AFFIDAVIT OF POVERTY AND MOTION TO PROCEED IN FORMA 
PAUPERIS IN HABEAS CORPUS PROCEEDINGS: 
Respectfully represents your petitioner, Samuel H. Sheppard, as fol-
lows: 
I. He is a citizen of the United States, and is presently incarcerated 
in the Ohio State Penitentiary in Columbus serving a sentence of life im-
prisonment. 
2. He is the petitioner in the above-entitled action, wherein he claims 
that his liberty is restrained by the respondent in violation of the Unite_d 
States Constitution, and he seeks release from said restraint; your petitioner 
honestly and verily believes that he is entitled to the relief s;:mght in this 
action. 
3. Your petitioner is insolvent, and owns no property, real or personal 
which he might use to secure payment of the fees and costs of this action; 
he makes this affidavit for the purpose of availing himself of the rights and 
privileges afforded indigents under Title 28 U.S. C., § 1915. 
4. Petitioner avers that unless he is permitted to proceed in the man-
ner provided by said statute, he will be unable to avail himself of his con-
. stitutional rights.
