로는 저지방 식이 섭취, 경정맥 영양, 금식, octreotide의 사용과 수술 등을 시행하나 아직까지 명확한 치료의 가이드라인이 없는 실정이다. 저자들은 복강내 유미 누출의 치료 방침에 대하여 문헌 고찰을 하고자 한다. 림프관의 해부 및 생리   흉관은 몸에서 제일 큰 림프관이며 복강내 유미조(cisterna chyli)에서 출발하여 대동맥 열공(aortic hiatus)를 통하여 흉곽으로 들어가서 대동맥과 기정맥 사이로 흉추의 오른쪽으로 주행하다가 제5, 6, 7흉추 부위에서 왼쪽으로 주행한 후에 좌측 쇄골하 정맥까지 종격동을 통과하게 된다.(1)  유미액의 구성성분은 대부분이 중성 지방(triglyceride)인 지방(1∼3%)과 단백질(3%)로 구성되어 있으며 하루에 2,000 ml에서 4,000 ml가 배액된다.(2) 유미액의 배출액은 활동 정도, 장기능, 식이의 지방 함유량 등에 따라 다르며, 금식 시에는 배출량은 시간당 10 ml까지 줄어드나 고지방 식이를 한 직후에는 유미 배출이 늘어나게 된다.(3) 지방 중에서 long-chain 중성 지방은 소장에서 흡수 후 림프관으로 이동되지만, me-dium-chain 중성 지방은 림프계를 통하지 않고 바로 문맥으로 들어가게 된다.(3) 원  인  유미 복수(chyle ascites)의 원인은 다양하다. 서양에서 악성 종양과 간부전이 전체 2/3를 차지하고 있으나 동양에서는 결핵과 같은 감염성 질환이 많은 원인을 차지하고 있다. 그 외에도 많은 원인이 있다(Table 1).40 ❚ Surgical Metabolism and Nutrition Vol. 2 No. 2, 2011Table 1. Characteristics of chyle ascitesColor Milky and cloudy  Triglyceride level Above 200 mg/dl Cell count Above 500 (predominance of lymphocytes)  Total protein Between 2.5∼7.0 g/dlCholesterol Low (ascites/serum ratio ＜1)임상 증상 및 진단  유미 복수의 증상은 다양하나 주로 무통성의 지속적인 복부 팽만이 주요 증상이며, 수술이 원인이 된 경우에는 갑작스런 복부 팽만을 호소한다. 그리고 체중 증가, 빈호흡, 호흡 곤란, 조기 팽만감, 설사, 지방변, 영양 실조와 부종등이 동반되기도 한다.      유미 복수를 진단하는데 가장 중요한 진단 방법은 복수 천자이다. 유미 삼출액은 특징적으로 우유빛 색깔의 냄새가 없는 알칼리성 액이다. 복수액은 결핵등과 같은 감염성 질환과 감별하기 위하여 세포 배양, 그람 염색등을 보내야 한다. 유미 삼출액 진단에서 가장 중요한 것은 중성 지방(triglyceride)의 농도이며 200 mg/dl 이상 시 확진할 수 있으며 그 외 유미 복수의 특징적 소견은 다음과 같다(Table 2).치  료  유미 삼출시 처음에는 보존적 치료가 우선된다. 대부분의 유미 삼출은 고단백과 mediun-chain 중성 지방이 함유된 저지방 식이가 초기 치료에 추천이 되나, 이러한 식이제한으로 호전이 되지 않으면 경정맥 영양과 함께 금식이 이뤄진다.(4)  하지만, 이러한 보존적 치료로 호전되지 않을 경우에는 수술이 고려되기도 한다. 수술의 적응증은 5∼7일 이상 하루에 1,500 ml 이상의 유미 삼출이 있을 때이며, 수술 전 림프관 조영술로 누공과 막힌 곳을 찾는 것이 도움이 되기도 한다. 결  론  결론적으로, 유미 복수는 상대적 드물지만 종종 경험하는 합병증 중의 하나이다. 유미 복수는 예전에는 영양 결핍과 면역학적 문제로 사망률이 높았으나 최근에는 적절한 영양 공급으로 사망률이 많이 낮아졌다. 유미복수의 치료 방법은 저지방식이 등과 같은 보존적 치료로 잘 치료가 되나, 오랫동안 지속될 경우에는 수술적 치료가 필요하게 된다.
