If you will have these filled out as completely as possible ., and returned to us., we will take the further steps nec-essary for admittance to our home. It will be necessary for each of these children to have a physical examination before entering our home. A form should be filled out and signed by a Doctor in your locality for each of these children. Their physical fitness and any physical defonni ties should be listed and this form re-turned to us along with the completed application forms.
